Code: 04118 Each enclosed space entry & rescue drill shall include: checking & use of personal protective equipment required for entry, use of communication equipment & procedures, use of instruments to measure the atmosphere in enclosed spaces, and use of rescue equipment and procedures. PSCO observed the vessel's crew have not been properly conducting enclosed space entry and rescue drills IAW SOLAS and the vessel's SMS. Drills are being carried out in conjunction with fire drills, and the required equipment for the enclosed space entry and rescue drill is not being utilized. The feedback unit for controling the steering pumps is made up of four limit switches each actuated by a small plastic cam. The crew found all four plastic cams were loose causing the limit switches to stop hydraulic fluid flow.
