Each towing vessel must have either a self-priming, power-driven, fixed fire pump, or a portable fire pump, with hoses and nozzles, in accordance with paragraphs (e) and (f) of this section. During the inspection, the required portable fire pump was not onboard the vessel. The owner/operator reported that the previous pump was non-operational and a replacement has been ordered. Prior to towing operations, ensure a compliant portable fire pump is placed onboard. The replacement pump must be self-priming, power-driven, and meet a minimum capacity of at least 300 LPM (80 gpm) at a discharge gauge pressure of not less than 414 kPa (60 psi) measured at the pump discharge. 46 CFR 142.325

Charts and nautical publications. A towing vessel must carry adequate and up-to-date charts, maps, and publications for the intended voyage. (Note: 33 CFR § 164.72(b)(2) specifically requires towing vessels 12 meters or more to carry a currently corrected U.S. Coast Pilot). The vessel's onboard U.S. Coast Pilot publication was not current. 46 CFR 140.705(a)

Requirement to have Certificate of Documentation on board. The original Certificate of Documentation must be kept on board the vessel at all times. The operator provided evidence of application for a COD, but the original Certificate of Documentation was not onboard the vessel at the time of inspection. 46 CFR 67.313(a)

Stowage of survival craft. Each survival craft must be stowed so as to float free in the event of the vessel sinking, and the weak link of the painter must be properly secured to the vessel. The vessel's Inflatable Buoyant Apparatus (IBA) was not properly installed. Painter line was not connected to the weak link. 46 CFR 141.330(b)

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. The medical certificate will be issued for various periods of time based upon endorsements the mariner holds. Vessel captain has an expired medical certificate. 46 CFR 10.301

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. Current installed smoke detectors are not UL217 approved. 46 CFR 142.330(B)

Each towing vessel must be equipped with an industrial type first aid kit. Multiple medications found expired. Replace expired medical kit. 46 CFR 140.435

A towing vessel subject to this subchapter and choosing the Coast Guard inspection option, or required to have the Coast Guard inspection option, must undergo an annual inspection within 3 months before or after the COI anniversary date. No annual inspection was completed within the 3 month COI anniversary date window. Vessel shall not operate in any capacity permitted by the issued COI. COI is not valid, until a COI inspection is completed by a Marine Inspector and a new certificate is issued. 46CFR137.200

A vessel of 12 meters or more in length shall be provided with a whistle. Installed ship's whistle was reported inoperable. Repair and prove proper operation. 33 CFR 83.33(a)

Emergency lighting must be provided for all internal crew working and living areas. E-lighting for engine room and crew living area was inoperable. Repair and prove proper operation.

Personnel that are working in an area on the exterior of the vessel without rails and guards must wear a work vest. When worn at night, the work vest must be equipped with a light that meets 46 CFR 141.340(g)(1).

Each towing vessel must be equipped with an industrial type first aid cabinet or kit. Medication in first aid kit was expired. Replace expired medication.

Towing vessels must meet the communications requirements of 33 CFR part 26 and 33 CFR 164.72, as applicable. The vessel's MMSI number was not entered in the VHF radio. Properly input number and provide proof to the marine inspector.

Each escape hatch used as means of escape must be marked on both sides in clearly legible letters at least 2 inches high: “EMERGENCY EXIT, KEEP CLEAR”. Escape hatch from generator space was only labeled within the space. Properly label on the external side.

The master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. Grab line for the stbd ring life buoy was deteriorated. Repair or replace grab line.

Each towing vessel must carry on board and maintain a currently corrected edition of, or a corrected extract from each of the following: Coast Guard Light List (LL), Notices to Mariners (NTM), U.S. Coast Pilot (CP). LL, NTM and CP were either missing or out of date. Provide current editions of required publications.

Each remote [fuel shutoff] valve control must be marked in clearly legible letters, at least 1 inch high, indicating the purpose of the valve and the way to operate it. Labeling for the fuel shutoff valve was faded and barely legible. Replace labeling.

The hailing port must be marked on some clearly visible exterior part of the stern of the vessel. The hailing port must include the State, territory, or possession in which it is located. Hailing port was missing the State. Properly label the stern.

All lifesaving equipment must be tested and maintained IAW the minimum requirements of 199.190 and recorded in the TVR. Weekly and monthly inspections were not being recorded. Begin record of inspections and maintenance and provide to the CG for the month of October.

The master of a towing vessel must ensure that each crew member participates in fire-fighting drills and receives instruction at least once each month. Fire-fighting drills were not being completed once per month. Complete and record drills and provide record to CG inspector.

Operator has not been logging Navigation Assessments as required. Ensure logging of Nav Assessments in log and provide proof of logging for 30 days.

Provide proof of latest updates to Electronic Nav system.

repair starboard running light and aft white stern light.

Replace expired Alcohol Screen 02 testing strips.

Replace broken heat detector housing in generator room.
