Means shall be provided whereby normal operation of propulsion machinery can be sustained or restored even though one of the essential auxiliaries becomes inoperative. Master reported the loss of the starboard propulsion motor (azimuth Z-Drive). Affect repairs to the satisfaction of the RO / Coast Guard.

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Crew discovered a quarter-size hole in a flange downstream of the emergency fire pump and made temporary repairs. Affect repairs to the satisfaction of the RO/Coast Guard prior to departing Port Everglades. SOLAS (2020) II-2 /Reg 14.2.1.2

The following fire protection systems shall be kept in good order so as to ensure their required performance if fire occurs: fire detection and fire alarm systems. Interview with the crew indicates that the system is fully operational but is unable to clear the “trouble” in the system. Maintenance, testing and inspections of life-saving appliances shall be carried out based on the guidelines developed by the Organization and in a manner having due regard to ensure reliability of such appliances. Identified steel plate corrosion in both lifeboat internal release mechanism support system. The annual inspection was completed by Palfinger in Apr 2023, with no noted concerns or notation of condition of these areas. Cite: SOLAS (2020) III/Reg 20.3.1

Spares and repair equipment shall be provided for lifesaving appliances and their components which are subject to excessive wear or consumption and need to be replaced regularly. Noted cracking in port lifeboat (stbd/inboard side) gel coating and the ship does not maintain a repair kit to address this area. Cite: SOLAS (2020) III/Reg 20.3.1

Doors required to be self-closing shall not be fitted with hold-back hooks. •During fire drill, galley door was found locked, and crew unable to enter through this door. Door is a selfclosing door, with closing hinge disconnected from door frame. Also, found hook tie-back attached to door/ bulkhead. •Adjust self-closing door to pump room 3-73-0. Crowns and casing of machinery spaces of category A shall be of steel construction adequately insulated and openings… shall be suitably arranged and protected to prevent the spread of fire. • At MVZ-24 overhead (BOSN stores) two sections of insulation missing. • Tape missing in overhead in fwd pump room. Electrical Installations shall be such that the safety of crew and ship from electrical hazards will be ensured. The following light fixtures were found with broken diffusers (IEEE: 33.14): • Bosn store room (x3 lights) • Pump room • Throughout High Density room • Fan space by library • Stairway (2nd platform) above door to Passage 3-65-0 • Fix lights in overhead in linen locker and adj cleaning gear locker The following improper electrical connections were identified (IEEE 23.6): •Replace/repair electrical cable for lights on stbd bridge wing and other light fixtures on deck •Junction box cover detached by light in stairwell (near Bow Thruster control panel -Bosn stores?) •SOPEP locker (above light) has cable terminations with wire nuts not in junction box •In High Density room, junction box by emergency escape hatch has wire backing out •Improper electrical connection for special winch system• Cable penetration on bottom right side of MSD control panel • GMDSS battery cable insulation is deteriorated and shows signs of possible splice (46 CFR 111.60-19) Cite: SOLAS (1997) II-1/Reg 40.1.3; 46 CFR 110.10-1(b)(4)(iii) (old J); IEEE-45-1983

The Administration shall take appropriate steps to ensure uniformity in the implementation and application of the provisions of this part in respect of electrical installations. The following items were identified using daisy chains and portable cables (extension cords), which should be permanently wired (IEEE 19.6.1) •Daisy chain and splice in cable for intrinsically safe lights in fwd fire locker. •Daisy chain in Bosn locker. The following wiring items were identified (IEEE: 20.2): • Dead end wires on stbd side bridge (external windows) • Dead end wire in deck penetration on aft port side between rail course and conex box • Cable termination for freezer fan unit not in junction box (IEEE: 21.43) Cite: SOLAS (1997) II- 1/Reg 40.2; 46 CFR 110.10-1(b)(4)(iii) (old J); IEEE-45-1983

