All automatically or remotely controlled or monitored vital systems addressed by part 62 of this subchapter must be subjected to tests and inspections to evaluate the operation and reliability of controls, alarms, safety features, and interlocks. Test procedures must be submitted to the Coast Guard for approval. Generate and submit PSTPs for #1 Generator to MSC for approval.

The installation of all systems of a marine engineering nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter F (Marine Engineering) of this chapter. Nonmetallic flexible hose must be in accordance with SAE J1942 (incorporated by reference; see § 56.01-2) and may be installed only in vital and nonvital fresh and saltwater systems, nonvital pneumatic systems, lube oil and fuel systems, and fluid power systems. Nonmetallic flexible hose may be used in lube oil, fuel oil and fluid power systems only where flexibility is required and in lengths not exceeding 30 inches. Two fuel hoses on stbd generator was well over 30 inches. One fuel hose on each main engine was well over 30 inches. All fuel hoses on all engines were unmarked and could not ascertain if any of the hoses met SAE J1942. 46 CFR 77.03-1(a) 46 CFR 56.60-25(b)(1) 46 CFR 56.60-25(b)(4)

MSC correspondence (E2-2302177, dated 31MAY24), indicates the following requirements: 1) for permanent communications between primary remote engine control location and manual alternate control location. 2) Manual alternate control locations must include shaft speed and direction indicators and emergency stop. Revise test procedure to demonstrate compliance with above.

Doors in stairway enclosures and Main Vertical Zone bulkheads shall, in addition to meeting the requirements of this paragraph, also meet the following requirements: All doors, except those that are kept normally closed, shall be of a type which are capable of release from the control station and from a position at the door. Door on 04 deck meeting stairway enclosure could not be released (closed) from bridge control station. Repair release mechanism and demonstrate closing from bridge control.

All electrical engineering details and installations shall be designed and installed in accordance with subchapter J (Electrical Engineering) of this chapter. All emergency lights shall be marked with a letter “E” at least 1/2 inch high. Port side main ladderwell e-light installations missing "E" label. 02 emergency lights out on 01 deck. 02 emergency lights out on hold deck. 01 battery powered emergency light underpowered/dimmed on 04 deck ladderwell. Replace lights, batteries, labels.

Lifesaving appliances, instructions to passengers, and stowage locations must be marked in accordance with subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Containers, brackets, racks, and other similar stowage locations for lifesaving equipment must be marked with symbols in accordance with IMO Resolution A.760(18) indicating the device stowed in that location. Ring buoy placards found weathered. Replace placards.

The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Current hailing port and name not updated or legible on stern. Update markings.

Lifebuoy of the vessel fitted with self-igniting light approved under approval series 161.010. Found 04 float lights missing the tensioning O-ring at the retention pin location and all of the floating water light lanyards were deteriorated. CITE 46 CFR 70.28-1 46 CFR 199.70(a)(3)

Each vessel must carry lifebuoys approved under approval series 160.050. Party deck aft and multiple other ring buoy’s beckets were deteriorated. CITE 46 CFR 70.28-1 46 CFR 199.70(a)

Each device must have a nameplate showing the device's function. Noted several electrical distribution panels without breakers being labeled throughout vessel. CITE 46 CFR 70.25-1 46 CFR 111.30-15

Electric installations must ensure protection of passengers, crew, other persons, and the vessel from electrical hazard. Found globe lights in dry store that were missing the light bulbs and globes. CITE 46 CFR 70.25-1 46 CFR 111.01-1(a)(2)

Wires/cables must be in an enclosure. Multiple instances of dead-end wires were noted. wires/cables to be removed or properly enclosed prior to next quarterly. CITE 46 CFR 70.25-1 46 CFR 111.60-11

Life jacket bins were not stowed securely. Prior to completion of inspection, life jacket bins were placed in their footings and stowed securely. CITE 46 CFR 70.28-1 46 CFR 199.70(b)(2)

Suitable storm rails shall be installed in all passageways where passengers or crew have normal access. Rails in stairwell found loose and/or not secured properly to deck. Proper repairs to be carried out. 46 CFR 72.40=10(a)

Cables should be adequately protected where exposed to damage. Multiple cables should be supported in metal hangers or trays. Multiple unsupported wires were found in the overhead of the EDG space.

