At each initial and subsequent inspection for certification and reinspection the marine inspector shall inspect all lifesaving equipment to determine its serviceability. Cable for rescue boat davit was observed deteriorated and needs to renewed. Vessel is equipped with a rescue platform. 46 CFR 176.25-20(a) (OLD T)

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. Annual Inspection window lapsed 05MAR2026. Conduct satisfactory annual inspection prior to operations. 46 CFR 176.500(b)(1)

Vessel operator reported experiencing a loss of propulsion that reduced the maneuverability of the vessel. CG-835V to be cleared upon submission of causal analysis, tech report and satisfactory completion of an underway sea trial with an MI present.

Suitable guards, rails, or covers must be installed in hazardous or exposed places, such as gears, machinery, etc. MI identified missing guard on raw water pump. Place cover/ guard around rotating machinery.

Battery chargers must have an ammeter connected in the charging circuit. Identified missing ammeter for 24V amp charger.

The shutoff valve at the tank shall be operable from outside the tank compartment preferably from an accessible position on the weather deck. Fuel tank shut off required for day tank from accessible position on the weather deck.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 19.8 meters (65 feet) in length must also have navigation lights that meet UL 1104, “Standards for Marine Navigation Lights,” or other standard specified by the Commandant. Navigation lights must be approved type. Submit proof of type approval.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Cherts; U.S. Coast Pilot; and Coast Guard Light List. Navigation publications must be electronically downloaded.

Before getting underway on a voyage or as soon as practicable thereafter, the master of a vessel shall ensure that suitable public announcements are made. Vessel must provide a written document of the safety orientation meeting regulation.

All vessel's shall be provided with an approved type life preserver for each person carried. Identified 03 unsatisfactory life jackets. Replace 02 adult life jackets and 01 child life jackets.

Shall keep a correct, written count of all passengers that embark on and disembark from the vessel. Prior to departing on a voyage, the passenger count must be communicated verbally or in writing, and available ashore at the vessel's normal berthing location or with a representative of the owner or managing operator of the vessel. Vessel must provide an established passenger count plan.

All vessels of at least 26 feet in length and over shall be provided with individual bilge lines and suctions for each watertight compartment. MI identified high level bilge alarm in laz not operable.

At each inspection and subsequent inspection for certification the marine inspector shall examine and test each pressure vessel to determine that the condition is satisfactory. Must have Coast Guard Marine Inspector witness testing of the pressure vessel by hydrostatic means per regulation. Safety valves must also be tested.

Drills must be conducted in the presence of Coast Guard Marine Inspector.

All vessel's shall be provided with an approved type life preserver for each person carried. At the time of inspection, vessel was missing 01 life jackets.

Each life preserver carried must have at least 200 sq. cm of retro reflective material attached to its front side, at least 200 sp. cm on its back side, and at least 200 sq. cm of material on its reversible sides. Attending inspector observed multiple life preserves with wasted and inadequate retro reflective material at the time of inspection.

Life preservers shall be distributed through the upper part of the vessel in protected places convenient to the persons on board. Attending inspector observed vessel's child life preservers being stowed on top of the house, not in a convenient location to passengers.

A vessel must carry either a first-aid kit that meets the requirements of 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Vessel's asprin in first aid kit was expired at the time of inspection. Replace with asprin that is not expired.

Vessel's not limited to daytime operation shall be provided with an approved water light. At the time of inspection, vessel's water light attached to the ring buoy was not in operation. Vessel is restricted to daytime operation only until water light is replaced with an approved light.

All vessels of 26 feet in length and over shall be provided with individual bilge lines and bilge suctions for each watertight compartment... Vessel must demonstrate proper operation of engine room bilge suction in the presence of Marine Inspector.

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the EPA under 40 CFR 140.3, the operator must secure each Type III device in a manner which prevents discharge of sewage. Acceptable methods of securing the device include padlocking each valve leading to an overboard discharge in the closed position... Vessel's MSD overboard discharge valve was not restricted by any proper means IAW the cited regulation.

