Each panelboard must have: A circuit directory that has: (i) The circuit designation of each circuit; (ii) A description of the load of each circuit; and (iii) The rating or setting of the overcurrent protective device for each circuit. Vessel found with suspect lockout of breaker 25 on the 1st deck forward port panel. 46 CFR 111.40-11(b)

At each annual inspection the passenger and crew quarters, toilet and washing spaces, galleys, serving pantries, lockers, etc., shall be examined by the inspector to be assured that they are in a sanitary condition. Vessels galley hood grease trap system found with excess oils. 46 CFR 71.25-40

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. Vessel found with missing firestop in cable transits on the 01 and 03 decks. 46 CFR 72.03-5

Where the general emergency alarm signal cannot be heard over the background noise, there must be a red-flashing light or rotating beacon, that is activated whenever the general emergency alarm signal in the space is activated. Emergency alarm signal in the emergency generator room found with a broken mount. Repair to the satisfaction of the Marine Inspector. 46 CFR 113.25-10(a)(2)

All valves and closing appliances, or other mechanisms which may be required to be operated for damage control purposes in case of emergency shall be conspicuously marked with letters at least 1 inch high identifying the control and the direction of operation. In all cases indication shall be provided to show whether the control is open or closed. Starboard main deck emergency fuel shutoff valve found to be improperly labeled. Properly label to the satisfaction of the Marine Inspector. 46 CFR 78.47-38(a)

The annual inspection shall include an inspection of the structure, boilers, and other pressure vessels, machinery and equipment. The inspection shall be such as to insure that the vessel, as regards the structure and piping is in satisfactory condition and fit for the service for which it is intended. Galley grey water drain located above Fire Station No. 1 found wasted. Replace prior to credit dry dock. 46 CFR 71.25-10

Sufficient illumination must be provided by the emergency lighting source under emergency conditions to effect damage control procedures and to provide for safe egress from each space. 03 emergency lights discovered inoperable. Repaired in the presence of the attending Marine Inspectors. 46 CFR 111.75-15(c)(2)

At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010. Port bridge wing floating water light found to be inoperable. Repaired in the presence of the attending Marine Inspector. 46 CFR 199.70(a)(3)(ii)

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. 3rd deck starboard life buoy found with wasted retro reflective material. Replaced in the presence of the attending Marine Inspector. 46 CFR 199.190(a)

Each pressure vessel listed on the Certificate of Inspection that is fitted with a manhole or other inspection opening so it can be satisfactorily examined internally, must be opened for internal examination every 5 years. Bridge unfired pressure vessel unavailable for internal inspection. Make available for internal inspection in the presence of the attending Marine Inspector. 46 CFR 61.10-5(b)(2)

Each self propelled vessel must be provided with a power-driven pump or pumps connected to the bilge main as required by table 56.50-55(a). Forward bilge pump found with wastage on the pump casing. Repair bilge pump to the satisfaction of the Marine Inspector. 46 CFR 56.50-55(a)(1)

Electric installations on vessels must ensure the maintenance of services necessary for safety under normal and emergency conditions. Light fixture in steering gear compartment found broken. 46 CFR 111.01-1(a)(1)

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel experienced loss of bow thruster. Vessel is required to have tug alongside for docking and undocking until bow thruster is repaired and operational test complete to satisfaction of attending Marine Inspector. 46 CFR 71.40-1

All doors shall be capable of operation from either side by 1 person. During inspection, FSD 18 was found to be hard to operate as designed without removing hardware from door. Repair to satisfaction of attending Marine Inspector. 46 CFR 72.05-25(a)(1)

Vessels of 1,000 gross tons and over…shall have permanently exhibited for the guidance of the officer in charge of the vessel the following plans: General arrangement plans showing for each deck. OCMI approved Fire Control Plan was reviewed after extensive drydock. The following were found: Spanner wrench at Fire station #6 is missing; Fire axe at Fire Extinguisher #31A is missing; Alarm bell next to Fire Extinguisher #18 is missing. Replace all to satisfaction of attending Marine Inspector. 46 CFR 78.45-1(a)(1)

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Vessel replaced upper rescue boat falls on schedule. However, sling for rescue boat was not replaced. Replace sling with new IAW manufacturer recommended specs. Weight test to 1700lbs to meet load test requirements of falls that were replaced. Secure all shackles with mousing wire. 46 CFR 199.190(a)

Wire must be in an enclosure. Exposed wire found in Fireman’s Outfit locker and on an exposed circuit board next to Fire Station #22. Remove or properly enclose wire to satisfaction of the attending Marine Inspector. 46 CFR 111.60-11

The bilge pumping system shall be capable of operation under all practicable conditions after a casualty whether the ship is upright or listed. Marine Inspector found trash bag covering bilge pump pick-up in Void #2 port side. 46 CFR 56.50-50(a)(1)

