11101 - Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. The port side stern life boat caught fire rendering it inoperable. The amount of persons permitted on the vessel is reduced to 50. 74 SOLAS (25) III/20.2 46 CFR 199.261(b)(1)

Before the ship leaves port and at all times during the voyage , all life saving appliances shall be in working order and ready for immediate use. The rescue boat was improperly positioned in its davit and may not be designed for this davit, posing a fall hazard. The inflatable pontoon was partially deflated and pressing against the davit, creating a potential pressure point. The fiberglass hull was in direct contact with the davit. Additionally, the boat does not match the one listed in the SOLAS training manual and could not start due to a dead battery, however a replacement battery is on order. 17ac 74 SOLAS (2024) III/20.2

An embarkation ladder complying with the requirements of paragraph 6.1.6 of the Code extending, in a single length, from the deck to the waterline in the lightest seagoing condition under all conditions of trim of up to 10° and a list of up to 20° either way shall be provided at each embarkation station or at every two adjacent embarkation stations for survival craft launched down the side of the ship. The embarkation ladders, were not LSA 6.1.6 ladder but were pilot ladders constructed under ISO-799 standard, which had surpassed the 30-month weight test requirement specified by this standard. 74 SOLAS (2024) III/11.7

The machinery shall be of a design and construction adequate for the service for which they are intended. It is reported that a jacket water leak, and later, overheating, occurred on the #10 cylinder of the center main engine. **Based on objective evidence provided by the master and chief engineer attesting to satisfactory temporary repairs to put #10 cylinder out of operation, this deficiency (originally issued on 9/18/22) has been amended from a code 60 (prior to movement) to a code 15 (by next port). As part of 5 year dry dock and renewal of statutory certificates and COI, full and permanent repairs to the center main engine shall be made to the satisfaction of the RO (ABS) and the CG. *****ABS opened class finding no. 1597 based on initial report from vessel master to ABS; deficient condition not yet corrected. The jumper line that failed on the 18th is reported to have also failed back in 5/22. Recommend follow-up on repeated part failure.*****

The machinery shall be of a design and construction adequate for the service for which they are intended. It is reported that a jacket water leak, and later, overheating, occurred on the #10 cylinder of the center main engine. Prior to movement (code 60), provide a written attestation from the master and chief engineer reporting to the cause of the overheating and that temporary repairs have been made to permit the continuation of a safe transit to Philadelphia Ship repair at reduced speed. *After receipt and review by the CG, this deficiency may then potentially be changed to a code 15 (by next port), permitting the vessel's one-time transit to Philadelphia Ship Repair for dry dock. Nonetheless, prior to departure from dry dock and further underway operations, full repairs shall be made to the main engine. UPDATE 9/19/22: *Based on objective evidence provided by the master and chief engineer attesting to satisfactory temporary repairs to put #10 cylinder out of operation, this deficiency (originally issued on 9/18/22) has been amended from a code 60 (prior to movement) to a code 15 (by next port). As part of 5 year dry dock and renewal of statutory certificates and COI, full and permanent repairs to the center main engine shall be made to the satisfaction of the RO (ABS) and the CG.
