The arrangements for the storage, distribution and utilization of oil used in pressure lubrication systems shall be such as to ensure the safety of the ship and persons on board. PSCO observed signs of an oil leak under the terminal box on the No. 2 generator.

The main steering gear and the auxiliary steering gear shall be so arranged that the failure of one of them will not render the other one inoperative. PSCO was notified from pilot that vessel experienced loss o fsteering while getting underway from Redwood City, CA.

All external high pressure fuel delivery lines between the high pressure fuel pumps and fuel inspectors shall be protected with a jacketed piping system. PSCO found double jacketing on high pressure fuel line leading to injector control unit to be ripped and torn compromising the integrity of the hose. PSCO also found high pressure fuel hose on #4 cylinder to be deformed. 74SOLAS(01)II-2/15.2.9

Machinery space openings in position 1 or 2 shall be properly framed and efficiently enclosed by steel casings of ample strength, where the casings are not protected by other structures their strength shall be specifically considered. PSCO observed multiple bolts missing from access hatch leading to engine room from main deck. ICLL I/17(1)
