IN ADDITION TO THE REQUIREMENTS CONTAINED ELSEWHERE IN THE PRESENT SOLAS REGULATIONS, SHIPS SHALL BE DESIGNED, CONSTRUCTED AND MAINTAINED IN COMPLIANCE WITH THE STRUCTURAL, MECHANICAL AND ELECTRICAL REQUIREMENTS OF A CLASSIFICATION SOCIETY. THE PORT GENERAL SERVICE PUMP, WHICH IS PRESENTLY BEING UTILIZED AS THE VESSELS MAIN FIRE PUMP, HAS TWO SOFT PATCHES ON THE DISCHARGE PIPING. 74SOLAS(04)II-1/3-1

THE COMPANY SHOULD ESTABLISH PROCEDURES TO ENSURE THAT THE SHIP IS MAINTAINED IN CONFORMITY WITH THE PROVISIONS OF THE RELEVANT RULES AND REGULATIONS AND WITH ANY ADDITIONAL REQUIREMENTS WHICH MAY BE ESTABLISHED BY THE COMPANY. SHIPS SMS STATES THAT THE DRAIN LINES ARE TO BE INSPECTED AND TESTED WITH WATER TO ENSURE OPERABILITY. PSCO OBSERVED MISSING DRAIN LINE ON FORWARD STBD SIDE OF NUMBER TWO CARGO HATCH AFTER MAINTENANCE RECORDS INDICATED THAT IT HAD BEEN VERIFIED OPERATIONAL ON 25JUL2022. 74SOLAS(20) IX/3.1, ISM CODE PART A/10.1

THE COMPANY SHOULD ESTABLISH PROCEDURES TO ENSURE THAT THE SHIP IS MAINTAINED IN CONFORMITY WITH THE PROVISIONS OF THE RELEVANT RULES AND REGULATIONS AND WITH ADDITIONAL REQUIREMENT WHICH MAY BE ESTABLISHED BY THE COMPANY. DUE TO THE DEFICIENCIES OF A TECHNICAL AND /OR OPERATIONAL NATURE IDENTIFIED ABOVE WHICH INDIVIDUALLY OR COLLECTIVELY PROVIDES OBJECTIVE EVIDENCE OF A SERIOUS FAILURE, OR LACK OF EFFECTIVENESS OF THE ISM CODE. AN EXTERNAL AUDIT SHALL BE CONDUCTED BY THE FLAG ADMINISTRATION OR RECOGNIZED ORGINIZATION ACTING ON BEHALF OF THE FLAG ADMINISTRATION BEFORE THE VESSEL WILL BE RELEASED FROM ITS DETENTION TO DETERMINE WHETHER THE VESSEL IS OPERATING IN COMPLIANCE WITH THE ISM CODE. The rating or appropriate setting of the overload protective device for each circuit shall be permanently indicated at the location of the protective device. PSCO observed emergency steering pump motor controller was triple-tapped due to installation of 440V outlets. PSCO observed boat davit, main engine aux. blower and #4 crane motor controllers similarly double-tapped.

Fire doors in main vertical zone bulkheads and stairway enclosures shall satisfy the following. The doors shall be self closing. PSCO observed steering gear door being propped open by security door and STBD engine room door to main deck would not self close.

Any discharge into the sea of oil or oily mixtures from ships shall be prohibited. PSCO observed a sheen coming from the #3 crane hydraulic cooling discharge port. Hydraulic system restricted from any use resulting in oil discharge.

All hatchways in position 1 and 2 shall be weathertight and fitted with gaskets. PSCO observed gaskets for the hatch to cargo hold #1 were loss and deteriorated.

Fire Safety - Means shall be provided to control leaks of flammable liquids. PSCO identified leak on the Number 1 Fuel Oil Supply Pump with oil accumulating in the bilge. 74 SOLAS (04) II-2/4.1.1

PSCO observed excessive saltwater leak on frost pump for cargo refrigeration causing large accumulation of water in bilge. 74 SOLAS (20) II-1/3-1

Satisfactory means in the form of guard rails and lifelines shall be provided for the protection of the crew in getting to and from their quarters, the machinery space and all other parts used in the necessary work of the ship. PSCO observed multiple ladder rails and guard rails with broken or missing safety devices while on deck walk. ICLL I/25-4

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire-resisting divisions, and protection of openings and penetrations in these divisions. PSCO identified a fire door on deck A and deck B leading outside the ship without a proper seal allowing air and light to pass through. 74 SOLAS (20) II-2/14.2.1.1.1

No electrical equipment shall be installed in any space where flammable mixtures are liable to collect, e.g. in compartments assigned principally to accumulator batteries, paint lockers, acetylene stores or similar spaces, unless the Administration is satisfied that such equipment is: of type which will not ignite in the mixture concerned. PSCO noted explosion proof light in Paint Locker did not meet that of original design. Explosion proof light was missing protective cage. 74 SOLAS (04) II-1/45.10.2

Self-igniting lights required by regulations III/7.1.3 shall be capable of burning continuously. PSCO noted self-igniting light on a starboard lifebuoy near the pilot ladder would not remain lit when tested. 74 SOLAS (97) III/31.2.2

0999-Other(safety in General) - The fire resistance of doors shall, as far as practicable, be equivalent to that of the division in which they are fitted. Doors and Door frames in "a" class divisions shall be constructed of steel. PSCO observed door in galley portside witha vent at the bottom and multiple air gaps around the frame. 1020-Fire Alarm-Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. 02 alarms inoperative and 01 alarms broken. 0936-Steering gear-In the event of a failure of electrical power supply to the steering gear control system, an audible and visual alarm shall be given on the navigation bridge. PSCO observed no visual or audible indication when power was secured to both #1 and #2 steering pumps. Chief had to pull relay in each motor controller to initiate alarms. SOLA 74 (2014) II-1/29.8.4 17a

1420-Cleanlieness of Engine Room-In a ship in which oil fuel is used, the arrangements for the storage, distribution, and utilization of the oil fuel shall be such to ensure the safety of the ship and persons on board. PSCO observed containment placed throughout the engine room full of oil. 74 SOLAS (14 CONS) Chapt II-2 Reg 4/2.2