All passenger vessels shall have efficient guard rails or bulwarks on decks and bridges as follows: The height of the rails shall be at least 39 ½ inches from the deck. Courses shall not be more than 15 inches apart. Vessel is replacing guard rails on stern, either side of paddle wheel. Make repairs to satisfaction of attending Marine Inspector. 46 CFR 72.40-5(a) 705 - Prior credit drydock

In general, compliance with the standards established by the American Bureau of Shipping, see subpart 70.35 of this subchapter, will be considered satisfactory evidence of the structural efficiency of the vessel. However, in special cases, a detailed analysis of the entire structure or some integral part may be made by the Coast Guard to determine the structural requirements. Vessel has additional structural framework in the bow that has cracked welds. Submit repair proposal prior to making repairs or submit alteration plans to MSC for approval prior to removal. Complete prior to credit drydock. 46 CFR 72.01-15(a)

Doors in bulkheads required to be Class A-0 shall be of solid or hollow steel or equivalent metal construction capable of meeting the requirements of a Class A-0 bulkhead. A-0 door on aft main deck starboard CO2 room catches on the deck and does not freely close.

All doors shall be capable of operation from either side by 1 person. Bow and starboard quarter first deck doors were missing handles and incapable of operation on the weatherdeck side.

Doors other than those which are normally locked, such as from staterooms, fan rooms, lockers, etc., shall be of the self closing type capable of closing against a 3.5 degree list. Starboard bow foyer second deck door damaged at upper frame and not able to self close.

A receptacle outlet must not have any exposed live parts with the plug opening uncovered. Electrical outlet on 3rd deck HVAC room had broken face plate allowing exposure to internal parts.

Each credentialed crewmember must become familiar with the relevant characteristics of the vessel appropriate to his or her duties and responsibilities prior to assuming those duties and responsibilities. As appropriate, these may include, but are not limited to, general arrangement of the vessel, maneuvering characteristics, proper operation of the installed navigation equipment, proper operation of firefighting and lifesaving equipment, stability and loading characteristics, emergency duties, and main propulsion and auxiliary machinery, including steering gear systems and controls. During the inspection, the on-board engineers were not readily familiar with the bilge management system, caused a blackout while starting a fire pump and were observed silencing safety alarms without observing the equipment. Vessel shall demonstrate crew training relevant to operational and emergency duties prior to getting underway with passengers.

The main steering gear must be operable from the pilothouse by mechanical, hydraulic, electrical, or other means acceptable to the Commanding Officer, Marine Safety Center. Vessel reported limited maneuverability due to right rudder failure. 46 CFR 58.25-70(b)

The marine employer shall ensure that all persons directly involved in a serious marine incident are chemically tested for evidence of dangerous drugs and alcohol in accordance with the requirements of 46 CFR 4.06. Company’s procedures for post incident testing were found inadequate.

Fire hydrants must be of a sufficient number and so located that any part of the vessel accessible to the passengers or crew while the vessel is being navigated, other than main machinery and cargo holds, may be reached with at least two streams of water from separate outlets, at least one of which must be from a single length of hose. During inspection of the fire station, MI discovered that the lower connection of Fire Station #7 was clogged and inoperable.

Fire drills must, as far as practicable, be planned with due consideration given to the various emergencies that may occur for that type of vessel and its cargo. Vessel did not have procedures for crew training on breakers which would need to be secured in the event of a fire, with only the currently Chief Engineer being aware of their locations.

Vital systems that are automatically controlled must be provided with an alarm system for detection of a failure or unsafe condition. The Voltage Differential alarm monitoring equipment for all 3 generators did not actuate for high or low voltage during the tests of the Periodic Safety Test Procedures.

As an alternative to meeting the survival craft equipment requirements of 199.175, a vessel may carry the equipment specified in table 199.620(j). Lower rescue boat missing one repair kit and two sponges.

As an alternative to the survival craft requirements of § 199.201(b), vessels may have a safety assessment approved by the local OCMI…A comprehensive shipboard safety management and contingency plan that is tailored to the particular vessel, is easy to use, is understood by vessel management personnel both on board and ashore, is updated regularly… The Shipboard Safety Management and Contingency Plan has not been updated since 2003.

Each vessel must carry lifebuoys approved under approval series 160.150. Port bridge wing and 3rd deck starboard lifebuoy’s grab line has deteriorated affecting the breaking strength of the line.

A ship of 300 gross tons or more constructed after June 30, 1974 must have fixed container or enclosed deck area under or around each fuel or bulk lubricating oil tank vent, overflow, and fill pipe, that: For a ship of 300 or more but less than 1600 gross tons has a capacity of at least one half barrel. Drain plug on port side fuel oil manifold containment is missing.

