Stairway enclosures shall have direct access to the corridors and be of a sufficient area to prevent congestion, having in view the number of persons likely to use them in an emergency. Within the perimeter of such stairway enclosures, only public toilets. lockers of non-combustible material providing storage for safety equipment and open information counters are permitted. Only public areas, corridors, public toilets. special category spaces, other escape stairways required by regulation 28.1.5 and external areas are permitted to have direct access to these stairway enclosures. Items found stowed in Cat 2 stairs, Deck B MVZ 4 & Deck A Fwd Passenger Stairs. 74SOLAS 01/II-2/29.2

Crowns and casings of machinery spaces of category A shall be of steel construction adequately insulated and openings therein, if any, shall be suitably arranged and protected to prevent the spread of fire. FSD TT3.02 locking mechanism was missing allowing for penetration of flame and smoke. 74 SOLAS 01 II-2/23.3

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSSCO observed lifeboat #3 starboard engine would not start. 74 SOLAS 20 III/20.2

Openings shall be provided with permanently attached means of closing which shall be at least as effective for resisting fires as the divisions in which they are fitted. PSCO observed on deck 4 forward portside sliding door FSD 4.05.06 and on dek 8 fire door 8.2.10 not closing. 74 SOLAS 01 II-2/30.1

In addition, routes to muster stations shall be indicated with the muster station symbol, intended for that purpose. Insufficient exit signage in the emergency generator space, by ships main diesel generators and in the main gym that didn't lead occupants to exit. 74 SOLAS 20 III/11.5

Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergency source of electrical power required by regulation II-I/42 or II-I.43. Embarkation light starboard side forward life raft station was extinguished. 74 SOLAS 20 III/11.4

The hull, superstructures, structural bulkheads, decks, and deckhouses shall be constructed of steel or other equivalent material. Category 10 AC spaces behind FD 9.6.0.7 was found to contain numerous combustible storage items. 74 SOLAS 01 II-2/23.1

Stairway enclosures in accommodation and service spaces shall have direct access from corridors and be of a sufficient are to prevent congestion, having in view the number of persons likely to use them in an emergency. Aft crew stair 10 was found to have carts and cleaning supplies in way of escape path. 74 SOLAS 01 II-2/29.2

All bulkheads which are not required to be "A" class division shall be at least "B" or "C" class division. B-15 door 02 kids club was being held open and not maintaining the required structural fire protection boundary. 74 SOLAS 01 II-2/25.1.1

Before the ship leaves port and at all times during the voyage all lifesaving appliances shall be in working order and ready for immediate use. Lifeboat number 4 starboard engine was no operating. The crew was able to repair and start after being identified by the U.S. Coast Guard Team. 74 SOLAS 20 III/20.2

Before the ship leaves port and at all times during the voyage all life saving appliances shall be in working order and ready for immediate use. The port side rescue boat was unable to start due to emergency cutoff being not fully seated. 74 SOLAS 20 III/22.2

07105 - Fire doors in main vertical zone bulkheads and stairway enclosures shall satisfy the following requirements: the doors shall be capable of self closing and be capable of closing against an angle of inclination of up to 35 degrees opposing closure. The following fire screen doors would not latch when tested: 8.4.01, 8.2.06, 8.2.10, and 8.2.5. 74 SOLAS (01) II-2/30.4.1

FIRE DOORS IN MAIN VERTICAL ZONE BULKHEADS AND STAIRWAY ENCLOSURES SHALL SATISFY THE FOLLOWING REQUIREMENTS: A DOOR DESIGNED TO RE-OPEN UPON CONTACTING AN OBJECT IN ITS PATH SHALL RE-OPEN SUFFICIENTLY TO ALLOW A CLEAR PASSAGE OF AT LEAST 0.75 M BUT NO MORE THAN 1 M. PSCO OBSERVED SLIDING FSD 2.3.01 WOULD NOT RE-OPEN UPONG CONTACT WITH AN OBJECT IN ITS PATH. 74 SOLAS(01) II-2/30.4.11 10c

FIRE PROTECTION SYSTEMS AND FIRE-FIGHTING SYSTEMS AND APPLIANCES SHALL BE MAINTAINED READY FOR USE. PSCO OBSERVED FIRE STATION 2.1.2 AND 2.1.1 ON DECK 2 AFT MAIN DINING WERE BEING BLOCKED BY DINING ROOM CHAIRS AND TABLES. 74 SOLAS(20)II-2/14.1.1 10c

