MI discovered an exposed live wire in vicinity of port side gasoline fuel tank. Remove to satisfaction of USCG. 46 CFR 183.200

Flammable vapor detection systems. A sensor must be located above the expected bilge water level in the following locations:(2) The lowest part of a space containing a fuel tank when separate from the machinery space. Lower port side vapor detector as indicated by attending marine inspector. 46 CFR 182.480(e)(2)

A flammable vapor detection system must provide a visual and audible alarm at the operating station. STBD side vapor detector is not alarming at the operating station. 46 CFR 182.480(d)

Each survival craft must be:(1) Secured to the vessel by a painter with a float-free link permanently attached to the vessel. Painter line not connected to weak link. 46 CFR 180.130 (a)(1)

(p) The servicing facility must issue a certificate to the liferaft owner or owner's agent for each liferaft it services. The certificate must include— (1) The name of the manufacturer of the liferaft; (2) The serial number of the liferaft; (3) The date of servicing and repacking; (4) A record of the fifth-year gas-inflation test required in paragraph (g) of this section, whenever that test is performed; (5) A record of the hydrostatic test of each inflation cylinder required in paragraph (e) of this section, whenever that test is performed. Provide liferaft servicing documentation to include hydrostatic release install date. Mark hydrostatic for two years from install date. 46 CFR 160.151-57(p)(5)

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided that:(3) The pump is permanently mounted. STBD FWD bilge pump needs to be mounted. 46 CFR 182.520 (e)

c) A person may not employ or engage an individual in a position required to hold an MMC unless that individual maintains a current medical certificate. Medical certificates must be issued and will remain current for the following periods of time, unless otherwise noted on the certificate: (2) Five years for all other mariners. Provide documentation of a valid medical certificate. 46 CFR 15.401(C)(2)

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines, one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located. Emergency fuel shut off valves not fully closing. 46 CFR 182.455 (b)(4)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: (2) Inspection and operation of all watertight closures in the hull, decks, and bulkheads including through hull fittings and sea valves;Side shell fitting on the stbd side quarter needs to be replaced. 46 CFR 176.802

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an ergency. The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Fire in engine room reported. 1) Make full repairs and attest to serviceability of main propulsion engines and associated components. Conduct repair verification and sea trial with Marine Inspector.

Unless otherwise stated in this section, life jackets must be stored in convenient places distributed throughout accommodation spaces. Adult and child PFDs are stored below vessel operating station, an area that is more conducive to being a fire source. Relocate PFDs to a different area that can be easily accessible by passengers. 46CFR180.78

Port and stbd gasoline fuel tank space flammable vapor detectors are not located at the lowest part of the space. Reposition detectors to lower portion of space. 46CFR182.480(e)(2)

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Three fire buckets have capacity larger than 2.5 gallons. Replace or modify bucket to meet quantity amount. 46CFR181.610

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Operator did not have servicing report for fixed fire suppression system. Provide report to Marine Inspector.46CFR810(b)(2)

Each heater must be so constructed and installed as to prevent contact with combustible materials such as towels and clothing. Heating unit in accommodation/vessel operating station area is in close proximity to combustible materials. Remove, tag out (take off-line) or reposition heating unit away from combustible materials. 46CFR184.210(a)

Each floating waterlight installed after March 11, 1997, on a vessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. Float light lanyard does not have a corrosion resistant clip. 46CFR180.70(d)

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Operator could not provide application completed for EPIRB registration. Provide documentation for EPIRB registration. 46CFR184.502

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. An inspection of radio installation equipment was not conducted by a licensed FCC technician. Complete inspection of equipment no latter than 28JUN2024.47CFR80.903

Except as provided in §§ 80.177 and 80.179, operation of transmitters of any ship station must be performed by a person holding a commercial radio operator license or permit of the class required below. The operator is responsible for the proper operation of the station. Vessel master did not have marine operator permit. 47CFR80.155

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel, and free from exposure to water splash or spray. Batteries are not secured with a strap to prevent movement. 46CFR183.350(b)

Pre-employment testing requirements. (a) No marine employer shall engage or employ any individual to serve as a crew-member unless the individual passes a chemical test for dangerous drugs for that employer. Perspective employee has not completed pre-employment testing and consortium enrollment. Provide evidence of new employee meeting requirements prior to filling safety sensitive position (crew member). 46CFR16.210