Where pressure gages are used, they shall be chosen to be compatible with the pressure to be indicated. The size of the visual display, the fineness of the graduations, and the orientation of the display shall be considered. In no case shall the upper range of the gage be less than 1.2 times nor more than 2 times the pressure at which the relieving device is set to function. Air receiver under bridge has a gage that has an upper limit of 200psi which is 1.0 times the MAWP.

Unless otherwise specified in this section, the Vessel Security Officer must keep records of the activities in paragraph (b) of this section for at least 2 years and make them available upon Coast Guard request: Annual audit of the VSP. For each annual audit, a letter certified by the Company Security Officer or the VSO stating the date the audit was completed. Operator could not provide such letter at time of inspection.

During the testing of the control station, Fire Screen Doors #3 and #12 did not latch closed after activation.

Each liferaft must be arranged to permit it to drop into the water from the deck on which it is stowed. A liferaft stowage arrangements meets this requirement if it… (iii) Has a gate or other suitable opening large enough to allow the liferaft to be pushed directly overboard. The chains securing the opening for the liferafts have releasing clips seized in place.

A vessel required to have Type I or Type II subdivision must have enough main transverse watertight bulkheads to separate the the machinery space from the remainder of the vessel. All portions of these bulkheads must be watertight below the bulkhead deck. Vessel has multiple cable transits which have been modified from original construction leaving their integrity in question.

No repairs or alterations shall be made to any lifesaving or fire-detecting or fire-extinguishing equipment, except in an emergency, without advance notice to the Officer in Charge, Marine Inspection. Rescue boat #1 in off vessel for maintenance/repower.

All portable fire extinguishers and semi-portable fire extinguishing systems must be maintained in accordance with NFPA 10. 04 Fire extinguishers were found to be under required charge.

Tests and inspections of the lifesaving equipment shall be carried out during each inspection for renewal of certification and periodic inspection, and shall include, as applicable, a demonstration of; The proper condition of flotation equipment such as lifebuoys, lifejackets, immersion suits, work vests, lifefloats, buoyant apparatus, and associated equipment. Lifebuoy on 2nd deck port side missing plug.

All fire detection and extinguishing systems, all piping controls, valves, and alarms must be checked to ascertain that the system is in operating condition. Fire detection system and panel overdue for its annual servicing. 46 CFR 71.25-20(a)(3)

The inspector shall examine all accessible parts of the vessel’s equipment to be assured that it is in a satisfactory condition. Discovered grease buildup in grease traps over oven, deep fat fryer, and griddle and also covering the external exhaust fans for the galley. 46 CFR 71.30-5(a)

Fire control diagram showing location and type of all fire-screen insulation, and type of all doors in such subdivisions and enclosures is required. Currently maintained fire control plan requires updating, including the classification of aft storage spaces on main deck, verification of all fire safety equipment (including relocated general alarm bells in the vicinity of bars), verification of doors, etc. Require walkthrough and verification of all required fire boundaries, systems, and equipment on station with local OCMI inspection team. 46 CFR 71.65-5(d)

Emergency lighting must be arranged so that it can be energized from the temporary emergency power source, including low location lighting for passageways, stairways, crew quarters, public spaces, and work spaces sufficient to allow passengers and crew to find their way to open decks and to survival craft, muster stations, and embarkation stations. Discovered burned out light bulbs for emergency lighting located: 3rd deck port ladder forward; AC flat; AC flat pump room on stbd side; 3rd deck stbd (most forward section by lifejacket locker); 3rd deck interior port side (near cash cage); and 4th deck patio lights (seven total on stbd side). 46 CFR 77.05-1(a) [Sub H], 46 CFR 112.15-1(c) [Sub J]

All emergency lights shall be marked with a letter “E”. Emergency lights in the 3rd deck male and female heads discovered without required label. 46 CFR 78.47-33(a).

Each generator prime mover must shut down automatically upon loss of lubricating pressure. Low lube oil pressure alarm/shutdown for port SSDG was inoperable. 46 CFR 111.12-1(c).

An audible and a visible alarm must activate in the machinery space upon occurrence of a low oil level in any oil reservoir for a hydraulic, power-operated steering-gear system. Low fluid level alarm for main steering gear was inoperable. 46 CFR 58.25-25(e)(3).

Each opening in an exposed weather deck must have a means for closing it watertight. Forward void watertight hatches & forepeak watertight hatch openings have wasted gasket edge, submit repair proposal. 46 CFR 171.122(f)(2).

An overload protective device must be in each battery conductor. The 24V batteries for the propulsion control system did not have overcurrent protection devices. 46 CFR 111.15-25(a).

Each panelboard must have a circuit directory that has the circuit designation of each circuit. Circuit breaker #12 in panel 401 (located in control room) is not labeled. 46 CFR 111.40-11(b)(2)(i).

