The rescue boat was improperly positioned in its davit and may not be designed for this davit, posing a fall hazard. The inflatable pontoon was partially deflated and pressing against the davit, creating a potential pressure point. The fiberglass hull was in direct contact with the davit. Additionally, the boat does not match the one listed in the SOLAS training manual and could not start due to a dead battery, however a replacement battery is on order. 17ac 74 SOLAS (2024) III/20.2

An embarkation ladder complying with the requirements of paragraph 6.1.6 of the Code extending, in a single length, from the deck to the waterline in the lightest seagoing condition under all conditions of trim of up to 10° and a list of up to 20° either way shall be provided at each embarkation station or at every two adjacent embarkation stations for survival craft launched down the side of the ship. The embarkation ladders, were not LSA 6.1.6 ladder but were pilot ladders constructed under ISO-799 standard, which had surpassed the 30-month weight test requirement specified by this standard. 74 SOLAS (2024) III/11.7

Applicable vessels are now required to install and maintain video and audio surveillance systems and retain recordings for a specified time. Vsl did not have video and audio surveillance system installed IAW CVC Policy Letter 23-05.

The company and the ship shall comply with the requirements of the international safety management code. Security records were not available for presentation.

Cables and wiring shall be installed and supported in such a manner as to avoid chafing or other damage. General alarm light fixture wiring was severed in Cargo Deck B3.

Falls used in launching shall be inspected periodically with special regard for areas passing through sheaves, and renewed when necessary due to deterioration of the falls or at intervals of not more than 5 years. Renewal date of the falls could not be determined, provide documentation attesting to base or renewal and stencil in way of lifeboats.

As part of 5 year dry dock and renewal of statutory certificates and COI, full and permanent repairs to the center main engine shall be made to the satisfaction of the RO (ABS) and the CG. *****ABS opened class finding no. 1597 based on initial report from vessel master to ABS; deficient condition not yet corrected. The jumper line that failed on the 18th is reported to have also failed back in 5/22. Recommend follow-up on repeated part failure.*****

Nonetheless, prior to departure from dry dock and further underway operations, full repairs shall be made to the main engine. UPDATE 9/19/22: *Based on objective evidence provided by the master and chief engineer attesting to satisfactory temporary repairs to put #10 cylinder out of operation, this deficiency (originally issued on 9/18/22) has been amended from a code 60 (prior to movement) to a code 15 (by next port). Individuals qualified as AB-limited may constitute not more than 50 percent of the number of able seaman required on board vessels of > 1600 gross tons. Of current compliment of 6 ABs, there are 4 AB-limited and 2 AB-unlimited. This means that >50 percent of the ABs are AB-limited.

Each Administration shall, in accordance with the provisions of section A-I/14, hold companies responsible for the assignment of seafarers for service on their ships in accordance with the provisions of the present Convention, and shall require every such company to ensure that each seafarer assigned to any of its ships holds an appropriate certificate in accordance with the provisions of the Convention and as established by the Administration and its ships are manned in compliance with the applicable safe manning requirements of the Administration. Provide MMC for all crew members upon activation of vessel. STCW Reg. I/14.1

Vessels in ocean, coastwise, or Great Lakes service, except unmanned barges, shall be fitted with anchors. chains, and hawsers in general agreement with the Standards established by the American Bureau of Shipping. Provide this office with Classification Survey Report attesting to the satisfactory condition of the Port Anchor Motor and associated components.

Every crew member shall participate in at least one abandon ship drill and one fire drill every month. The drills of the crew shall take place within 24 h of the ship leaving a port if more than 25% of the crew have not participated in abandon ship and fire drills on board that particular ship in the previous month. Upon activation and prior to getting underway, conduct satisfactory fire and abandon ship drills.

Every officer in charge of a navigational watch serving on a seagoing ship of 500 gross tonnage or more shall hold a certificate of competency. Every officer in charge of an engineering watch in a manned engine-room or designated duty engineer officer in a periodically unmanned engine-room of a seagoing ship powered by main propulsion machinery of 750kw or more shall hold a certificate of competency. Upon activation and prior to getting underway, provide crew license and documents IAW vessel COI.

Vessel shall be towed by a minimum of two tugs of at least 9,000 aggregate total horsepower, or; your vessel shall be towed by a minimum of two tugs of at least 90 ton aggregate bollard pull. 33 CFR 160.111(c)

Public address system does not fucntion properly on all weather dec stations. All work parties on the weather deck shall have a portable handheld radio until system is repaired to the satisfaction of ABS surveyor.

Make lifeboat #1 clutch operable. Vessel restricted to 50 total persons.
