Vessel SMS states in "Operations in (extremely) cold climates" to drain fire mains (pipelines). Also that exposed pipelines should be insulated. Fire main froze due to water trapped after improper cleaning. Ship's SMS has no procedure to drain all water to lowest point. Additionally, piping is not insulated. Given objective evidence observed in deficiency 05 and 06, an external audit of the ship's SMS is recommended.