Fixed CO2 fire extinguishing systems shall be installed to protect the following spaces: machinery.. vessel's fixed CO2 system installed to protect the machinery space is inaffective and a hazard to passengers as the machinery space is open to the accommodation space. Seal the machinery space so that it is vapor tight prior to carrying passengers.

The engine head, block, and exhaust manifold shall be water jacketed and cooled by water from a pump which operates whenever the engine is operating... At the time of inspection, vessel's main engine raw water and jacket water pumps were leaking. In addition, a main engine jacket water line was corroded and leaking.

Appropriate for intended voyage a vessel must carry up to date Cherts, Coast Guard Light List, U.S. Coast Pilot; vessel charts and pubs not current and up to date, must provide current and up to date items.

Where at least monthly manual inspections are conducted, the date the manual inspection was performed and the initials of the person performing the inspection shall be recorded; vessel has not conducted monthly checks.

Vessel is required to conduct drills and crew training at least once every 3 months and/or when first being employed on a particular vessel as to the duties expected to perform in an emergency to include logging the date of training and general description of the training topics; vessel crew the day of inspection were new and no training or drills had been conducted, must provide proof of training and drills as per regulations.

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer; crew the day of inspection were not enrolled in the companies Drug & Alcohol program, must provide proof of enrollment for all employees of the vessel.

Marine employer shall keep records of chemical and alcohol testing and make available upon request by the Coast Guard, positive test for 5 years and negative test for 1 year; must provide required test results as per regulation.

The employer shall provide an Employee Assistance Program for all crew members and display informational material including community hot-line telephone number and conduct supervisor training and for crewmembers; vessel did not display required information and show proof that evryone conducted the required training.

All marine employers must submit drug testing program data required by 49 CFR 40.26 and appendix H to 49 CFR part 40 (MIS report); must provide MIS report as required.

Vessel's passenger safety orientation was not conducted during the inspection due to vessel not getting U/W to conduct drills during the inspection, vessel must provide a written document that the safety orientation meets the regulation or conduct it verbally at next inspection.

A hatch exposed to the weather must be watertight. Visual inspection of the vessel indicated most gaskets on watertight doors were wasted, broken, and not in acceptable condition. Replace gaskets and conduct a chalk test and/or hose test to prove watertight integrity.

Provisions must be made to close supply ducts, cowls, or scoops, and exhaust duct for a space protected by a fixed gas extinguishing system, vessel's covers on upper deck did not fit properly and main deck were missing covers, must prove proper fit for all covers.

Rails must be fitted on all weather decks accessible to passengers and crew at the appropriate height; vessels upper deck where child life-jacket and IBA is stored are only 24" and must be 42", must install proper rails.

The intent of this part is to...provide for the safe operation with the terms of its COI. Submit repair proposal and crop/renew areas with more than 25% wastage on gauging report. Area is concentrated on the STBD side shell and highlighted on pg 3 of the gauging report.

Where the internals of the vessel are completely concealed, sections of the lining or ceiling may be removed or the parts otherwise probed or exposed so that the marine inspector may satisfy himself as to the condition of the hull structure. Marine Inspector was not able to visually inspect a satisfactory proportion of the fore and aft accommodation spaces due to concealment from deck plates and equipment stored along bulkhead. Additionally, condition of forward lazarette framing and engine room framing under starboard generator could not be fully evaluated due to heavy scaling and presence of lazarette tank. Expose all mentioned areas for further USCG inspection, prior to credit dry dock. 15JUN2023: Lazarette was descaled, Engine room was descaled under generator, aft acom space deckplates removed. Laz tank not yet removed, but area beneath was adequately inspected visually and with UT guaging. Fwd Acom space still needs to be de-cluttered so deckplates can be removed for bilge inspaction of that space. OLD T 46 CFR 176.25-5(a)(1)

All vessels of 26 feet in length and over shall be provided with individual bilge lines and bilge suctions for each watertight space. Forward void space bilge pipe has small fracture at bulk head penetration preventing proper bilge suction. The officers’ quarters’ bilge suction was unable to drain space under the gray water tank. 46 CFR “OLD T” 182.25-1&5 701 c

