The minimum capacity of the power fire pump shall be 50 gallons per minute at a pressure of not less than 60 pounds per square inch... Fire pump could not achieve 60 psi (56 psi). 46 CFR 181.10-1(c)

All piping, valves,and fittings shall be in accordance with good marine practice and suitable for the purpose intended. Fire main pump seal is leaking. 46 CFR 181.15-5(c)

Exhaust piping shall be led to the point of escape with minimum number of bends or elbows. It shall be gas-tight throughout its length... The exhaust hose after the cooling injection point has multiple hose clamps and silicone RTV sealant as a temp repair. 46 CFR 182.15-20(a)

An internal structural examination conducted in compliance... consists of of a complete examination of the vessel's main strength members, hull plating, voids. Inspector's were unable to examine space that contained sewage tank due to excessive water, salt build up. Clean and vent space for re-inspection. Descale STBD fwd bulkhead in fuel tank space where coating is failing for re-inspection. 46 CFR 176.610(b)

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment other than small buoyancy compartments, under all practical conditions. Lazerette bilge suction line wasted and ingesting air preventing bilge pump from gaining suction to de-water the space. Repair piping and prove proper operation of the Lazerette bilge suction line to the satisfaction of the attending MI. 46 CFR 182.25-1(a) Old T-L

Shutoff valves, installed so as to close against the fuel flow, shall be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank shall be operable from outside the tank compartment, preferably from an accessible position on the weather deck. Emergency fuel shutoff valves leaking fuel from packing gland nut. Repair and prove proper operation to the attending MI. 46 CFR 182.20-40(b)(3) OLD T-L

Vessel owners or operators must ensure their vessels are operating in compliance with this part. Vessel failed to fully comply with the requirements contained in 33 CFR 104.140 and the approved Alternative Security Program by removing vessel from the list of covered vessel's listed on letter VS-331669-2842533 due to request for change of service from K-Existing (Old T-L) to Old T-L carrying no more than 150 passengers. Vessel passenger carriage reduced to no more 150 passengers until final Coast Guard approval of service change request. 33 CFR 104.115(a)

Cables entering boxes or fittings shall be protected from abrasion, and shall conform with the following requirements: openings through which conductors enter shall be adequately closed and the cable armor shall be secured to the box or fitting. Generator distribution panel not closed in way of the bottom panel through which conductors enter. Additionally, the cable armor for the conductor wiring from the generator to the distribution panel and the distribution panel to the transformer are not secured the box or fitting. Secure as described above to the satisfaction of the attending MI. 46 CFR 183.10-20(f)(1-2) Old T-L

Where flexibility is necessary, a section of flexible metallic hose may be used. Nonmetallic hose may be used for wet exhaust systems provided it is especially adapted to resist the action of oil, acid, and heat, and has a wall thickness sufficient to prevent collapsing or panting, and is double clamped where practicable. Port generator exhaust hose collapsing. Replace hose to the satisfaction of the attending MI. 46 CFR 119.430(e)

ll electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Starboard side engine room light lighting fixture is not mounted to the overhead. Mount light fixture to the satisfaction of the attending MI. 46 CFR 183.01-15(a) Old T-L

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including test of machinery alarms including bilge high level alarms. Vessel's crew was unable to conduct a satisfactory function test of the "Neutral Safety Throttle Only" Periodic Safety Test Procedure (PSTP). The Owner/Operator shall ensure that the crew can perform a satisfactory function test of the "Neutral Safety Throttle Only" PSTP to the satisfaction of the attending USCG Marine Inspector. If the crew cannot perform a satisfactory test of the "Neutral Safety Throttle Only" PSTP due to the current installation of the engines and controls the "Neutral Safety Throttle Only" PSTP shall be revised accordingly and submitted to the Marine Safety Center for review and approval. Once approved, a satisfactory test of the revised "Neutral Safety Throttle Only" PSTP will need to be completed to the satisfaction of the attending USCG Marine Inspector. 46CFR115.804(i)

Materials used in piping systems must meet the requirements of this subpart and be otherwise acceptable to the cognizant OCMI. The pipe fitting above the through hull valve that leads to the air conditioning system is fractured and shall be replaced. 46CFR182.700

Each vessel must carry a certificate with a valid endorsement while subject to the Bridge-to-Bridge Act. Vessel is a power-driven vessel of 20 meters or over in length and is required to have a valid Bridge-to-Bridge Act Certificate. Vessel does not have a valid Bridge-to-Bridge Act Certificate. 47CFR80.1005

The installation requirements for machinery are contained in applicable regulations in subchapter F (Marine Engineering of this chapter. Attending MI observe the outboard port main engine turbo oil drain line actively leaking during engine operation. Attending MI observed the outboard port generator fuel return line fitting leaking while the engine was static and the centerline generator fuel supply line actively leaking during engine operation.

Fill pipes for fuel tanks must be so arranged that overflow of liquid or vapor cannot escape to the inside of the vessel. The 02 fuel fill pipes on the starboard side of the Tank Compartment are wasted. Provide a repair proposal to the satisfaction of the Marine Inspector to ensure fuel vapors do not escape into the compartment, and the main deck is made watertight.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provision for a seaworthy hull. The forward watertight bulkhead of the Tank Compartment in way of the starboard winch box has corrosion which penetrates through the entire plate. Investigate the extent of the corrosion, and submit a repair proposal to the satisfaction of the Marine Inspector to restore the watertight integrity of the transverse bulkhead.

