12109-Dangerous Goods-The carriage of solid bulk cargoes other than grain shall be in compliance with the relevant provisions of the ISMBC Code. In accordance with Appendix 1 of ISMBC the oxygen concentration in the cargo spaces carrying Direct Reduced Iron (C) shall be maintained at less than 5% throughout the duration of the voyage by topping up with inert gas. PSCO observed cargo tanks 1,3,5 were not inert at any time during the voyage. Vessel could not provide a valid Coast Guard special Permit on board.

Resolution: Reviewed SP 1-12 issued by CG. Verified VSL logged oxygen and hydrogen content and within cargo holds. VSL took readings every 6 hours. SP 1-12 is attached.
08104-Alarms-In the event of a power failure to any one of the steering gear power units, an audible and visual alarm shall be givin on the navigation bridge. PSCO observed #2 steering gear power unit did not give audible or visual alarm upon loss of power.

Resolution: Received class report stating that the refractory material has been patched satisfactorily.
1730-Oil filtering equipment/ system shall be provided with arrangements to ensure that any discharge of oily mixtures is automatically stopped when the oil content of the effluent exceeds 15 ppm. Oil water separator overboard discharge line has cut out valve installed in Oil Content Meter supply line as well as a fresh water supply to the inlet of the Oil Content Meter, this could cause a bypass of or dilution of actual supply. This could cause inaccurate parts per million readings of Oil Content Meter-MARPOL (11 Cons.) Annex I/Reg. 14.7
