A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. While conducting the Internal Structural Exam, found the following: #1 starboard void -02 tripped welds - top frame 4th back & top frame 2nd back -01 crop and renew - top frame first back #2 starboard void -01 tripped weld - top frame first back -03 crop and renew - top of vertical U channel 3rd frame back, 4th frame down 3rd back, 5th frame down 3rd back. #3 starboard void -01 tripped weld - top frame 4th back -02 crop and renew - top frame first back & top frame 3rd back. #4 starboard void -02 crop and renew - 4th down 4th back & Vertical U channel IWO 3rd down first back #1 Port void -01 tripped weld - 3rd down 6th back #2 Port void -01 crop and renew - transverse stiffener angle 2nd frame back #3 port void -01 tripped weld - top frame 2nd back -02 crop and renew - vertical U channel 1st frame down 3rd frame back, Vertical U channel top frame 1st back #4 port void -01 tripped weld -top frame 2nd back -01 crop and renew - top frame 4th back

A survey shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel representative reported a pin hole leak along a weld approximately 4-5’ back from the head log and 2.5-3’ down from the rake deck. Vessel rep applied a temp repair utilizing wood shims. Vessel will transit directly to Cincinnati for cargo offload and return to Campbell Transportation in Newell, WV for repairs and inspection. While underway the following precautions will be made: - Have a dewatering pump on standby - Conduct regular sounding of space to monitor water level and log in towing vessel record. Once in a repair facility, the vessel operator must submit a repair proposal containing permanent repair plans to [name withheld].

Cargo piping systems shall be arranged and fabricated in accordance with this section and Subchapter F. Piping, including valves, pipe fittings and flanges, conveying vapors, gases or liquids whose temperature exceeds 150 °F., shall be suitably insulated where necessary to preclude injury to personnel. Steam pipe insulation wasted and unions exposed. Replace insulation as required.

Damage is located 10' inboard of starboard bow, 3” below the head log extending down a 24" long vertical fracture just on the inboard side of starboard tow knew skeg. Located at the top of the fracture, there is a 3" x 4" hole. Fracture and hole have been temporarily repaired utilizing epoxy to prevent water intrusion.
