Vessel's emergency lighting did not function in various locations throughout the vessel to include: officers galley, escape trunk outside of engine room and all lifesaving appliance embarkation areas.

The Administration shall be satisfied that adequate arrangements are in place to ensure that the performance of equipment in chapter V is maintained. Various navigation light bulbs are burned out with no spares available on-board the vessel.

Each operation described in paragraph 2 of this regulation shall be fully recorded without delay in the Oil Record Book Part I. Each completed operation shall be signed by the officer or officer in charge of the operations concerned and each completed page shall be signed by the master of ship. Vessels ORB has 6 pages that were not being signed by the master of ship.

Each completed page of the Garbage Record Book shall be signed by the master of ship. Vessel’s Garbage Record Book most current completed page was not signed.

A certificate called a Cargo Ship Safety Certificate may be issued after an initial or renewal survey to a cargo ship that complies with the relevant requirements. Vessel's Form E is showing an incorrect number of liferafts listed. Prior to sailing foreign vessel must have a correct CSSE Certificate on board.

Structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these divisions shall be kept in good order so as to ensure their required performance if a fire occurs. In both the LP CO2 Room and the emergency generator room the vessel is utilizing two different penetration materials for a single cable run.

Accumulator batteries shall be suitably housed, and compartments used primarily for their accommodation shall be properly constructed and efficiently ventilated. Vessel’s battery box for the GMDSS equipment is rusted out with various holes on the bottom edges.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations and to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Vessel has two portable accommodation modules (PAM). Both have multiple holes in modules completely through to the deck. Furniture is not secured within and one head is out of commission. PAM’s can not be occupied by personnel until repaired.

All lifeboats shall be monthly turned out from their stowed position, without any persons on board if weather and sea conditions so allow. Vessel is moored stbd-side-to and unable to lower stbd lifeboat. Prior to activation vessel must exercise lifeboats to the satisfaction of the attending marine inspector.

Fire extinguishing appliances in new and existing cargo ships shall be kept in good order and available for immediate use at all times during the voyage. Vessel’s fire pump would not immediately turn on upon testing. After various attempts vessel was able to start and test fire pump.

Before the ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. Vessel is unable to make any announcements to the entire ship due to a non functioning public address system.

The general principle governing the provision of lifeboats, life rafts and buoyant apparatus in a ship to which this chapter applies is that they shall be readily available in case of emergency. Vessel's STBD lifeboat propeller shaft packing was found sticking and binding, resulting in engine stall. Vessel's crew provided water flow lubrication through the cutlass bearing, causing lifeboat to operate satisfactory, as designed.

The purpose of this Chapter is to require the fullest practicable degree of fire protection, fire detection and fire extinction in ships. The following basic principles underlie the Regulations in this Chapter and are embodied in the Regulations as appropriate, having regard to the type of ships and the potential fire hazard involved: containment and extinction of any fire in the space of origin. After any survey of the ship under Regulations 7, 8, 9 or 10 of this Chapter has been completed, no change shall be made in the structural arrangements, machinery equipment, etc. covered by the survey without the sanction of the Administration. Visual alarm light positioned in the lower fwd level of of the machinery space is out. Push button for fire alarm in fire control room is missing cover glass. Low hydraulic oil light indicator at local steering control station is out. 74 SOLAS (Unamend) I/11

The company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The company shall establish procedures to ensure that the ship is maintained in conformity with the provisions of the company. In meeting these requirements, the company shall ensure that: inspections are held at appropriate intervals, any non-conformity is reported, with its possible cause, if known, appropriate corrective action is taken, records of these activities are maintained. PSTP's state that the SSDG's are to shutdown upon the jacket water reaching 92.3 degrees Celsius. While using dry-well to heat JW shutdown RTD's, shutdown failed to activate for all 3 SSDG's. 74 SOLAS (14) IX/3.1 ISM Code Part A/10.1 + 10.2

Every crew member shall participate in at least one abandon ship and one fire drill every month and a drill shall take place within 24 hours for a 25% crew change. Conduct abandon ship and fire drill in the presence of CG MI's. 74 SOLAS (14) III/19.3.2

There is no power to the bridge wings for the signaling lamp. The lamp can only be utilized on the battery pack.

At each inspection for certification and periodic inspection, the marine inspector shall conduct such tests and inspections of the main propulsion and auxiliary machinery and its associated equipment, including the fluid control systems, as he feels necessary to check safe operations. Technical publications for the completion of machinery testing were not available onboard the vessel, and such testing could not be completed.

Complete ABS listed conditions of class # 1015, 1010, 1007, 1004, 1003 prior to continuation of CG inspection.

Fire extinguishing appliances in new and existing passenger ships and cargo ships shall be kept in good order and available for immediate use at all times during the voyage. Bridge fire alarm panel is showing an unexplained "trouble" alarm in Zone 9.

A-60 fire door for the emergency generator space on the starboard side did not close and latch properly.

Every company should develop, implement and maintain a safety management system which includes the following functional requirements: Instructions and procedures to ensure safe operation of ships and protection of the environment in compliance with relevant international and flag state legislation. Mandate external ISM audit based on findings from deficiencies # 1 (lifesaving maintenance program) and 2 (lack of technical publications) listed above.

Oil purifier room needs to be clean of excessive amounts of build up oil on bulkheads and in the bilge; All 835's were given to be done within 30 days after issuing date.
