At each initial and subsequent inspection for certification the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. STBD forward rails on the second deck have excessive play. The upper weather deck area to starboard of the stairwell must be inaccessible to passengers until satisfactory repairs are verified by MI. 46 CFR 176.25-50(a)

The discharge ends of vent pipes shall be fitted with removable flame screens or flame arresters. Fuel vent not removable. 46 CFR 182.20-35(d)

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material. Both lights in engine room missing covers. 46 CFR 183.410(a)

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information:(1) Date of the drill and training; and(2) General description of the drill scenario and training topics. No fire drill has been logged/conducted since July of 2025. Provide fire drill documentation to the satisfaction of MI. 46 CFR 185.524

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. All trash cans on board were unapproved plastic receptacles. 46 CFR 177.405(f)

A suitable marine type strainer, meeting the requirements of the engine manufacturer, must be fitted in the fuel supply line in the engine compartment. Fuel filters contained excessive amounts of sediment, not maintained IAW manufacturer's instructions. 46 CFR 182.455(b)(6)

Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Flexible hoses in engine room being solely supported by zip-ties. 46 CFR 176.800(a)

The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support. Wires in tank space and bar void are being held up strictly by zip ties. 46 CFR 183.340(b)(4)

Unless otherwise stated in this section, each item of equipment carried, whether required under this section or not, must be secured by lashings, stored in lockers, compartments, brackets, or have equivalent mounting or storage arrangements. Lifebuoy floating water light bracket is not secured. 46 CFR 180.175(a)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. Hole found in the port side aft corner of engine room overhead. Vessel made temporary repair using J-B weld to the satisfaction of the [name withheld] permanent repairs during next dry dock by crop and renewing the section of the hull that has the temporary patch to the satisfaction of the MI. 46 CFR 176.802 (a)(1)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. Hole found in the port side aft corner of engine room overhead. Submit repair proposal and make repairs to the satisfaction of the MI. 46 CFR 176.802 (a)(1)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment and systems, including the following: Inspection of each life jacket, work vest, and marine buoyant device. Life ring found to be in unserviceable condition, with wasted grab lines. Replace life ring and grab lines to the satisfaction of the MI. 46 CFR 176.808 (a)(2)

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew and other person and the vessel from electrical hazards. Heavy chafing from engine supports on two start cables on each engine. Replace all four cables. 46 CFR 183.200

At each initial and subsequent inspection for certification of a vessel, the owner or operator shall be prepared to conduct tests and have the vessel ready for inspection of machinery...including the following: Inspection of all machinery essential to the routine operation of the vessel including generators and cooling systems. Large non metallic hose sitting on top of the strainer associated with the starboard main was chafed on the underside and requires replacement. 46 CFR 176.804

Receptacle outlets of the type providing a grounded pole or a specific direct current polarity must be of a configuration that will not permit improper connection. More than five power strips found throughout the vessel that were loaded with electrical connections. Remove power strips or provide electrical plans for review for permanent electrical connections. 46 CFR 183.220

At each initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and shall conduct the following tests and inspections of the hull structure and its appurtenances: inspect all railings and bulwarks and their attachment to the hull structure. Rail above stairwell, and starboard rail for cabin door to forward weather deck, on 2nd deck found to be loose. Repair to the satisfaction of the MI. 46 CFR 176.25-5(a)(4)

At each initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and shall conduct the following tests and inspections of the hull structure and its appurtenances: inspect all railings and bulwarks and their attachment to the hull structure. Main deck, Forward, starboard side, plastic side paneling has crack underneath black duct tape. Replace side paneling to the satisfaction of the MI. 46 CFR 176.25-5(a)(4)

Machinery and fuel tank spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors. Engine room access door was not vapor tight. 46 CFR 177.405

In systems and applications where flexible hoses are permitted to be clamped: Double hose clamping is required where practicable. Flexible hose for generator exhaust had one hose clamp near the starboard side shell. 46 CFR 182.410

