Controlled tank venting systems shall consist of a primary and a secondary means of allowing full flow relief of vapour to prevent over-pressure or under-pressure in the event of failure of one means. Alternatively, the secondary means may consist of pressure sensors fitted in each tank.
The vacuum side of 07 pressure-vacuum valves and the pressure side of 01 pressure vacuum valves were blocked off to prevent the passage of vapors, leaving the ship with no vapor relief, and causing substantial risk to the structural integrity of the cargo and the ship.

Tank venting systems shall be designed so as to minimize the possibility of cargo vapour accumulating about the decks, entering accommodation, service and machinery spaces and control stations and, in the case of flammable vapours, entering or collecting in spaces or areas containing sources of ignition.
Cargo tanks 1C, 2CP, 3CP, 4CP, 3CS, 3S, 9C were losing pressure from an unknown source. Vapor tightness of tanks and venting system must be verified to eliminate risk of vapor accumulation.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations.
Company procedures and manufacture maintenance procedures require that tanks be gas-freed and depressurized before conducting maintenance on the pressure-vacuum valves. Company and manufacturer procedures were not followed by the ship's crew. The only source of vapor relief for the tanks were blocked off with no risk analysis conducted and no additional safety measures implemented to mitigate the danger to the ship and crew.
Company shall conduct analysis and corrective actions in accordance with SMS procedures and provide to USCG.

The vessel experienced an equipment failure while transiting from Odjfell terminal to Barbours-Cut, which adversely affected its maneuverability and posed a hazard along the Houston Ship Channel. Resolution: Received, reviewed and accepted class report from DNV stating the deficiency was rectified. Goltens technicians onboard conducted maintenance on fuel pumps #1 and #6. #2-5 fuel pumps replaced entirely. Op-test engine, test sat.