Except on a vessel limited to daytime operations, the compass must be illuminated. Light in compass unit is inoperable. Restricted to day light operations. 46CFR184.402(c)

A hatch cover must: (2) Be attached to the hatch frame or coaming by hinges, captive chains, or other devices of substantial strength to prevent its loss. Aft port hatch is not secured with a device to prevent its loss. 46CFR179.360(b)(2)

When, in accordance with § 178.210 of this chapter, a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. If posting is impracticable, the stability letter must be kept on board in a weathertight container readily available. Operator did not have stability letter on-baord. Post letter at operating station. 46CFR178.210

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. IBA hydrostatic release unit was marked with installation date not expirations date. Re-stencil hydrostatic release unit with appropriate expirations date and provide evidence of change to Marine Inspector. 46CFR185.740(b)

A marine employer whose vessel is involved in a casualty or incident that is, or is likely to become, a serious marine incident as defined in § 4.03-2 of subchapter A of this chapter shall comply with the requirements of § 4.06 in subchapter A of this chapter. Operator did not have any alcohol testing swabs onbaord. 46CFR185.212

Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be drydocked or taken out of service to assess the extent of the damage, and to effect permanent repairs. The OCMI may also decrease the drydock examination intervals to monitor the vessel's structural condition...Hull gauging report required on all bellow the water line hull plating prior to bringing vessel back into commercial service due to large amounts of corrosion and pitting noted during a non credit ise exam on 23-24 Feb 2023 in Petersburg,AK. The two generator voids require hull plate replacement at this time due to heaving pitting and corrosion beyond 50% wastage noted during this inspection. Submit hull gauging report and repair proposal to the local OCMI prior to completing any work.46 CFR 176.600 / (Old T) 46 CFR 176.25-5.

Vessel is required to have x3 Day and x3 Night, CG Approved Visual Distress Signals. Vessel does not have the correct day/night VDS pyro.

Vessels IBA needs to be float free arrangement, discovered vessels IBA improperly installed, was not connected to the weak link on the hydrostatic release.

Vessel is required to have an anchor light and fwd mast head light while operating at night or reduced visibility, Found the anchor/fwd mast head light not working. Vessel restricted to daytime operations until the navigation lights are repaired.

Vessel is required to have bonding wire at the hose connections for the port and stbd metallic fuel fill pipes to the fuel tank. Bonding wires are missing on both fuel tanks.

Fuel tank voids, power ventilation fans have internal electric motors that are installed inside the the ventilation duct and have plastic vent hose, vent fan motors shall be external to the ventilation and have metallic ventilation hose.

Fuel fill hoses installed over pipe must be double clamped. The port side fuel fill hose is missing double clamps.

Vessels life ring buoy shall be CG Approved with x4 bans of retroflective tape. Vessels life ring buoy missing x3 bans of retro tape.

Fuel tank voids, power ventilation system must shut down when the pre engineered fire extinguishing system discharges. Prove proper operation of power ventilation shut downs.

Hatches accessible to the weather deck must be watertight, the two aft deck hatch gaskets are worn out and partially missing.

Electrical installations shall be safe, hazard free, and prevent ignition source. Remove dead end wires in the port fuel tank void.

Vessels EPIRB (NOAA)registration shall be onboard while operating, vessels name shall be marked onto the EPIRB. Vessels EPIRB registration and decal where not onboard.

Vessel must post emergency radio operating procedures close to the vessel's radiotelephone (VHF). Operating procedures are not present/missing.

The required life raft was not installed. Prior to carrying passengers, the required IBA must be installed IAW 46 CFR 180.130.

The hydrostatic release on the EPIRB was expired. Prior to carrying passengers, install a new hydrostatic release on the EPIRB.

Prior to carrying passengers, a fire drill must be completed in the presence of a Coast Guard Marine Inspector.

Prior to carrying passengers, prove proper operation of the ship’s machinery, to include propulsion, steering, bilge high level alarms and pumping, and main engine shut downs associated with the fixed extinguishing system.

Vessel is restricted to operating as an uninspected passenger vessel carrying no more than 6 passengers for hire. The vessel is restricted to no more than 6 passengers for hire until a properly registered EPIRB is placed in service on the vessel. The vessel’s current EPIRB’s battery expiration date cannot be determined.

Vessel’s Safety Radio Certificate is expired.

Demonstrate man-overboard and fire drills to the attending Marine Inspector Vessel owner/operator has been conducting drills with crews and has logged all drills.
