A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. First-aid kit had expired aspirin and eye wash items. 46 CFR 184.710

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Child life jackets were not separated or marked from adult life jackets. 46 CFR 180.78

An existing vessel with a hull, or a machinery space boundary bulkhead or deck, composed of wood or fiber reinforced plastic, or sheathed on the interior in fiber reinforced plastic, must comply with the requirements of § 181.400 of this part on or before March 11, 1999. The following spaces must be equipped with a fixed gas fire extinguishing system...A space containing propulsion machinery. Where the amount of carbon dioxide gas required in a fixed fire extinguishing system can be supplied by one portable extinguisher or a semi-portable extinguisher, such an extinguisher may be used subject to the following: (a) The cylinder must be installed in a fixed position outside the space protected. (b) The applicator must be installed in a fixed position so as to discharge into the space protected. (c) Controls must be installed in an accessible location outside the space protected. Vessel operator improperly removed CO2 Cylinder from vessel and must service and reinstall the cylinder in a fixed position as described above. 07Jul2023: Vessel operator reinstalled clyinder but the cylinder is not permanetly affixed to the pvc piping. Granted 30 day extension to maintain current arrangement and install fireboy system.

Certification or licensing by a state or local jurisdiction as a fire extinguisher servicing agency will be accepted by the Coast Guard as meeting the personnel certification requirements of NFPA 10 for annual maintenance and recharging of extinguishers. Vessel must service and reinstall portable fire extinguishers.

An adult life jacket must be provided for each person carried on board a vessel. Vessel only has 11 adult life jackets. Vessel is restricted to carrying 10 passengers. Total Persons allowed: 11.

Employers must maintain records of chemical tests as provided in 49 CFR 40.333 and must make these records available to Coast Guard officials upon request. Vessel rep did not provide records. Submit records of enrollment and chemical tests to USCG via email listed below.

The emergency instruction placard should contain at least the applicable portions of the “Emergency Instructions” listed in § 185.512. Vessel is missing heavy weather and more detailed info. Review 185.512 and update emergency instructions.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables, or a river current publication issued by the U.S. Army Corps of Engineers or a river authority. Vessel is missing publications.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. First-aid kits were both expired.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. If the cognizant OCMI determines that there is no suitable mounting surface aboard the vessel, the life jacket placards need not be posted but must be carried aboard the vessel and be available to the crew and passengers for familiarization. Life Jacket placards not on vessel. Vessel does not have a suitable mounting location but shall carry placards to be available upon request.

All vessels must have navigation lights that are in compliance with the applicable sections of Nav. Rules. The port and starboard side navigation lights were inop.

At least one ring life buoy must be fitted with a lifeline. Each lifeline must have a diameter of at least 5/16 inch. The lifeline on board was incorrect.

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. The operator could not provide proof of a medical certificate.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crew members on inspected vessels are are required by the vessel's COI. No proof of drug testing program could be provided.

Each life jacket must be approved in accordance with either 160.002, 160.005 or 160.055 in subchapter Q. The life jackets were missing retro reflective tape.

Rails or equivalent protection shall be installed near the periphery of all weather decks accessible to passengers and crew. The port side was missing railings.

Not to operate with passengers till fuel tank and all fuel lines were cleaned and opened for inspection by Coast Guard Marine Inspector; 46CFR176.702.

Provide Gages to indicate engine cooling water discharge temp and lubricating oil pressure for propulsion engine.

Provide 6 adult life jackets with proper retro-reflective tape. (Passenger count reduced to 6 until cleared).

Prove proper operation of submersible bilge pump with bilge free of contaminants.

Provide proper hand fire pump with adequate length of hose.

Provide waterlight for ring buoy. (Limited to daylight operations)