The Administration shall take appropriate steps to ensure uniformity in the implementation and application of the provisions of this part in respect of electrical installations. The following items for unsupported electrical cables were identified (IEEE 23.9): • Unsupported cables in aft part of aft fire locker, and cables resting on edge of metal cabinet. • Electrical cables running from conex boxes on aft deck are not properly secured and are improperly connected to service panel box in aft vestibule [and also IEEE 23.9/23.10, 46 CFR 195.11-25(a)(1)] • Electrical cables are not properly supported in wire run above Main deck stairway Passage 1-24-1 (including dead end wires) and also FSD 1-72-1. Guard rails or bulwarks shall be fitted around all exposed decks. Rail courses were missing between newly installed Conex Box (port side aft) and port beam/ fantail railing. Cite: ILLC Reg 25(2)

All waste receptacles shall be constructed of noncombustible materials with no openings in the sides or bottom. Plastic trash cans, including plastic waste receptacle were used for oily rags. Cite: SOLAS (1997) II-2/Reg 18.5

Medical waivers may be granted with specific conditions to which the applicant must adhere, such as more frequent monitoring of the medical conditions, submission of medical exams and/or tests at varying intervals to track the ongoing status of the medical condition, or operational limitations in the manner the mariner may serve under the MMC. • 03 crew members were missing medical certificate waivers. • Current steward med certificate indicated not fit for food handling. Steward recently received new medical certificate and provided documentation supporting no medical issues. Provide supporting documentation from NMC of updated medical certificate.

The training manual…shall contain instructions and information, in easily understood term illustrated wherever possible, on the lifesaving appliances provided in the ship and ton the best methos of survival. The immersion suits found in the SOLAS training manual were for a different model than crew had on board. Cite: SOLAS (2020) III/Reg 35.3

Two means of escape shall be provided from each machinery space of category A. A box was stored in emergency escape ladder. Cite: SOLAS (1997) II-2/Reg 45

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. There is a steady drip of hydraulic oil from a linkage between the pump motors on the tank top.

The following fire protection systems shall be kept in good order so as to endure their required performance if a fire occurs: structural fire protection including fire resisting divisions, and protection of openings and penetrations in these divisions. A rock block type multi-cable transit installation in the steering gear room is not installed correctly, pieces are protruding or recessed and should be installed as designed.

Doors provided to ensure the watertight integrity of internal openings which are used while at sea are to be sliding watertight doors capable of being remotely closed from the bridge and are also to be operable locally from each side of the bulkhead. The WTD on winch room fwd bulkhead has a faulty power switch.

Power control systems shall close doors at an approximate uniform rate of closure, but in no case shall the closure time, from the time a door begins to move to the time it reaches the fully closed position, be less than 20 s or more than 40 s. Vsl's Class 3 WTDs reached complete closure after 1 minute.

Maintenance of conditions after survey. On 3/2/2021 at approximately 2330 Sector Virginia Command Center received a report of a fire in the starboard thruster room that occurred at 2130 in the brake band of the starboard thruster. 74 SOLAS (14) I/11

Vessel has posted through-out the ship Fire Control Plans (FCP) that are Revision "A". The most current revision on the bridge is Revision "D". Plans shall be kept up to date; any alterations thereto shall be recorded as soon as practicable. The total number of FCPs on-board shall be the amount as indicated on the FCP Revision "D".

The Recognized Organization is in the process of issuing certificates; the certificates issued shall be readily available on board for examination. The RO shall emails electronic copies to USCG before departure CLEARED

The following fire doors are not closing on their own: laundry room doors / door leading into draft room from main lab: fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs.

The following flood/over the side lights need to be properly mounted; starboard bridge wing light is secured with a line, port side at bottom of ladder light is just laying on the deck; electrical apparatus shall be installed as not to cause injury when handled or touched in the normal manner

Make repairs to the small oil leak on the #1 ship service diesel generator turbo flange. Means shall be provided to control leaks of flammable liquids; The arrangements of distribution and utilization of oils shall be such as to ensure the safety of the ship and persons on board.

The port outboard steering pump is acting and making a abnormally loud noise,the pump needs to be investigate /trouble shoot to see the cause of the noise and make repairs as needed. The ships equipment shall be maintained to conform with the present regulations to ensure that the ship may fit to proceed to sea.
