Vessel’s life-saving equipment stowage locations across the vessel in watch stations and passageways had appropriate symbols but was missing number of devices indicated for each stowage location.

Vessels various oxygen, acetylene, and refrigerant R-134 cylinders were stowed outside of brackets via ratchet strap configurations directly on the deck. Move DOT cylinders into storage brackets.

a) Simple, complete operating instructions must be conspicuously located at or near any release station and in the extinguishing agent cylinder storage room.
(b) On a system in which extinguishing agent cylinders are stored outside the protected space, operating instructions must also:
(1) Include a schematic diagram of the system; and
(2) Describe alternate methods of discharging the extinguishing agent into protected spaces should the manual releases or stop valve controls fail to operate.
NOVEC fixed firefighting system protecting the generator space had operating instructions posted at the release station and the cylinder bank for remote release. Operating instructions posted included did not address if remote failure activation occurred how to operate manually. In OEM’s manual, manual activation operation discussed “actuation can also be initiated at the pilot cylinder location by manually operating the pressure actuated stop valve and operating the lever releases located on the nitrogen cylinder valves.” Include operating instructions to describe alternate methods of discharging NOVEC fixed firefighting system.

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Replace or repair the following to the satisfaction of the Class surveyor:
1) Repair last bottom rung on at starboard bridge wing (04-03)
2) Replace missing vent for stbd side anti-roll tank vents.
3) Repair stepladder on port side (04-03) with wastage and replace damaged ground cable with new.
4) Replace missing screen on sewage flange.
5) Repair ground cable controller box (NMIS OPS) on 01 deck.
6) Repair forward line locker with missing dogs and wasted bracket for pin for holdback. Pending repair to bracket, implement safety measures to prevent injury to persons when entering/exiting space through line locker hatch.
7) Replace missing bolt on ballast vent in way of starboard anchor windlass.
8) Replace missing vent on gooseneck on forward port freshwater fill line.
9) Repair and prove operation of paint locker exhaust fan light.
10) Replace missing hinge bolts on wet lab door.
11) Repair and properly reconnect wires in controller power box on starboard bridge wing to prevent from chaffing and secure weather deck mats to prevent further damage to cables when mats are stepped on.
12) Repair and prove operation of sound powered phone on starboard bridge wing.
