The construction of all doors and door frames in "A" class divisions with the means of securing them when closed, shall provide resistance to fire as well as smoke and flame, as far as practicable, equivalent to that of the bulkheads in which the doors are situated.
PSCO observed that three doors in class "A" divisions/boundary type bulkheads had damage or defect which may allow for passage of smoke or flame. Doors were located at separation of steering gear room and passageway, main engine room and port-side passageway, and galley to stores on port side.

Cargo operations in cargo hold 3 are restricted until vsl's classification society provides a satisfactory report to the USCG attesting to the structural integrity of cargo hold 3.

Lifesaving › Inflatable Liferafts › Painter

On cargo ships, every liferaft shall be stowed with its painter permenantly attached to the ship and with a float-free arrangement so that the liferaft floats free and inflated automatically when the ship sinks. PSCO found the painters on both port and starboard liferafts not arranged in a manner that would allow the liferaft to float free in an emergency. 74SOLAS(97)III/29
