Propulsion machinery shall be suitable in type and design for propulsion requirements. MI identified exhaust leak coming from Port MDE #6 and #7 cylinders. Repair to the satisfaction of the attending [name withheld] 46 CFR 182.05-1(a)

AIS Class A device. The following vessels must have on board a properly installed, operational Coast Guard type-approved AIS Class A device: (i) A self-propelled vessel of 65 feet or more in length, engaged in commercial service. Vessels installed AIS receives but does not transmit. Prove proper operation of AIS transmission. 33CFR164.46(b) 16c

Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. If the storage cylinders are separate from the protected space, the instructions must also include a schematic diagram of the system and instructions detailing alternate methods of releasing the extinguishing agent should the local manual release or stop valve controls fail to operate. Local controls for CO2 release not marked with instructions.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible. Vessel does not have up to date charts. 46 CFR 184.420

Cooking and heating equipment must be suitable for marine use. Equipment designed and installed in accordance with American Boat and Yacht Council (ABYC) A-3, “Galley Stoves,” and A-7, “Boat Heating Systems,” or with National Fire Protection Association (NFPA) 302, “Pleasure and Commercial Motor Craft,” complies with this requirement. Following the electrical fire that occurred inside the galley oven: Repair/replace galley oven and have attending MI verify installation and operation prior to use while carrying passengers for hire. Vessel may also remove and discontinue use of any cooking equipment. While passengers are carried onboard, vessel is restricted from operating cooking equipment and shall lock out/tag out electrical source of power for such equipment at panel if hardwired into electrical circuit.

Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. While mooring the vessel lost the stbd main propulsion engine. Vessel was able to dock safely. Vessel must prove proper operation of the stbd main propulsion engine prior to movement. 46 CFR 182.05-1

Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions. DC generator failed resulting in a loss of propulsion. Vessel operator to prove satisfactory condition (i.e. fitness for service) of the DC generator. DC generator is not to be utilized while passengers are onboard and/or while operating under the vessels Certificate of Inspection. To rectify this deficiency vessel operator must demonstrate proper function/operation of the DC generator to the assigned USCG marine inspector.

A VESSEL MUST BE OPERATED IN ACCORDANCE WITH APPLICABLE LAWS AND REGULATIONS AND IN SUCH A MANNER AS TO AFFORD ADEQUATE PRECAUTION AGAINST HAZARDS THAT MIGHT ENDANGER THE VESSEL AND THE PERSONS BEING TRANSPORTED. DISCOVERED ROTTED WOOD ON 03 STRUCTURAL MEMBERS SUPPORTING APPROXIMATELY 15' OF FREE STANDING PORTION OF THIRD DECK. CONDUCT DAMAGE ASSESSMENT AND PROVIDE SATISFACTORY REPAIR PROPOSAL.

Install bell of at least 300 mm in diameter.

Replace hull planking on port quarter; as indicated.

Sand/grind 2 areas of hull planing on the port quarter; as indicated, to enable inspection.

Replace frame on stbd side in hot water tank space; as indicated.

Replace frame on stbd side fwd in day tank space; as indicated.

Replace frames (05) total stbd side laz; as indicated.

Replace planking on transom; as indicated ensuring proper spacing & location of butts; as indicated.

Crop to good wood frame & planking stbd side aft engine room; as indicated.

Crop, renew & treat areas of delignification &/or wood rot in the following areas; as indicated. chain locker port side, bow thruster fwd stbd side, bow thruster aft stbd side, hot water tank space stbd side, dish washer space stbd side.

Sand/grind on external of hull to good wood as indicated, in order to identify rotted planking.

Remove epoxy and filled hull planking in order to make identified areas on external of hull available for inspection.

Provide USCG a list of sea valves, size, types & locations.

Provide USCG with a list of all frames renewed or repaired, with location.

Document of Compliance had not been renewed as the late fee had not been paid. The Hornblower Company had paid to renew the DOC, but they were late and NVDC would not process renew the DOC until a late fee had been submitted.

Identify source and cause of stack fire in starboard muffler. Vessel is prohibited from operating with passengers for hire until corrected.

Prove proper operation of starboard engine. Vessel is prohibited from carrying passengers for hire until corrected.

Found #1 relief valve stuck open when tested. Issued CG-835 to repair/replace valve.

MIS form not submitted by March 15 as required by 46 CFR part 16.500.

No copy of contract or proof of enrollment in Consortium as required by 46 CFR 16.

Provide strainer bowls for all fuel filters IAW 46 CFR 182.455(b)(6).

Clean port and starboard main engine exhaust pipes from the exhaust manifolds to the top of the stacks. The exhaust interior is coated with excessive oil and the vessel suffered a stack fire. Must be completed prior to carrying passengers.

Remove hard wired portable extension cord from electrical service panel.

PROVIDE OPERATING INSTRUCTIONS FOR FIX FIREFIGHTING SYSTEM.

Provide primary lifesaving for 50%/83 of the total persons allowed on COI of 165. Vessel is restricted to carrying a maximum of 160 total persons until this requirement is cleared to the satisfaction of the attending marine inspector.