Suitable storm rails shall be installed in all passageways where passengers or crew have normal access. Rails in stairwell found to be broken, loose and/or not secured properly to the deck. Cite: 46 CFR 72.40-10 (a)

Piping... whose temperature exceeds 150°F, shall be suitably insulated where necessary to preclude injury to personnel. Insulation missing on port main engine exhaust. Cite: 46 CFR 77.03-1 46 CFR 56.50-1(k)

Lighting: each fixture and lampholder must be fixed. A fixture must not be supported by the screw shell of a lampholder. Light in starboard generator space found not secured. Cite: 46 CFR 77.05-1 46 CFR 111.75-20(d)(2)

Connections and terminations to all conductors must retain the original electrical, mechanical, flame-retarding and fire-resisting properties of the cable. -Exposed wire found on cord used for the starboard generator room light. Cite:46 CFR 77.05-1 46 CFR 111.60-17(a)

Use of flexible cord (extension cords) is not acceptable for use. Extension cord found being used as connection for light in starboard generator space. Cite:46 CFR 77.05-1 46 CFR 111.60-13

Each alarm, fire detector, test station etc. must be installed in accordance with 161.002 and meet the requirements of this subpart. Heat temperature sensor not properly secured above starboard generator. Cite: 46 CFR 77.05-1 46 CFR 113.01-1

Approved hand portable fire extinguishers shall be installed on all vessels as set forth in subpart 76.50. Extinguishers found to be using bracket that is not approved. Cite: 46 CFR 76.05-25

At each inspection the marine inspector shall conduct such tests and inspection of the main propulsion and auxiliary machinery. Unable to carry out testing of starboard generator shut downs. Cite:46 CFR 77.03 46 CFR 61.20-3

Each fixture globe, lens or diffuser must have a high strength guard or be made of high strength material. Port generator space missing light covers in two locations. Cite: 46 CFR 77.05-1 46 CFR 111.75-20(b)

Connections and terminations to all conductors must retain the original electrical, mechanical, flame-retarding and fire-resisting properties of the cable. Dead end wires found above the forward end of the port generator. Cite: 46 CFR 77.05-1 46 CFR 111.60-17(a)

Twist-on type of connectors may not be used for making joints in cables, facilitating a conductor splice, or extending the length of a circuit. Twist-on type connectors were found throughout the engine room and on the inboard aft side of the starboard main engine. Cite: 46 CFR 77.05-1 46 CFR 111.60-17(b)(c)

Cables should be adequately protected where exposed to damage. Multiple cables should be supported in metal hangers or trays. Cables found with out protection from chaffing and not supported in metal hangers. Cite: 46 CFR 77.05-1 46CFR 111.60-5 IEEE 45-2002 (25.4) IEEE 45-2002 (25.5)

NON-METALLIC FLEXIBLE HOSE MUST HAVE FACTORY ASSEMBLED END FITTINGS REQUIRING NO FURTHER ADJUSTMENT OR FIELD ATTACHABLE FITTINGS. FLEXIBLE FUEL HOSE ON STARBOARD GENERATOR DO NOT HAVE PROPER END FITTINGS.

EACH LIFEJACKET CONTAINER MUST BE MARKED IN BLOCK CAPITAL LETTERS AND NUMBERS WITH THE QUANTITY AND IDENTITY. MARK ALL LIFEJACKET STOWAGE LOCATION WITH THE APPROPRIATE NUMBER.

MEANS SHALL BE PROVIDED FOR CLOSING ALL OPENINGS TO THE SPACE PROTECTED BY CARBON DIOXIDE. ADJUST DOOR TO EDG ROOM FOR SMOOTH CLOSURE.

ALL CREW SPACES ARE TO BE CONSTRUCTED AND ARRANGED IN A MANNER SUITABLE TO THE PURPOSE FOR WHICH THEY ARE INTENDED AND SO THAT THEY CAN BE KEPT IN A CLEAN, WORKABLE, AND SANITARY CONDITION. UNSECURED DECK PLATING IN THE ENGINE ROOM IS CREATING A SAFETY HAZARD. SECURE DECK PLATING.