FIRE DOORS IN MAIN VERTICAL ZONE BULKHEADS AND STAIRWAY ENCLOSURES SHALL SATISFY THE FOLLOWING REQUIREMENTS: A DOOR DESIGNED TO RE-OPEN UPON CONTACTING AN OBJECT IN ITS PATH SHALL RE-OPEN SUFFICIENTLY TO ALLOW A CLEAR PASSAGE OF AT LEAST 0.75 M BUT NO MORE THAN 1 M. FIRE DOORS IN MAIN VERTICAL ZONE BULKHEADS AND STAIRWAY ENCLOSURES SHALL SATISFY THE FOLLOWING REQUIREMENTS: THE DOORS SHALL BE SELF-CLOSING AND BE CAPABLE OF CLOSING AGAINST AN ANGLE OF INCLINATION OF UP TO 35 DEGREES OPPOSING CLOSURE. PSCO OBSERVED THE FOLLOWING DOORS WOULD NOT LATCH: FSD A.2.08, A.1.04, A.0.10, AND 9.2.12 WOULD NOT LATCH. 74 SOLAS(01)11-2/30.4.1 10c

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board. PSCO observed damage to boiler due to over pressurization and observed damage to boiler piping immediately adjacent to the boiler. 74 SOLAS (04) II-1/26.1

Within the perimeter of stairway enclosures, only public toilets, lockers of non-combustible material providing storage for safety equipment, and open information counters are permitted. Storage rack with combustible storage items were found in stairway C260 under the stairs. 74SOLAS(01)II-2/29.2 50ac 07103

Where the contents and use of a space are such that there is a doubt as to its classification for the purpose of this regulation, it shall be treated as a space within the relevant category having the most stringent boundary requirements. Within the category 10 spaces between frames 186 and 214 in the engine room forward numerous combustible storage items were found. These have no fire suppression and are shown at category 10 spaces on the structural fire protection plan. Flammable items are found in the carpenters workshop not in an approved locker. 74SOLAS(01)II-2/29.2.2 50ac 07101

Structural fire protection, including fire-resistant divisions, and protection of openings and penetrations in these divisions, shall be kept in good order so as to ensure their required performance if a fire occurs. Means of escape system and appliances, shall be kept in good order so as to ensure their required performance if a fire occurs. The emergency exit from the carpenter space was found locked. Corrected on the spot. 74SOLAS(20)II-2/14.2.1.1.3 10c

07105 - Fire Safety - Fire doors in main vertical zone bulkheads and stairway enclosures shall satisfy the following requirements: The doors shall be self-closing and shall have an approximately uniform rate of closure of no more than 40s and no less than 10s with the ship in the upright position. PSCO observed a broken hinge on FSD A.4.20 which prevented proper closure. Additionally, FSD A.1.27, FSD A.1.14, FSD 8.3.02, FSD 9.6.05, FSD 3.1.08, FSD 3.1.11, FSD 10.3.04, FSD 8.3.01, FSD 7.2.08, FSD 7.3.04, and FSD 9.6.04 were not fully closing. Also, the door handle on FSD 9.6.04 is getting caught on door frame, preventing door from opening. 74 SOLAS 01 II-2/30.4.1

07126 - Fire Safety - The arrangements for storage, distribution and utilization of oil used in pressure lubrication systems shall be such as to ensure the safety of the ships and persons onboard. PSCO observed excessive hydraulic fluid leaks in the hydraulic room near side shell door A.3.06. 74 SOLAS 01 II-2/15.3

07105 - Fire Safety - Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, indication shall be provided at the fire door indicator panel in the continuously manned central control station whether each door is closed. PSCO observed indicator light for MVZ FSD 5/W continued to indicate the door was open, despite confirmation the door was closed from on-site personnel. 74 SOLAS 01 II-2/30.4.6

07122 - Fire Safety - General arrangement plans shall be permanently exhibited for the guidance of the ship's officers, showing clearly for each deck the control stations, the various fire sections enclosed by "A" class divisions, the sections enclosed by "B" class divisions together with particulars of the fire detection and fire alarm systems the sprinkler installation, the fire-extinguishing appliances, means of access to different compartments, decks, etc., and the ventilation system, including particulars of the fan control positions, the position of dampers and identification numbers of the ventilating fans serving each section. PSCO observed that the fire plan inaccurately indicated where the fire dampers control unit in fire locker 9.6.2 was located. 74 SOLAS 20 II-2/15.2.4.1

07105 - Fire doors/openings in fire-resisting divisions - Fire doors in main vertical zone bulkheads, galley boundaries, and stairway enclosures, other than power-operated watertight doors and those which are normally locked, shall satisfy the following requirements: the door shall be self-closing and capable of closing against an angle of inclination of up to 3.5 degrees opposing closure. PSCO observed FSD in Raphael Room, deck 3, had paintings obstructing the closing of the door. 74 SOLAS (01) II-2/30 17c

04109 - Fire drills - Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. PSCO observed insufficient evacuation of crew accommodations surrounding the affected space during the fire drill. 74 SOLAS (20) III/19.2.1 17c