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Dead end wires found on second deck cabin starboard side near the weather deck door. 46 CFR 183.200(c)

These regulations prescribe the minimum standards, procedures, and means to be used to test for the use of dangerous drugs. Vessel was not able to provide all necessary records and paperwork to complete DAPI Audit. Send all required documents for MI to perform satisfactory DAPI Audit. 46 CFR 16.101(b)

The shutoff valve at the tank must be operable from outside the tank compartment preferably from an accessible position on the weather deck. Fuel shutoff valve did not close completely when crew pulled remote handle. After MI exercised the valves locally at the tank, crew was able to successfully close the valves completely from the remote handle.

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection its fire protection equipment, including the following: Operation of the fire main system and checking of the pressure at the most remote and highest outlets. Threaded pipe connection 1' above fire pump was covered in temp patch and when removed showed a slow weep. Affect permanent repairs that do not include the use of temporary patches.

Lamp, paint, and oil lockers and similar compartments shall be constructed of metal or lined with metal. Marine inspector observed 5 plastic totes in main deck accommodation space containing paint, primer, and glue. 46 CFR 177.10-5(c)

At each initial and subsequent inspection for certification, the marine inspector...shall conduct the following tests and inspections...examine all weathertight closures. Marine inspector found the port side aft door to accommodation space to be missing a doorknob and not weathertight. 46 CFR 176.25-5(a)(5)

1) Repairs or alterations to the machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable. The vessel experienced a temporary loss of propulsion on the vessel's port ME and conducted repairs without the approval of the [name withheld] to submit repair proposal and undergo visual inspection prior to vessel's return to commercial service. 46 CFR 176.700 (a)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces: (1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette. Lazarette high level visual alarm is inoperable and must be made operable to the satisfaction of the attending MI.

Exhaust piping shall be led to the point of escape with a minimum number of bends or elbows. It shall be gas-tight throughout its length and shall be properly supported by non-combustible hangers or blocks. STBD engine exhaust pipe is mounted with combustible blocks and is unsecured. Generator exhaust pipe is secured to combustible blocks. Effect repairs to the satisfaction of the attending marine inspector. OLD T 46 CFR 182.15-20(a)

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. Original Certificate of Documentation must be provided.

By March 15 of the year following the collection of the data in paragraph (a) of this section, marine employers must submit the data on the form titled U.S. Department of Transportation Drug and Alcohol Testing MIS Data Collection Form. MIS Report for 2022 was not available and must be provided to the attending MI.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Ophthalmic eyewash solution is expired and must be replaced.

The provisions of Part III of Title III of the Communication Act require US vessels which transport more than six passengers for hire, while being navigated on any tidewater, to carry a radiotelephone installation. Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician once every five years. Vessel's Safety Radiotelephony Certificate was not onboard the vessel.

Unless a splice is made by an insulated pressure wire connector listed by UL, Inc., it shall be thoroughly soldered, taped first with rubber and then with friction tape, or the soldered joint shall be otherwise protected as to provide an insulation the same as that of the conductors joined. Electrical connections above the freshwater tank and in lazarette near the hydraulic piping must be enclosed with marine grade electrical connectors. OLD T 46 CFR 183.05-50(b)(2)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests including the following: Inspection of all machinery essential to the routine operation of the vessel including generators and cooling systems. Exhaust from both main engines had slow leak at the base of non metallic connection joint when operating at higher RPMs.

Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces. Two limber holes present in engine room/lazarette bulkhead that need to made vapor tight.

All vessels must undergo an annual inspection within the 3 months before or after each anniversary date. Vessel has not been scheduled within the annual inspection time frame. 46 CFR 176.500(a)(1)

One length of hose shall be provided for each hydrant required. Main deck fire hose had broken fitting during drill. 46CFR 181.15-10(a)

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the USCG. No log entries provided this inspection for drills or training.

