33CFR151.10 (b) When within 12 nautical miles of the nearest land, any discharge of oil or oily mixtures into the sea from a ship other than an oil tanker or from machinery space bilges of an oil tanker is prohibited except when all of the following conditions are satisfied— (1) The oil or oily mixture does not originate from cargo pump room bilges; (2) The oil or oily mixture is not mixed with oil cargo residues; (3) The oil content of the effluent without dilution does not exceed 15 ppm; (4) The ship has in operation oily-water separating equipment, a bilge monitor, bilge alarm, or combination thereof as required by part 155 subpart B of this chapter; and (5) The oily-water separating equipment is equipped with a 15 ppm bilge alarm; for U.S. inspected ships, approved under 46 CFR 162.050 and for U.S. uninspected ships and foreign ships, either approved under 46 CFR 162.050 or listed in the current International Maritime Organization (IMO) Marine Environment Protection Committee (MEPC) Circular summary of MARPOL 73/78 approved equipment.

26DEC24: Received a duty call on the Small passenger vessel "T" Monster wth a report of hitting the south jetti on Savannah River with 34 passengers onboard and 2 crewmembers. Vessel dropped anchor when engines shutdown. Master tried starting both engines before realzing the engines were still clutched in. After throttles were placed back in neutral postion, engines were re-started. Master and crew verified vessel was not taking on water and headed back to dock. Master reported minimal vibration on starboard engine on return trip. Passeners disembarked vessel and owner [name withheld] will conduct diving operations on 27DEC24 to identify damage. Master thinks that the starboard propeller may have been damaged. Issued no sail code 701c CG-835V. (01) Deficiencies issued (00) Deficiencies cleared (01) Deficiencies remain outstanding

Replace retro-reflective tape on (2) ring life bouys

Re-route control cable away from belt pulley on forward port engine.

Provide means to connect and hold open stbd side engine room emergency vantilation closure flap.

Re-connect ground cable on stbd side battery box that was disconnected or remove if no longer necessary.

Secure all engine room wireing with proper cable clamps IAW USCG standards. Also remove any engine room cable clamps no longer in use.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. The starboard engine secondary fuel filter split causing a loss of propulsion, repair in kind to the satisfaction of the cognizant OCMI prior to the carriage of passengers.

47 CFR 80.411:COMMS ACT CERT: 01199 Provide a Communications Act, Safety Radiotelephony Certificate in accordance with 47 CFR 80.411 to the satisfaction of an attending Coast Guard Marine Inspector. The vessel will be restricted from carrying passengers after 9SEP2023 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

47 CFR 80.59 Vessels radio installation certificate is expired.

182.720(e) (2) Flexible hose may be used provided it is fitted with suitable connection fittings and has high resistance to saltwater, petroleum oils and vibration. Port side fuel supply hose in fuel tank space is worn and needs to be replaced.

46 CFR 176.25-35 (a) The STBD rudder upper bearing support housing was found split.

176.25-5(a)1 In the lazarette a deck support 2x4 on the port side just off centerline was found split causing the deck to flex in the area when walked on.

Diesel fuel flexible hose shall be fabricated with an inner tube and a cover of synthetic rubber or other suitable material reinforced with wire braid. Vessel utilized approximately 33 feet of electrical tape as a soft patch on a diesel fuel flexible hose on the starboard engine in the engine room. Upon removal of the tape, the wire braid was showing (roughly the size of a quarter) and the hose was leaking a solid stream of diesel. 46CFR182.20-40 (a)(2)(ii)

Vital systems on mechanically propelled vessels which carry more than 49 passengers shall comply with the provisions of Subchapter F of this chapter. Vessel utilized electrical tape as a soft patch on a metallic raw water pipe on the starboard engine in the engine room. Upon removal of the tape, a solid stream of raw water was leaking in to the bilge. 46CFR182.40-5 (b), 46 CFR 56.10-1

