A suitable metal marine type strainer must be fitted in the fuel supply line. A drip pan fitted with a flame screen must be installed under gasoline strainers. Fuel filters and strainers bowls must be highly resistant to shattering. Vessel Racor fuel filters corroded and missing drip pan bowls. Provide new approved fuel filters. 46 CFR 182.455(b)(6)

The minimum number of portable fire extinguishers required on a vessel must not be fewer than the minimum number required by Table 181.500(b). Vessel needs an additional 2-A rated portable fire extinguisher for the Accommodation space. 46CFR181.500(a)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. The vessel does not have an all-round white light. Place an all-around white light on the vessel. 46CFR183.420

Each survival craft required by a vessel must meet one of the following: For a life float - approved under approval series 160.027 or other standard specified. Net in life float was broken in several areas. Repair net. 46 CFR 180.200 (a)(2)

An embarkation ladder, approved IAW 160.017 in subchapter Q of this chapter, or other standard specified by the Commandant. Embarkation ladder bolt holes are worn. Repair holes. 46 CFR 180.50 (b)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Extra child size life jackets stowage location was not marked. Label stowage area. 46 CFR 180.78

Exhaust blower switches must be located outside of any space required to be ventilated by this section, and must be of type interlocked with the starting switch and the ignition switch so that the blowers are started before the engine starter motor circuit or the engine ignition is energized. Exhaust blower switches were not interlocked accordingly. Interlock exhaust blower switches IAW this subpart. 46 CFR 182.460(e)

The materials and construction of fuel lines, including pipe, tube, and hose, must meet the requirements of this paragraph. Discovered cracked fuel hose on starboard engine. Replace cracked hose. 46 CFR 182.455(a)

Shutoff valves so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line. Shut off valves missing for the engine connection. Install shutoff valves for the engine connection. 46 CFR 182.455(b)(4)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Hydraulic hose mounted is rusted through. Restore mount. 46 CFR 182.100

At each inspection, the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended. Discovered severe pitting on steering connected rod. Replace rod. 46 CFR 176.814

Battery installations must be mounted to protect against shifting of the vessel and free from exposure to water splash/spray. Battery covers were loose. Secure covers. 46 CFR 183.350(b)

Each ship transporting more that six passengers for hire equipped with a radiotelephone station must carry a radio operator who: holds a marine radio operator permit or higher class license. Master did not have marine radio operator permit. Provide proof of marine radio operator permit. 47 CFR 80.158 (e)

A drydock exam shall include inspection of hull plating from interior and exterior of vessel. Could not examine the internal hull plate where fuel tank is located. Make fuel tank space ready for inspection. 46 CFR 176.610

Provide Letter of Regulatory Compliance (LORC) for Reliable Drug Testing Services Consortium.

Update EAP Information to reflect current changes to DAPI program (i.e., Provide EAP information posted in vessel common space; Identify SAP and update EAP information with contact information; Hot-line information, etc).

Provide the DAPI covered employees list for BWR vessels. Confirm all employees have record of preemployment test or subject to random since COVID-19 shutdown.

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and the employer's supervisory personnel (continued on second page).Supervisory personnel must receive at least 60 minutes of training. Provide proof of completed EAP training for all personnel.

Confirm Blue Water Rafting's policy regarding employment for mariner's who have tested positive on any of the five types of DOT chemical tests and the MRO return to work process.

Gages to indicate lubricating oil pressure shall be provided for all propulsion engines and located so as to be in view of the operator. The port/stbd engine oil pressure gage was not operable during inspection. Provide operable oil pressure gage at operating station.

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel, and free from exposure to water splash or spray. Outboard battery was found to be sitting in approximately 5-6 inches of standing water. Remove water and dry out battery container.

Hydrostatic release for EPIRB was expired. Install new hydrostatic release to ensure installment is set to automatically float free and activate in accordance with regulations.

MMSI was not operable on VHF radio at time of inspection. Ensure MMSI # is properly entered into VHF radio for emergency use of the MOB/Distress button.

Non-return devices on port and stbd scuppers were cracked. Renew in accordance with 46 CFR 176.802.

Ventilation duct under console was deteriorated. Renew in accordance with 46 CFR 176.816.

Port tachometer was inoperative. Repair in accordance with 46 CFR 176.804.

Cracks were noted at center console running outboard, stbd approx 12"/port approx 6". Repair in accordance with 46 CFR 176.802.

VHF DSC function was not programmed. Prove DSC capability in accordance with 46 CFR 184.502.

Hull inspection plates were not removed for inspection. Remove plates in accordance with 46 CFR 176.802.

Replace mounting bracket.

Provide 18 adult PFD's

Provide one 24" life ring.

Repair torn area on buoyant apparatus

Remove or properly repair dead end wires located at operating station IAW 46CFR183.340. LEGACY COMPONENT DESC: Wiring

Provide plans for repair to stbd bow fracture IAW 46CFR176.700.

Remote fuel shutoff not working properly. Prove proper operation or shield valve IAW 46CFR182.455(b)(4) NLT 20JUL01.

Hydraulic steering hose fitting rusted and damaged the hose. Renew as appropriate with hose/fitting assemblies that meet SAE J-1942 NLT 20JUL01._x001C__x001C_ _x001C_

Prove proper operation of starboard engine temperature gauge IAW 46CFR176.804 NLT 17 AUG 00.

Prove survival craft are in a float free arrangement NLT 17AUG00, IAW 46CFR180.130(a)(2).

Conduct satisfactory alternate chamber pressure test in the presence of a Marine Inspector not later than 17AUG00. Vessel reduced to 15 passengers until in compliance. 46CFR176.800 and MSO inspection note 22.

Prove proper operation of fuel gauge IAW 46CFR176.804 NLT 17AUG00.

Repair or renew Ring Buoy NLT 24JUL00 IAW 46CFR180.70 and 160.050.

Provide proof of current hydro test on Fixed CO2 bottle.

Provide proof of operational mast head light. Vessel is restricted to daylight operations until in compliance.

Renew EPIRB battery.

Provide evidence of compliance with chemical testing regulations to include: a)60 minutes supervisor training, b)Employee assistance program and c) employee acknowledgements.

MAKE VESSEL AVAILABLE FOR INFLATION TESTS PER LOCAL POLICY (CIDNOTE 22).

MARK THE LIFEFLOAT WITH THE TOTAL NUMBER OF PERSONS (15).

PROVIDE WATERLIGHT FOR RING BUOY. VESSEL IS RESTRICTED TO DAYLIGHT_x001C_ OPERATIONS UNTIL COMPLETE.

PROVIDE OIL DISCHARGE PLACARD.

PROVIDE CHART FOR THE KAUAI OPERATING AREA. VESSEL IS RESTRICTED FROM OPERATING IN KAUAI UNTIL COMPLETED.

PROVIDE COPIES OF ALL REQUIRED CHARTS AND PUBLICATIONS NECESSARY FOR SAFE VESSEL OPERATION.

PROVIDE WRITTEN PROOF OF VESSEL'S STABILITY INCLUDING A DIAGRAM SHOWING FIXED SEATING ARRANGEMENT FOR PASSENGERS AT 18 INCHES PER PERSON.

PROVE VESSEL IS FULLY OUTFITTED AND OPERATIONAL TO MARINE INSPECTOR IN AREAOF OPERATION.