04109 - Fire drills - Crew members shall be trained to be familiar with the arrangements of the ship as well as the location and operation of any fire-fighting systems and appliances that they may be called upon to use. PSCO observed main galley, pizzeria, and Good Eats crew members were unfamiliar with ship's manual call points, fire extinguishing equipment, and hi-fog fire suppression system for deep fat fryer. 74 SOLAS (20) II-2/15.2.2.1 17c

11101 - Lifeboats - Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed lifeboat #12 did not start. 74 SOLAS (20) III/20.2 17c

07105 - Fire doors/openings in fire-resisting divisions - Fire doors in main vertical zone bulkheads and stairway enclosures shall satisfy the following requirements: The doors shall be self-closing. PSCO observed FSD B302 did not self-close completely. 74 SOLAS (01) II-2/30.4.1 10c

07105 - Fire doors/openings in fire-resisting divisions - The construction of all doors and door frames in A class divisions, with the means of securing them when closed, shall provide resistance to fire as well as the passage of smoke and flame, as far as practicable, equivalent to that of the bulkheads in which the doors are situated. Such doors and door frames shall be constructed of steel or other equivalent material. PSCO observed FSD 4.14 did not latch when closed. 74 SOLAS (01) II-2/30.2 10c

07120 - Means of escape - The means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicators placed not more than 300 mm above the deck at all points of the escape route, including angles and intersections. The marking must enable passengers to identify the routes of escape and readily identify the escape exits. PSCO observed low location lighting, in the vicinity of WTD #18, did not illuminate. 74 SOLAS (01) II-2/28.1.5.10 10c

11129 - Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed rescue boat suffered difficulty starting, including multiple failed starting attempts over several minutes during launch. 74 SOLAS (2020) III / 20.2 17c

11131 - On-board training in the use of the ship's life-saving appliances,including survival craft equipment, and in the use of the ship's fire-extinguishing appliances shall be given as soon as possible but not later than two weeks after a crew member joins the ship. PSCO observed davit launched liferaft crews lacked sufficient knowledge of launching operations, and various lifeboat crews lacked sufficient knowledge of emergency fuel shutoff locations and emergency steering procedures. 74 SOLAS (2020) III / 19.4 17AC

11101 - Except where otherwise stated, the normal equipment of every lifeboat shall consist of: if the lifeboat is not automatically self-bailing, a manual pump suitable for effective bailing. PSCO observed inoperable means of bailing on lifeboat # 2-4-6-10. 74 SOLAS (2020) III / 34 LSA Code 4.4.8.25 17AC changed to 40AC

11101 - When a wheel or other remote steering mechanism is also provided, the tiller shall be capable of controlling the rudder in case of failure of the steering mechanism. PSCO observed crew of lifeboat 11 and 17 were unable to set up emergency steering due to a corroded wingnut. 74 SOLAS (2020) III / 34 LSA Code 4.4.7.2 17AC changed to 40AC

11129 - Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed lifeboats 14 and 17 would not start. SOLAS 74 (2020) III / 20.2 17AC

15109 - The company should establish and maintain procedures for identifying any training which may be required in support of the SMS and ensure that such training is provided for all personnel concerned. PSCO observed a systematic failure of crewmember knowledge and a lack of proper lifesaving appliance maintenance and operation. Recommend external audit. 74 SOLAS (2020) IX / 3.1 ISM Code 6.5 50AC

07105 - Fire doors in main vertical zone bulkheads, galley boundaries, and stairway enclosures, other than power-operated watertight doors and those which are normally locked, shall satisfy the following requirements: the approximate time of closure for hinged fire doors shall be no more than 40 s and no less than 10 s from the beginning of their movement with the ship in an upright position. PSCO observed the following fire doors did not meet the time requirement for closing: FSD's 8.3.01, 9.3.24, 9.2.16, 10.3.06, 9.3.21, 7.2.04, 7.3.01, 2.3.07, 3.5.15. 74 SOLAS (01) II-2 / 30 40AC

07103 - In addition to complying with the specific provisions for fire integrity of bulkheads and decks mentioned elsewhere in this part, the minimum fire integrity of all bulkheads and decks shall be as prescribed in tables 27.1 and 27.2. PSCO observed pipe penetration with no insulation in A-0 bulkhead. 74 SOLAS (01) II-2 / 27 40AC

PSCO observed the following fire doors reopened completely after contacting an object in their path: FSD's 5.3.08, 6.3.09, 6.3.08, 8.3.08, 9.5.04, 9.2.01, 9.2.17, 10.3.02, 1.1.02 74 SOLAS (01) II-2 / 30 40AC

11118 - On all passenger ships each lifejacket shall be fitted with a light complying with the requirements of paragraph 2.2.3 of the Code. PSCO observed more than 75% of life-jackets that were spot-checked as having inoperable lights. 74 SOLAS (2020) III / 22.3 40 AC