At each initial and subsequent inspection for certification the marine inspector shall examine and test….The means provided for pumping bilges. Vessel rep could not properly demonstrate operational test of individual bilge lines and bilge suctions. 46 CFR 176.25-10(a)(7)

Suitable storm rails or hand grabs shall be installed where necessary in all passageways, at deckhouse sides, and at ladders and hatches where passengers or crew might have normal access. The storm rails in way of the steps that lead to the forward passenger accommodation area on the starboard side was damaged at the base and loose creating a potential hazard. 46 CFR 177.35-5.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. There are a total of 22 adult life jackets that are stowed underneath the benches in the forward fixed seating within the benches that were not marked. Vessel is restricted to a maximum of 40 passengers until deficiency is rectified. 46 CFR 185.604 (f) and 46 CFR 185.115(b)

At each initial and subsequent inspection for certification, the marine inspector may require that a fire drill be held under simulated emergency conditions to be specified by the inspector. Vessel unable to conduct underway drills due to starboard main engine deficiencies found. 46CFR176.810(d)

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. Vessel unable to conduct underway drills due to starboard main engine deficiencies found. 46CFR176.808(g)

Whenever there is a hazardous condition, on board the vessel, the owner, master, agent, or person in charge shall immediately notify the Captain of the Port of the port of place of destination and the Captain of the Port of the port or place in which the vessel is located of the hazardous condition. Safety of the vessel was jeopardized after temporary repairs were made on the fuel oil and raw water vital systems without approval from OCMI. 46CFR185.203

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Temporary repairs were made on the fuel oil and raw water vital systems without approval from OCMI. 46CFR176.700(a)

Vessels indicator light for Class 1 watertight door for the bridge is not working as required by 46CFR170.255(e). The vessel will be restricted from carrying passengers after August 30, 2019 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Prior to Carrying Passengers,demonstrate proper operation of the pre-engineered (Fireboy) fire extinguishing system protecting the machinery space in accordance with 46 CFR 181.420

Make permanent repairs to the automatic, electric bilge pump located in the lazarette in accordance with 46 CFR 182.520. Crew made temporary wiring to ensure pump would activate in case of high water level; however, permanent repairs will be required no later than September 5, 2018. The vessel will be restricted from carrying passengers after that date if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector

Properly affix retro-reflective material on three (03) life jackets in accordance with 46 CFR 185.604(h) and IMO Resolution A.658(16). The vessel will be restricted from carrying passengers after September 5, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Properly mark all life jacket storage containers/locations in accordance with 46 CFR 180.78 and 46 CFR 185.604(f) with the type life jacket (adult or child) and the quantity. The vessel will be restricted from carrying passengers after September 5, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Remove all standing fuel in the flame impingement bowls in way of the fuel filters in accordance with 46 CFR 176.800(a). The vessel will be restricted from carrying passengers after September 5, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Properly post the Certificate of Inspection and the Stability Letter under glass or other suitable transparent material in accordance with 46 CFR 176.302 and 46 CFR 176.306. The vessel will be restricted from carrying passengers after September 5, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector.

Identify the leak and make permanent repairs to the section of pipe observed leaking in way of the port side Main Diesel Engine's power take of pump servicing the fire and bilge pump in accordance with 46 CFR 181.310(b). The vessel will be restricted from carrying passengers after September 5, 2018 if the deficiency has not been corrected to the satisfaction of an attending Coast Guard Marine Inspector. (EXTENDED UNTIL SEPTEMBER 19, 2018)

Vessels hull was damaged during Hurricane Irma and does not meet the structural design requirements per 46CFR177.300(c). Provide repair plan proposal to the Coast Guard and receive approval for repairs from the Coast Guard as described in 46CFR176.700 and carry out repairs after approval is received . Vessel restricted from carrying passengers until corrected to the satisfaction of the attending M

Vessels main engine suffered a casualty which made the engine non operational which does not meet the propulsion requirements per 46CFR182.200(a). Provide repair plan / continued operation proposal to the Coast Guard and restore main engine to suitable operations in the presence of a marine inspector prior to carrying passengers.