Approved fixed fire extinguishing systems must be installed in 'service spaces containing highly combustible materials'. 'Hard Count' room is not an approved service space to contain highly combustible materials and does not contain fixed fire extinguishing system. Remove oil drums, used oil filters, dirty oil and other highly combustible materials from space. 46 CFR 76.05-20.

Vessel shall undergo midperiod inspection between the fifth and seventh month of certification.

Nonmetallic flexible hose must be in accordance with SAE J1942 and may be installed only in vital and nonvital fresh and saltwater systems. Vessel changed configuration on both main engines and both diesel generators to outfit them with non-approved hoses for jacket water. 46 CFR 56.60-25(b)

All marine engineering details and their appurtenances installed on the vessel, shall be designed, constructed and installed in accordance with the provisions of subchapter F. Naming scheme on engine control room panel and general arrangement drawing crew uses for bilge alarms do not match in all instances. Update the naming scheme so drawings match alarm panel. 46 CFR 70.20-1

All emergency lights shall be marked with a letter “E” at least ½ inch high. Several markings for emergency lighting on upper exterior decks were missing. Affix proper marking to missing locations for emergency lighting. 46 CFR 78.47-33

All general alarm bells shall be identified by red lettering at least ½ inch high: GENERAL ALARM-WHEN BELL RINGS GO TO YOUR STATION.” Marking was not present next to general alarm on main deck forward exterior deck. Affix sign next to general alarm on main deck forward. 46 CFR 78.47-7

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Bracket for life ring on aft main deck near rescue boat was no longer affixed to lower rail due to wasted screw. Reaffix bracket. 46 CFR 199.190

The material, design and construction and workmanship of main and auxiliary machinery shall be at least equivalent to the standards established by the American Bureau of Shipping or other recognized classification society. Vessel was unable to demonstrate that governors will prevent the engines from exceeding the rated speed by more than 15%. Demonstrate governor will prevent the engine from exceeding the rated speed by more than 15%.46 CFR 58.05-1 (a) ABS Steel Rules 34.11.6 (1989) Correct Prior to credit dry dock.

All automatically or remotely controlled or monitored vital systems addressed by part 62 of this sub chapter must be subjected to tests and inspections to evaluate the operation and reliability of controls and safety features. Test Procedures shall be submitted to the Coast Guard for approval. Test procedure for verification that governors prevent Main Diesel Engines from exceeding 15% of the rated speed is not included in the PSTP's. Correct prior to credit dry dock. 46 CFR 61.40-1

Inspections shall be made of the main machinery that ensure the equipment and associated structure are in satisfactory operating condition and fit for the service which they are intended. Exhaust piping for both SSDG have evidence of leakage in steering space, and spaces directly aft of the engine room. Repair leaking exhaust piping prior to credit dry dock.46 CFR 61.01-1

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked. Entrances to three spaces containing CO2 cylinders were not fitted with warning signs. Affix proper warning signs outside of CO2 storage locations. 46 CFR 78.47-11

It shall be the duty of the owner, master, or person in charge to see that the vessel's firefighting equipment is at all times ready for use. Starboard bridgewing fire hose bursted at the fitting when charged. Replace fire hose. 46 CFR 78.17-80(a)

Rescue boat falls must be renewed every four years if not end-for-ended. Renewal is overdue on upper rescue boat, renew within 14 days. 46 CFR 199.190

Spanner wrenches must be provided for all fire stations. Provide spanner wrenches for stations with 2.5in hoses within 14 days. 46 CFR 76.10-10

All free standing furniture such as chairs, sofas, tables, etc., shall be constructed with frames of approved in-combustible materials in rooms containing fire resistant furnishings. Remove non-compliant chairs and other non-compliant free standing furniture from category "5" spaces or submit revised structural fire protection and fire safety plans to MSC to re-categorize affected spaces to category "7". 46 CFR 72.05-55/46 CFR 72.05-5

Bulkheads or decks of the "A" Class shall be composed of steel or equivalent metal construction. In addition, they shall be so insulated with approved structural insulation. Forward elevator on 3rd deck is missing approximately 1ft of insulation on the exterior side of overhead. Install proper insulation on missing portion. 46 CFR 72.05-10 46 CFR 72.05-10

As appropriate for the intended voyage, all vessels must carry adequate and up-to-date charts. Charts onboard are not up-to-date, last revision on chart indicates it was completed in February 2017. Provide up-to-date charts. 46 CFR 78.05-05

All decks: all stanchions wasted on gauging report dated 26JUN18.

Crop and renew cracked plate at port fwd ramp pin and port/stbd wasted ramp rails.

Crop and renew port outboard exhaust penetration and exterior portion of piping by next credit drydock due 10JUN20.

Provide technician's report outlining cause/correction associated with port engine propulsion loss.