11129 - Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed lifeboat #12 had a emergency fuel shutoff valve for the main engine that was seized shut and they had an oil pressure alarm that was active upon engine startup. 74 SOLAS (2020) III / 20.2 40AC

07103 - The minimum for integrity of all bulkheads and decks shall be as prescribed in tables 26.1 and 26.2. PSCO observed spa reception office (categorized as a category 7 space) has been turned into a private chemical locker (category 13 space) without approval. In addition, PSCO observed A/C room 13A (category 10 space) is being used as a general storage room. 74 SOLAS (01) II-2 / 26 40AC

07120 - A corridor, lobby, or part of a corridor from which there is only one route of escape shall be prohibited. PSCO observed 03 additional office spaces within the engine control room that do not directly lead to escape. 74 SOLAS 2001 II-2 / 28

11129 - Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed the crew being unable to start the engine on lifeboat number 9. 74 SOLAS 2020 III / 20.2

PSCO observed doors F.S.D.2.3.01, F.S.D.1.5.05, and F.S.D.9.6.05, which are classified as openings in an "A" class division as per the vessel's fire plan, not closing completely. 74 SOLAS 2001 II/2 / 30

PSCOs observed light bulbs, queuing line and hand sanitizer stations stand signs, and drums of lube oil within the perimeter of stairway enclosures. 74 SOLAS (01) II-2/29.2

11129 - Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. The rescue boat was not able to start for 10 minutes when tested. 74 SOLAS (01) III/20.2

07103 - The minimum for integrity of all bulkheads and decks shall be as prescribed in tables 26.1 and 26.2. The deck was holed penetrating the deck in the deck 4 AC room. 74 SOLAS (01) II-2/26

09209 - Electrical installations shall be such that all safety of the passengers, crew and ship from electrical hazards. There are cut electrical wires exposed on deck 3 near life raft station D, and the overhead by the 1st tender boat on the portside. Also numerous brackets holding lighting fixtures are missing bolts are held together by zip ties. Additionally, the switchboard in the fresh water valve room had open spaces leading on the board and the doors would not shut properly. There was also exposed wiring on the key pad for the engine room entrance and a missing overhead light with exposed wires on the bulkhead deck. 74 SOLAS (04) II-1/40.1

11124 - Survival craft muster and embarkation stations shall be adequately illuminated. Embarkation lighting between Lifeboats 15 & 17, along with the control station lighting for Lifeboats 16 & 18 is inoperable. 74 SOLAS (20) III/11

14614 - Sulphur content of any fuel oil used on board ships shall not exceed 0.5% m/m on or after January 1, 2020. Upon review of the vessel's engineering logs from 21-26 February, 2020, PSCOs discovered multiple discrepancies with the vessel's exhaust gas scrubber system. There were 13 incidents in which generators were running and the exhaust gas scrubber system was not. Additionally, there were only two locations recorded in the continuous monitoring log for the system, although the ship was underway on a round trip from Baltimore to the Bahamas. MARPOL VI/3.14.1.3

07126- THE CONDITION OF THE SHIP AND ITS EQUIPMENT SHALL BE MAINTAINED TO CONFORM WITH THE PROVISIONS OF THE PRESENT REGULATIONS. PSCO OBSERVED OIL LEAKS AND ACCUMULATION THROUGHOUT THE ENGINE ROOM TO INCLUDE SPACES SURROUNDING THE GENERATORS AND THE FORWARD SEPARATOR ROOM. 74 SOLAS (14) I/11(a)

07126- OIL FUEL LINES SHALL BE SUITABLY PROTECTED TO AVOID OILSPRAY OR OIL LEAKAGES ONTO HOT SURFACES. [name withheld] THROUGHOUT THE ENGINE ROOM WITH TORN OR MISSING INSULATION AS WELL AS OIL SOAKED LAGGING ON MULTIPLE LINES. 74SOLAS(01) II-2/15.2.11

07105 - FIRE DOORS SHALL BE CAPABLE OF RELEASE FROM A POSITION AT BOTH SIDES OF THE DOOR. SLIDING FIRE DOORS ON DECKS 5 AND 6 IN THE MIDSHIPS PASSENGER STAIRWAY DID NOT SHUT UPON ACTIVIATION. 74SOLAS(01)II-2/30.4.3

11118 - Lifejackets shall be so placed as to be readily accessible. Childrens lifejackets in the Deck 5 Childrens Care Space were stowed in a closet behind boxes adn were not readily accessible. 74 SOLAS (14) III/7.2.2 10c

04114 - THE CONDITION OF THE SHIP AND ITS EQUIPMENT SHALL BE MAINTAINED TO CONFORM WITH THE PROVISIONS OF THE PRESENT REGULATIONS.
