Your vessel must undergo an annual inspection within 3 months before or after each anniversary date, except as specified in paragraph (b) of this section. Vessel's anniversary COI date was 01March2026 and did not have an annual inspection within the 6-month window. 46 CFR 31.10-17(a)

46 CFR32.02-15 All exposed and dangerous places such as gears and machinery shall be properly protected with covers, guards or rails in order that the danger of accidents may be minimized. Barge generator had exposed rotating machinery. A guard or cover needs to protect the rotating machinery.

A vessel may not provide any part of the transportation of merchandise by water, or by land and water, between points in the United States to which the coastwise laws apply, either directly or via a foreign port, unless the vessel—(1)is wholly owned by citizens of the United States for purposes of engaging in the coastwise trade; and(2)has been issued a certificate of documentation with a coastwise endorsement under chapter 121 or is exempt from documentation but would otherwise be eligible for such a certificate and endorsement. Vessel is not wholly owned by US citizens. Vessel must provide proof of ownership eligibility via NVDC prior to operating and issuance of a COI. 46 USC 55102(b)

Operational tests and checks of all safety and limit controls, combustion controls, programming controls, and safety relief valves must be conducted by the owner, chief engineer, or person in charge at the inspection for certification. These tests and checks must be conducted in the presence of a marine inspector and must include the following: proper prepurge, burner ignition sequence checks, operation of the combustion controls, limit controls, fluid flow controls, fluid level controls, high temperature control, proper postpurge control, and verification of the flame safeguard. The owner or operator shall submit the following to the OCMI: An engineering analysis signed by a registered Professional Engineer licensed by any state of the United States or the District of Columbia, or signed by a Coast Guard-approved organization, that certifies the vessel's compliance with the minimum section modulus and plating thickness requirements of subpart 32.59 of this chapter. Vessel has not provided engineering analysis following mid-body gauging report. 46 CFR 31.10-21a(d)(2)(i)

To maintain a valid Certificate of Inspection, you must complete your annual and periodic inspections within the periods specified in § 31.10–17 (a) and (b) and your Certificate of Inspection must be endorsed. Vessel was previously enrolled in TBSIP. Upon sale, the COI became invalid. Request a new COI before vessel can be operated.

Thermal Fluid Heater (TFH) visible alarm would not function: unable to see any light. Repair/replace prior to using TFH.

During the visual inspection of the thermal fluid heater system, the power supplying generator engine tachometer continually fluctuated without providing a steady reading. Repair prior to conducting cargo thermal fluid heating operations.

SUBMIT VESSEL RESPONSE PLAN TO COMMANDANT(G-MEP) AND PLACE REQUIRED PARTS ON BOARD PRIOR TO CONDUCTING CARGO OPERATIONS.
