A survey shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel representative reported a 4” crack was found in the starboard side rake void at the 8'6" draft mark. Vessel rep reported that this crack appears to have been from an impact causing a slight inset(See pictures in documents). Vessel rep applied a temp repair utilizing epoxy on both sides of crack. Vessel will transit directly to Calvert City, KY to discharge cargo and then dry-dock in Paducah, KY for inspection and repairs. While underway the following precautions will be made: - Have a dewatering pump on standby - Conduct regular sounding of space to monitor water level and log in towing vessel record. Once in drydock, the vessel operator must submit a repair proposal containing permanent repair plans.

Transfer procedure required must contain a description of each transfer system on the vessel including: a line diagram of the vessel's transfer piping, and a description of and procedures for emptying the discharge containment system. Vessel did not have a line diagram for their transfer piping and 02 valves are not properly labeled nor identified in the procedures. 33CFR155.750(a)(2)

During a pre-loading inspection, water was discovered in the forward void. Space was de-watered and a bottom plate hull fracture was discovered IWO the aft bulkhead. Fracture is approximately 4" long and is associated with an approximate 6" inset. Fracture was temporarily repaired with oakum, wood wedges, and epoxy. Vessel must proceed, empty with no cargo, to a haul-out facility within the MSU Pittsburgh OCMI zone, and placed on blocks where a full watertight, structural, and stability survey be performed. Once survey is completed and repair proposal is developed it shall be submitted to the MSU Pittsburgh OCMI for review and acceptance prior to completing permanent repairs.