In systems and applications where flexible hoses are permitted to be clamped; double hose clamping is required where practicable. The nonmetallic hose for raw water suction for both generators contained a single hose clamp.

The design of all exhaust systems must ensure minimum risk of injury to personnel. Protection must be provided in compliance with §116.970 of this chapter at such locations where persons or equipment might come in contact with an exhaust pipe. Forward exhaust support stanchion for port outboard main engine exhaust not insulated. Aft exhaust support stanchion for port outboard main engine insulation loose and requires tensioning.

Attending MI sighted a missing hose clamp on the inboard port main engine exhaust flexible hose at the aft engine room bulkhead transition.

Vessels shall be inspected for compliance with the standards required by this subchapter. For items not covered by standards in this subchapter, inspection shall be in accordance with standards acceptable to the Officer in Charge, Marine Inspection, as good marine practice. Attending MI sighted the fire extinguisher for the number five fire station placarding on the aft port side of the passenger accommodation space but no fire extinguisher. The fire extinguisher was relocated to the aft starboard side of the passenger accommodation space. Vessel must move fire stations dedicated placard to appropriate fire station location.

Each vessel must be provided with a visual and audible alarm at the operating station to indicate a high water level in each normally unmanned spaces. The Void #1 high water indicator light illuminates in tandem with high water lights from other spaces.

A self-propelled vessel must meet the applicable requirements for main and auxiliary steering apparatus in subchapter F (Marine Engineering) and J (Electrical Engineering) of this chapter. The rudder angle indicator electrical box connected to the rudder post is not properly sealed to protect again water intrusion.

A vessel with installed toilet facilities must have a marine sanitation device that complies with 33 CFR part 159. The stbd MSD drain line fitting has failed and the drain line is no longer connected to the system.

All machinery must be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. Main engine sea water pipe has leak on a threaded flange prior to the sea water valve.

In systems and applications where flexible hoses are permitted to be clamped: Double hose clamping is required where practicable. Missing double hose clamps on multiple r/w hoses to include outboard generator.

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Flexible sea water suction line on inboard generator for wet exhaust collapsed possibly due to clogged sea water strainer.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Demonstrate proper operation of bilge high water level audible alarm for the Void # 1 space.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Demonstrate proper operation of both machanically driven bilge pumps.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Replace port outboard shaft no later than January 31, 2020. Port outboard shaft was found to have pit beyond allowable wastage limits. Vessel may continue to operate excluding the outboard port shaft.

Update Emergency Broadcast Placard Instructions with vessels correct call sign.

Make draft marks contrasting colors.

Properly mount bilge suction piping in way of #2 void space.

Replace worn inboard shaft cutlass bearings.

Clean and make shafts, props, and rudders available for inspection.

Renew piping for CHT tank at deck connection.

Remove seachest screens and make thru-hull penetrations available for inspection.

Make winch boxes located in tank space watertight.

Properly seal and make vapor tight all insulation located in tank space in accordance with fire protection standards.

Reweld nipple portside for CHT piping.

Replace worn engine exhaust hoses.

Renew floor drain located starboard side lazarete.

Provide specs for carpet replacement.

Submit plans for approval to crop and renew wasted hull plating in way of transom.

Submit plans for approval to renew all wasted shaft/stern tubes. Upon approval of plans crop and renew to the satisfaction of an attending Coast Guard Marine Inspector prior to credit drydock.

Replace missing structural fire protection in the tank room to A-0.

Update the fire control plan to indicate (SFP) requirement. Deck between the tank room and public space to A-0 and the deck between the engine space and the public space A-60.

Make repairs to the port outboard rudder to ensure rudder movement at helm command.

Make repair to the leaking stbd exhaust pipe located in the lazarette and prove.

Remove from service the holed port sewage tank (isolate and gas free).

Prove the watertight integrity of the lazarette in way of the wasted vent box and other unapproved penetrations and exhaust rerouting.

Make permanent repair to the bilge system in way of the temp repair.

Make permanent repairs to the deteriorated and leaking fire main system.

Seal the port winch box located in the engine space rigged for ventilation.

Provide a positive stop valve for the through hull penetration located in the lazarette (port side).

Remove unapproved power ventilation from the lazarette or provide plans for approval.

Remove unapproved bilge systems throughout the vessel or provide plans for approval.

Make repairs to the leak in the port genset cooling water overflow tank and prove.

Prove proper installation of cable penetration in way of loose cables in the engine room.

Replace the deteriorated after cooling hose in way of the starboard outboard MDE.

Repair and provide chafing in way of the warn electrical cable (stbd genset & fire pump).

Make repair to the leaking forepeak bilge suction line located in the tank room.

Make repairs to the deteriorated winch box located in the tank room.

Provide life jacket donning instructions for the 2nd deck.

Repair or replace life jackets in way of delaminating retro.

Provide up-to-date charts for all operating areas as noted on the COI

Repair/renew all vent closures found inoperable.

Make repair or replace P&S ring buoys in way of the deteriorated plug, life lines, and retro.

Renew the wasted coupling for the raw water intake (sewage sys.) located in the lazarette
