All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Bolt on steering arm vibrated free causing partial loss of steering. Provide permanent repairs to steering system.

Resolution: Steering rod reconnected with lock washers. Having new rudder bearing made to prevent vibrations.
All vessels shall be provided with suitable steering apparatus. Vessel reported steering apparatus failed while underway. Submit written repair proposal for steering failure. Repairs must be completed to satisfaction of CG marine inspectors.
