Vessel during UWILD discovered two hull breach holes in the wash water tank. Vessel effected temporary repairs with exterior puddy and internal Belzona and welded strong backs. Vessel must make permanent repairs to wash water tank by next drydock. Vessel must implement mitigation plan approved by USCG/ABS until permanent repairs completed.

Vessel must conduct hull gauging by intended departure date of 4-JAN-2026 on the following areas (with ABS approved gauger). --a. Forepeak tank had excessive scaling in the forward three baffles. Conduct hull gauging in the forward 3 baffles and man hole cover and sample remaining tank bottom plating and side shell (50 UT shots minimum). -- b. Machinery spaces (OT 3/8”) Conduct hull gauging of the bottom plating in the rudder room, aft thruster tunnel, aux generator space, and main engine compartment (200 UT shots minimum). --c. 6W-S Ballast Tank between stringers 8-9 must clean pitted area and gauge wastage.

All safety and relief valves for use on pressure vessels or piping systems must be designed to meet the protection and service requirements for which they are intended and must be set to relieve at a pressure which does not exceed the “maximum allowable working pressure” of the pressure vessel or piping system. During pressure relief valve testing, PRVs with lifting pressure setpoint of 150 psi failed to lift at set pressure. #1 PRV lifted at 190 psi, #2 PRV lifted at 175 psi, #3 did not lift by 200 psi (cut off due to PV MAWP of 200 psi), unable to test #4 PRV due to pressure gauge blockage. Vessel must replace PRVs for #1-#4 and have documented satisfactory bench tests. Vessel must install and verify PRV satisfactory operation from local pressure gauges at designed lifting pressure (iaw PV system drawing).

Vessel’s battery banks had twist on wing nuts for wire connections. One of vessel's emergency batteries did not have a weather-tight cover. Replace wire connections with configuration to include a non-rotating follower or self-tightening. Replace cover for emergency battery in space under vessel bridge. Remove painted/blocked ventilation cap next to #1 SSDG battery bank.