All vessels shall be provided with a satisfactory arrangement for draining any compartment. Bilge pipe for the forward most void is leaking in the main accommodation space port side of the watertight bulkhead forward. 46 CFR “OLD T” 182.25-1&5 701 c

A vessel carrying overnight passengers must have a suitable number of watchman. Provide standing orders for watchman during night time/ not underway. 46 CFR 85.410 16 c

Mattress is to be covered with material which has been treated to give it fire resistant properties. Remove all non-treated memory foam mattresses from accommodations. 46 CFR “OLD T” 177.30-7(b) 16 c

Provision shall be made by means of plugs, covers etc. to prevent the admission of air into the space protected. Provide covers for engine room vents. 46 CFR “OLD T” 181.20-35(a) 701 c

All machinery must be installed and maintained in such a manner to protect against fire, flood, explosions, machinery failure and injury. Oil leak discovered on oil pump starboard side of main engine forward. 46 CFR “OLD T” 182.01-5(a) 16 c

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural examination at least once every two years. Vessel is overdue for drydock and internal structure examinations and restricted from carrying more than 6 passengers when at least one passenger is for hire until satisfactory drydock and internal structural examinations have been completed.

Bunk arrangement shall be not more than three high, with a minimum distance of 24 inches vertically between bunks; ladders or steps shall be provided for each top bunk. Current bunk spacing is less than the required 24" for all racks and the top bunks did not have the required ladders or steps. 46 CFR 177.30-7(b) - (OLD T)

At each inspection for certification and reinsertion the marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter which might create a fire hazard. Vessel had excess amount of oil in bilge. 46 CFR 176.25-50(b)

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. Vessel does not have an established program for crewmembers. 46 CFR 16.230(a)

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Breakers in engine room must be labeled. 46 CFR 176.800

Tubular gauge glasses, if fitted to diesel fuel tanks, must be of heat resistant materials, adequately protected from mechanical damage, and provided at the tank connections with devices that will automatically close in the event of rupture of the gauge or gauge lines. The sight glass connected to diesel fuel tank is a tubular plastic material and not adequately protected or shielded from damage. The tank connect did not have an automatically closing device. 46 CFR 182.440(a)(7)

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Emergency instructions should be posted at the operating station. 46 CFR 185.510(a)

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR” Emergency exits signed should be installed in applicable locations with the accommodation spaces. 46 CFR 185.606

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: (1) Spaces storing carbon dioxide - “CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.”. (2) Spaces protected by carbon dioxide - “CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.” The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units. The CO2 storage space and both entrances into the engine room which is protected by CO2 should be labeled with a proper placards. 46 CFR 185.612(g)(1)(2)

Suitable thin heat and fire resistant gasket or suitable compound must be used between the bulkhead and the frame for watertightness. Gaskets on water tight doors need to be replaced. 46 CFR 170.270(d)(2)(i)

Suitable covers guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. A guard must be installed around the rotating machinery. 46 CFR 177.35-15(a)

All vessels of 26 feet in length and over shall be provided with individual bilge lines and bilge suctions for each watertight compartment. Vessel must prove operation of bilge system. 46 CFR 182.25-5(a)

Each life ring buoy on a vessel must be approved in accordance with 160.050 in subchapter Q of this chapter. Grab line on life ring must be replaced. 46 CFR 180.70(b)(1)

Any vessel of at least five net tons engaged in coastwise trade must have a valid Certificate of Documentation. Vessel must show proof of possession of Certificate of Documentation. 46 CFR 67.7

All vessels shall be provided with a hand operated portable fire pump having a capacity of at least five gallons per minute. Vessel must acquire a hand fire pump. 46 CFR 181.10-5

The marine inspector shall examine and test machinery installations and determine that they are in proper operating condition and fit for service. The water pump bearing on the main engine must be repacked. 46 CFR 176.25-10