Prior to departing on a voyage the passenger count must be communicated verbally or in writing ashore at the vessels' normal docking location or with a designated representative of the owner. When asked, no records or plan was found to communicate to USCG representattive (owner has online billing system which accounts passengers, table and service sales each as a number transaction but not able to communicate number of passengers onboard).

Generators and motors shall be mounted as high as practible above the bolge to avoid damage by splash or contact with low lying vapors. 110V CO2 mixer motor and wiring is mounted on bilge bottom. 46CFR 180.10-5(c)

Each ring buoy light must, unless limited to daytime operations, carry one floating waterlight with a lanyard 3' in length by a corrosion resistant clip. Vessel ring buoy floating light inop. (restricted to daytime operations).

Penetrations and openings in watertight bulkheads must be kept as high and as far inboard as practicable and have means to make them watertight. MI noted penetrations in #2 void (top bulkhead leading to #3 void) not watertight. Provide temporary repair to make watertight. 46 CFR 13-1 (OLD-T)- 46 CFR 171.114

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to protection against fire. Discovered alcohol storage in cardboard boxes on carpeted pilothouse deck. Remove the alcohol or remove the carpet.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: (4) Inspection of all railings and bulwarks and their attachment to the hull structure; (5) Inspection to ensure that guards or rails are provided in dangerous places; (6) Inspection and operation of all weathertight closures above the weather deck and the provisions for drainage of sea water from the exposed decks; and (7) Inspection of all interior spaces to ensure that they are adequately ventilated and drained, and that means of escape are adequate and properly maintained. (b) Deck rails must be designed and constructed to withstand a point load of 91 kilograms (200 pounds) applied at any point in any direction, and a uniform load of 74 kilograms per meter (50 pounds per foot) applied to the top rail in any direction. Marine Inspectors witnessed several areas of the upper deck exterior bulwarks to be cracking and daylight was evident. Prove the safety of the bulwark/railing installation in the interior upper deck space.

On a vessel of at least 7.9 meters (26 feet) in length, a visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Identified labeling discrepancies between pilot house bilge alarms and bilge pump spaces. Ensure that labeling at the pilot house matches labeling system used at the bilge manifold.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment and electrical shock. Marine Inspector found numerous dead-end wires/cables in the lazarette, engine room, and compartment number three.

Non-conducting mats or grating must be provided on the deck in from of the vessel’s switchboard. Vessel is missing non-conducting mat or grating in front of its switchboard.

Each portable fire extinguisher on a vessel must be of an approved type. Marine Inspector found that the fire extinguishers' brackets found onboard were too large for the extinguishers, provide approved brackets.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Identified excessive use of extension cords and daisy chaining on the first and second deck.

Electrical equipment exposed to the weather must be watertight. Discovered electrical plugs on the exterior second deck currently used for speakers that are exposed to the weather.

A bilge pipe in a vessel of not more than 19.8 meters (65 feet) in length must be not less than 25 millimeters (1 inch) nominal pipe size. A bilge pipe in a vessel of more than 19.8 meters (65 feet) in length must be not less than 40 millimeters (1.5 inches) nominal pipe size. A bilge suction must be fitted with a suitable strainer having an open area not less than three times the area of the bilge pipe. Bilge strainer in the lazarette is corroded and no longer serves its purpose as a strainer.

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. Found expired COD posted in pilothouse and no current COD onboard.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Observed alcohol bottles as potential hazard on first and second deck bars as they were not secured in cabinet by any lip or means to prevent bottles from falling.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. During a routine records review, Marine Inspectors noted several discrepancies on vessel's Simplified Stability Test Form (dtd 4/3/2014). Vessel rep shall schedule an inspection for Marine Inspectors to conduct measurements of vessel for stability and passenger count verification.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date:Charts of large enough scale to make safe navigation possible. updated charts were not present. 46 CFR 184.420.

High water alarm in number 2 void was not secured, allowing it to raise with the water level. 46 CFR 182.530

Visible audible alarm was not lighting up for compartment space #3.

provide and install mettallic supports for wiring above port main engine at 24 inch intervals currently held in place by plastic ties.
