Vessels on a coastwise route must have interconnected fire detection; the smoke detector in the passenger accommodation space is not functioning properly. 46 CFR 181.405

Each vessel must have a first aid kit with the proper contents IAW ISO 18813; the existing kit is missing some items and other items are expired.

The vessel must be equipped with an alcohol testing device; the alcohol swabs are expired.

Each life float shall have a waterlight attached around the body of the life float by a 3/8 inch lanyard, resistant to deterioration from ultraviolet light, and at least 18 feet in length. The lights were not attached to the floats as required. (46 CFR 180.175)

The life float painter must be attached to the painter connection or around the body of the device. The painter was not properly attached as required. (46 CFR 180.137)

An interconnected detection and alarm system must consist of multiple-station smoke detectors listed by an NRTL, or independent laboratory as meeting UL 217. The detectors were not operational due to missing batteries.

A vessel on a limited coastwise route must carry six hand red flare distress signals and six hand orange smoke distress signals; hand red flare distress signal may be substituted for the hand orange smoke distress signals. The vessel is missing (4) distress signals. (46 CFR 180.68)

The vessel must provide a minimum of (9) child size life jackets or such greater number as necessary to provide a life jacket for each person that is smaller than the lower size limit of the adult life jacket. The vessel does not have the required number of child life jackets and is restricted to the carriage of (4) children. (46 CFR 180.71)

All vessels must have navigation lights that are in compliance with the applicable sections of the Inland Navigation Rules. The mast head light is not operational, the vessel is restricted to day time operations. (46 CFR 183.420)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) (ISO 18813) or a kit with equivalent contents and instructions. The existing kit does not have the proper inventory and some items are expired. (46 CFR 184.710)

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. The vessel is missing (1) light and the other light does not work. (46 CFR 183.430)

Flexible hose must be fabricated with an inner tube and a cover of synthetic rubber or other suitable material reinforced with wire braid. The fuel return hose going to the fuel oil cooler on the starboard main engine is deteriorated and the outer cover is compromised. (46 CFR 182.720)

A hatch cover must have securing devices; and be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. The hatch cover to the lazarette does not have a securing devise. (46 CFR 179.360)

When operating a vessel on a body of water where the discharge of untreated sewage is prohibited by the Environmental Protection Agency the operator must secure each Type III device in a manner which prevents discharge of untreated sewage. Acceptable methods of securing the device include—(1) Closing the seacock and removing the handle;(2) Padlocking the seacock in the closed position;(3) Using a non-releasable wire-tie to hold the seacock in the closed position; or(4) Locking the door to the space enclosing the toilets with a padlock or door handle key lock. The sewage tank seacock was found to be unlocked and open. (33 CFR 159.7)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew; vessel’s analgesic medication is expired.

Vessel’s alcohol test strips are expired.

Vessel must have onboard updated Tide Tables for the are of operation; tide table is not onboard.

REMOVE AFT DECK IWO LAZARETTE FOR INSPECTION OF STRINGERS BEFORE REPLACEMENT IN KIND.

An adult life jacket must be provided for each person carried on board a vessel; Vessel is limited to the carriage of 83 Adult passengers until 17 Adult life jackets are replaced.

Each survival craft onboard a vessel must be in good working order; vessel's 04 life floats are in need of reconditioning during next drydock in Feb 24.

All engine exhaust pipes must be water cooled; vessel's port engine exhaust was leaking water into lazarette due to loose hose clamps.

Each ring lifeboat on a vessel must be approved IAW 160.050; vessel’s life ring has deteriorated retro tape.

Fuses must be of the cartridge type only and listed by UL; Two blade type fuses were noted under the console in the pilot house.

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operations; visual alarm is not operational in pilot house

A vessel must have navigation lights that are in compliance w/ the International and Inland Navigations Rules; vessel’s anchor light is inoperable.

Each vessel must be equipped w/ at least two portable lights; vessel does not have portable lights in pilot house or ER as required.

A life jacket must be provided for all persons onboard; vessel is restricted to the carriage of 84 adult pax.

An adult lifejacket must be provided for each person carried on board a vessel; 05 lifejackets were noted in disrepair. Vessel's pax count will be reduced to 80 adults until lifejackets are replaced.

Each vessel that operates on the high seas must have an EPIRB installed to automatically float free and operate; the existing EPIRB is not operational and was transmitting signal at the time of inspection.

An adult life jacket must be provided for each person carried on board a vessel; Vessel had 29 Life jackets in a non-serviceable condition. Vessel is restricted to 65 total persons.

Each life jacket carried on a vessel on a coastwise route must have a light attached; a number of lights were not operational.

Each life float and buoyant apparatus must be stowed as required under this section; The strength of the float-free link and the strength of the painter under §180.175(f)(3)(ii) must be determined by the combined capacity of the group of life floats and buoyant apparatus; Vessel's Float Free links are not rated for the two sets of 22 person life floats installed.

The name of the vessel and capacity must be marked or painted in clearly legible letter and numbers on each life float; the existing marking are faded.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions; instructions were not available.

An inspection for renewal of a COI is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and complies with the applicable regulations; the port rudder tube base plate is cracked where it is mounted to the hull allowing the ingress of water.

All vessels must have navigations light in compliance with the Inland Navigation Rules; the vessels side lights are not operational.

Each vessel subject to the Communications Act must have an inspection of the radio installation by an FCC Technician and have issued a FCC Safety Radio Certificate; the existing certificate is expired.

The pipe nipple from the sea valve to the flex hose for the generator cooling water is wasted. Replace nipple.

The metallic flashing for the boundary between the machinery and accommodation space is corroded. Replace with acceptable metal.

4 child and 7 adult lifejackets were taken out of service due to unserviceability, passenger count reduced to 90 adults and 6 children.

The port upper lifefloat sea painter eye is dry rotted. Place a 3/8" UV resistant line around the body of float and reattach painter line.

Where clamps are permitted for flexible hose they shall not rely on spring tension. Replace spring tension clips on generator.

Batteries for float lights must be changed annually IAW manufacturers instructions. The 4 float light batteries have not been changed.

Prove proper operation or replace SSB radio. As an alternative a equivalence from the FCC for a sat phone can be obtained. Vessel restricted to 20 miles

Replace 10 adult and 3 child type I PFD's. PFD's are no longer serviceable. Ensure vsl name is marked on replacement and light attached. Passenger count reduced to 87 adults and 7 children.

Replace hydrostatic release on EPIRB.

Reweld guide rings for both life float paiter coils.

Replace items in first aid kit as indicated on provided inventory.

Replace wing nuts on boattery under console with standard nuts.

Replace speaker wire under console that is being used as power wire. Use UL marine cable.

Remove plastic petcock on generator fuel filter. Install plug, 1/4 turn valve, or approved petcock.

Properly terminate dead end wires on engine room.

Provide documentation for newly replaced petroleum hose in engine room. Hose must meet SAE J-1942.

Renew Certificate of Documentation.

Properly terminate four dead end wires under the bridge console.

Remove excess conductors from the battery and install a bus bar.

Replace the port and stbd navigation lights due to faded lenses.

Replace the life line on the top port life float due to deterioration.

Replace the painter on the bottom port life float due to deterioration.

Replace the platform retaining line on the top stbd life float due to deterioration.

Provide the vessel with the Notice to Mariners.

Provide the vessel with a current chart.

Replace the missing globe on the light fixture in the lazarette.

Bring the vessel into compliance with the FCC regulations regarding the requirement for a Class "A" DSC capable VHF radio or apply the an exemption through the FCC as discussed.