EACH VESSEL MUST CARRY LIFEJACKET FOR EACH PERSON ON BOARD. TOTAL PERSONS ON BOARD IS LIMITED TO 1629. 1715 SERVICABLE LIFEJACKETS ARE ON BOARD. THIS ALLOWS FOR THE 5% EXTRA REQUIRED BY THIS PART.

DOORS IN STAIRWAY ENCLOSURES SHALL BE OF THE SELF CLOSING TYPE AND CAPABLE OF CLOSING AGAINST A 3 AND 1/2 DEGREE LIST. DOOR IN STAIRWAY NEEDED ASSISTANCE TO SHUT ALL THE WAY.

Bow thruster tunnel found excessively pitted in the band around the propeller. Hub found with excessive pits. The areas has been previously clad weld 3 times during dry dock in 2004, 2010 and 2015. Company to submit a written repair proposal by a responsible party attesting to to their plan with no concerns over the brittleness of the original material.

Openings on the underside of 2 electrical panels on the engine room port bulkhead, just aft of the engine room entrance (panel numbers L114 and L103)that need to be sealed. 46 CFR 111.40-5

Wiring found galley bilge found wrapped in electrical tape. Remove tape and make satisfactory connections. 46 CFR 111.60-17

Pipes piercing the collision bulkhead shall be fitted with screwdown valves operable from above the bulkhead deck. Verify this is kept in a ready condition. 46 CFR 56.50-1(b)(1)

Door to EDG is corroded at base and lower door frame. 46 CFR 71.55

Sufficient illumination must be provided under emergency conditions. Verify and replace emergency lighting.

Main propulsion machinery arrangements must be equivalent to ABS rules. Exhaust leak found on starboard main engine.

There shall be 2 means of escape from where crew may be normally employed. Engine room escape hatch has metal protrusion and broken dog.

Verify placement and operation of rescue boat

Main propulsion machinery must be provided with automatic shut off controls. Port main engine overspeed not functioning.

All relief valves on pressure vessels shall meet service requirements, set not to exceed MAWP. Replace valve on control air and verify valve on bow thruster reciever.

WTD indicator must show whether door is open or closed. Wiring for engine room WTD is broken.

Each lifebuoy must be capable of being rapidly cast loose. Bracket outside EDG is broken.

A class boundaries must be maintained. Bulkhead penetration not properly sealed.

Shut off valves shall be provided in fuel supply lines. Repair and lubricate as necessary.

Re-stencil the emergency pull for the CO2 system to the EDG.

Control air regulated air pressure gauge is broken. Renew as original.

Make permanent repairs to the machinery space emergency egress scuttle. Dog leg missing attachment to bottom side of scuttle.

Port Generator expansion joints are missing five bolts from the mounting collar. Install prior to sailing.

Renew the battery for the floatable waterlight on the starboard forward lifering.

replace 110 lifejackets.

Prover proper operation of reverse power relay on new (#2) generator.

Vessel is limited to 1610 total persons until all rejected life jackets are replaced

Vessel sustained damages to the starboard generator prime move after catastrophic failure. Renew or make permanent repairs to prime mover/generator.

Provide load analysis and updated one line diagram to Marine Safety Center for approval prior to new prime mover/generator install.

Provide third party testing results of elevator safety devices as per ANSI/ASME.

Renew the jacket water temp sending unit for the high level alarm.

Provide fluid pressure gauge and temp sensor to accomplish PSTP's.

Provide exhaust blanketing for turbo elbow on the starboard generator.

Provide circuit directory for panelboard outside of the main control room in the generator space.

Provide MOB recovery drills after installation of new rescue boat.

Install battery powered E-light in the emergency generator space. Missing from bracket.

Bilge alarm is hanging from wiring within the bilge of office compartment. Properly install on fixed bracket.

Bilge alarm in the forward office compartment is intermittently alarming.

Generator #1 voltage indicator inoperative in EOS.

High Jacket Water Temp gauge on PME is approximately 40 degrees lower than actual jacketwater temp. Alarm is working appropriately however.

Renew time delay(s) within the bow thruster CO2 piping and the same for the engine room system.

Renew high level alarm in the machinery space.

Make permanent repairs to starboard steering ram inboard seal which is actively leaking during rudder movement.

Ensure all watertight door indicator lights are installed where required and operating properly.
