Controlled tank venting systems shall consist of a primary and a secondary means of allowing full flow relief of vapour to prevent over-pressure or under-pressure in the event of failure of one means. Alternatively, the secondary means may consist of pressure sensors fitted in each tank. The vacuum side of 07 pressure-vacuum valves and the pressure side of 01 pressure vacuum valves were blocked off to prevent the passage of vapors, leaving the ship with no vapor relief, and causing substantial risk to the structural integrity of the cargo and the ship. 74SOLAS(14) VII/10.1 IBC 8.3.3 10c

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations. Company procedures and manufacture maintenance procedures require that tanks be gas-freed and depressurized before conducting maintenance on the pressure-vacuum valves. Company and manufacturer procedures were not followed by the ship's crew. The only source of vapor relief for the tanks were blocked off with no risk analysis conducted and no additional safety measures implemented to mitigate the danger to the ship and crew.
