On the starboard side, the middle course of the guard rail near midships was broken causing a gap greater than 380 mm.

All hatchways in position 1 and 2 shall be fitted with a hatch cover of steel and shall be weathertight and fitted with a gasket and clamping devices. The means for securing and maintaining weathertightness shall be to the satisfaction of administration. A number of hatches inside the forward tween deck had missing or broken dogs required to properly secure the hatches.
