PSCO observed a valve installed in the sample extraction line. This valve is not included in the vessel's approved piping diagram. The Insulation in the outer shell of the ships boiler is heavily burnt from a possible combustion gas leak. The possible gas leak and the failure of the insulation present a fire hazard to the ship and crew. Vessel does not use boiler underway, crew uses economizer restrict use of boiler until proper operation verified by class.
