At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery,. . . including the following: Operational test and inspection of engine control mechanisms including primary and alternate means of starting machinery. During inspection, vessel's starboard main diesel engine (MDE)could not be started from the pilothouse. Vessel staff determined a faulty solenoid was the issue, it was replaced during the inspection, and engine could once again be started from the pilothouse.

If a vessel is not equipped with a single source of power for emergency lighting, it must have individual battery powered lights which have sufficient capacity for a minimum of 2hrs of continuous operation. Battery for emergency light in second deck fire stairwell failed to power light for more than a few seconds. Replace battery in emergency light.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Port shaft separated at the coupling immediately forward of the shaft seal allowing shaft to slide back and water ingress. Affect temporary repairs to mitigate potential water ingress via shaft seal and prepare corrective action/repair plan to correct and mitigate potential for recurrence. Notify attending marine inspector for attendance of plan development @ [phone withheld].

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. Emergency lighting located above COI not working. Repair/replace light.

The owner, managing operator, or a qualified servicing facility as applicable shall conduct the following inspections and tests:(1) For portable fire extinguishers, the inspections, maintenance procedures and hydrostatic pressure tests required by Chapter 4 of NFPA 10, with the frequency specified by NFPA 10. Fire extinguisher on main deck missing inspection label, tamper seal missing, and crew has not weighed extinguisher. Complete maintenance in accordance with NFPA 10. -Check at least the following items: (a) Location in designated place (b) No obstruction to access or visibility (c) Operating instructions on nameplate legible and facing outward (d) Safety seals/tamper indicators not broken or missing (e) Fullness determined by weighing or "hefting" (f) Examination for obvious physical damage, corrosion, leakage or clogged nozzle (g) Pressure gauge reading in operational range (i) HMIS label in place

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Eyewash in first aid kit expired. Replace eyewash with new.

At least one ring buoy must be fitted with a floating waterlight, unless the vessel is limited to daytime operation, in that case no floating waterlight is required. Floating water light on stern inoperable. Repair as required.

46 CFR 182.05-1 (OLD T): Propulsion machinery shall be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements. The port shaft of the vessel came uncoupled. Repair port shaft.

Approximate 4" crack found in fore peak space in vicinity of keel. Vessel not allowed passengers until repairs are made. Affect repairs IAW NVIC 7-68. While in transit to shipyard: -Conduct hourly rounds of space -Maintain dewatering pump in vicinity of space -Notify MSU Pittsburgh duty Marine Inspector upon departure from current location, arrival at shipyard, and of any change in vessel condition at [phone withheld].

Grab line. The grab line shall be 3/8-inch diameter polyethylene, polypropylene, or other suitable buoyant type synthetic material having a minimum breaking strength of 1,350 pounds. Life buoy grab line aft of pilothouse wasted from environmental exposure. Repair/replace as required.

If your vessel's Certificate of Inspection is renewed less often than annually, the marine inspector must check the settings of the safety or relief valves on all pressure vessels, except cargo tanks, twice every 5 years. No more than 3 years may elapse between any check and its immediate predecessor. No documentation available for calliope air compressor relief valve testing. Test/replace as required.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, flooding, heavy weather, or man overboard conditions. Missing from pilot house. Replace as required.

A station bill must be posted by the master on a vessel of more than 19.8 meters (65 feet) in length having: (1) Overnight accommodations for more than 49 passengers; or (2) A Certificate of Inspection requiring more than four crew members at any one time, including the master. Station bill missing from pilothouse. Replace as required.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. A 1/2" hole was found on the bottom of the hull outboard of the stuffing box of the shaft log. Prior to carrying passengers, dry dock vessel and make ready for Coast Guard inspection to determine extent of hull damages.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. A 1/2" hole was found on the bottom of the hull outboard of the stuffing box of the shaft log. Inspection completed showing no further damage. Crop out and renew to original condition a section no less than 18" x 18" IAW NVIC 07-68. Complete NDT of repairs in the presence of CG Marine Inspector.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Electrical distribution panel in engine room missing breaker/blank and is open to accidental personnel contact. Install breaker or blank to prevent accidental contact to energized parts of distribution panel.

Rugs and carpets are not permitted in machinery spaces. Rug found at engine room entrance. Remove from machinery as required.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Highly flammable aerosol containers are being stored in the engine room, remove as required.

The 40-B:C portable fire extinguisher located in the 2nd deck concession area installed in wrong bracket. Provide approved bracket for extinguisher type.

Several portable fire extinguishers located throughout vessel have loose mounting brackets as they attach to stuctures/mounting surfaces including the brackets cylinder capture devices. Make all loose fire extinguisher brackets that attach to structures and mounting surfaces tight and make tight all bracket cylinder capture devices.

Severe internal bottom hull plating wastage noted along both sides of the CVK in Void #1 - biler Space. Provide internal hull thickness readings in affected area as identified by the attending CG MI and submit a proposal to mitigate and arrest any further corrosion.

Repair leaking valve in hot water heating system boiler. Prior to operating boiler.

Replace deteriorated grab line on stbd side lifebuoy.

Replace deteriorated waterlight lanyard.

Replace deteriorated grab line on 2nd deck lifebuoy.

Properly plug holes in both hot water boiler exhaust pipes.

Properly install exhaust pipe lagging on both boilers to cover piping completely.

Test safety relief valves on both hot water heating boilers boilers.

Properly secure all overhead wiring runs as identified by the attending CG MI.

Provide proof of annual VGP compliance verification.

Proove proper operation oof fwd mast head and stern lights.

Provide proper/approved fire extinguisher bracket for BI located in pilothouse.

Update Fire Control Pln to reflect equipment installed on vessel

Make permanent repairs to high water float alarms in Engine Room, Forepeak, and Beverage Space. Floats had come off the mounts.

Cable penetrations through watertight bulkheads required to be sealed in rudder room and beverage spcece.

Repair fuel shut off valve to port boiler to enable complete closing of the valve from remote shutdown.

Provide permanant cover over junction box located behind port boiler.

Repair lettering on port bow deck life ring.

Prior to seasonal service conduct required boiler tests and inspections to the satisfaciton of the CG Marine Inspector
