Penetrations and openings in watertight bulkheads must be made watertight. Wires/cables were found passing through the watertight bulkhead between the engine room and salon which were not watertight.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the serious marine incident. The alcohol testing strips on board were expired. Cite: 46 CFR 4.06-15(a)(1)

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. The safety radio certificate was expired. Cite: 47 CFR 80.903

At each initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and shall conduct the inspections of the hull structure. During the COI inspection, the marine inspector observed an active leak IWO of the horn timber in the main salon. The minimum number of mariners holding a MMC officer endorsement as mate, that are required to be carried on every inspected, seagoing, passenger vessel must not be less than the following: Vessels of less than 100 GRT—one mate. The vessel was preparing to get underway for a 10 day voyage with only 1 credentialed mariner on-board. Vessel must not be underway (including anchor) for more than 12 hours in any 24 hour period until a licensed mate is aboard. Due to passengers having access to the vessel during the periods when the vessel is moored, the Captain shall designate, in writing, a senior deckhand to be in charge of the vessel while the credentialed master is resting. The senior deckhand must meet the requirements set forth in NVIC 01-91, "Recommended Qualifications for Small Passenger Vessel Deckhands". Vessel in not authorized to transit outside of Boston's OCMI zone until a licensed mate is onboard.

CABLES ENTERING BOXES OR FITTINGS SHALL BE PROTECTED FROM ABRASION AND SHALL CONFORM WITH THE FOLLOWING REQUIRMENTS. OPENINGS THROUGH WHICH CONDOCTORS ENTER SHALL BE ADEQUATLY CLOSED AND THE CABLE ARMOR SHALL BE SECURED TO THE BOX OR FITTING. NEWLY INSTALLED GENERATOR CABLING IS NOT PROPERLY SUPPORTED AND ELECTRICAL BOXES PENETRATIONS ARE NOT ADEQUATLY CLOSED IN THE FOLLOWING LISTED LOCATIONS. 1. BOTTOM SIDE OF MAIN GENERATOR BREAKER 2. CABLE FEEDS INTO GENERATOR HEADS 3. CABLE FEED FROM MAIN BREAKER TO RANGE SWITCH

If individual wires, rather than cable, are used in systems greater than 50 volts, the wire must be in conduit. Power wires from both generators to their main breakers are individual conductors. Place existing wires inside of properly sized conduit or replace existing wires with properly sized/rated cable.

The generator circuit breakers must be interlocked to prevent the generators from being simultaneously connected to the switchboard, except for the circuit breakers of a generator operated in parallel with another generator when the installation meets §§111.12-11(f) and 111.30-25(d) in subchapter J of this chapter. Current dual generator system is not designed for parallel operation and lacks the required circuit breaker interlock. Install a interlock system that prevents both generators from being placed online simultaneously.

Each watertight bulkhead must be of sufficient strength to be capable of remaining watertight with a head of water to the top of the bulkhead. Water ingress observed in the foc'sle space at the forward watertight bulkhead stbd side. Exact location of observed leak marked; however, point of ingress has yet to be determined. Determine cause and location of water ingress and submit report to attending marine inspector with plan to repair.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI. Vessel is replacing both ship service generators with installations that are not in-kind. Operator must submit plans for for approval to the Marine Safety Center.

Adequate emergency lighting automatically actuated upon failure of the main lighting system shall be fitted along the line of escape in vessel's having lounge areas below the main deck as per §177.30-7 of this subchapter. No emergency lighting installed to show path to emergency escape in forward accommodation space and fish hold accommodations space. Provide required emergency lighting. 705: No Overnight passengers

Electrical installations and equipment should be installed in a manner that will result in a vessel that is reasonably safe from fire caused by and/or originating in electrical equipment, and reasonably safe from electrical shock of personnel. At numerous locations throughout the vessel's accommodation spaces there are exposed direct current terminal strips and improperly maintained lighting fixtures that present potential shock and fire hazards. Many of these terminal strips and fixtures are located inside of bunk areas. Enclose or protect terminal strips throughout the vessel and make repairs to all damaged fixtures throughout the vessel to prevent electrical shock. 705: No overnight passengers

Each overnight accommodation space on a vessel with overnight accommodations for passengers must be fitted with an independent modular smoke detector meeting requirements of 181.450. No UL217 smoke detectors installed onboard vessel. Install UL217 smoke detectors as required. 705: No overnight passengers

Cables entering boxes or fittings shall be protected from abrasion, and shall be secured to the box or fitting. Starboard aft cable on main generator distribution switch in the engine room is not adequately secured to the box with a fitting. Secure cable entry into box with a fitting designed for enclosure type.

All escape hatches and other emergency exits shall be marked on both sides using at least 1-inch letters: "Emergency Exit, Keep Clear" Emergency escape hatch from the engine room to the galley is not clearly marked. Clearly mark emergency escape hatch from the engine room as required.

Flexible or portable cables cannot be used for permanently installed wiring. Numerous portable extension cords with deteriorated jacketing and numerous power strips found in use throughout all accommodation spaces. Remove all extension cords and power strips being used.

Except as otherwise provided in this section, all vessels shall be provided with not less than two avenues of escape from all general areas accessible to the passengers or where the crew may be quartered or normally employed, so located that if one is not available the other may be. Fwd emergency escape from fwd accommodations space had gear blocking the usage of the hatch. Clear pathway to allow for usage.

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Fire pump rotating motor coupling is not covered and creates a hazard while the fire pump is operating. Provide a suitable guard or cover to prevent contact with rotating parts.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radio beacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessels route requires a operational MF/HF single side band radio. Could not verify operation of Installed MF/HF single side band radio. Provide operational radio as required. 705: Vessel route restricted to no more than 20NM from land

Each garbage management plan under paragraph (a) of this section must be in writing and designate the person who is in charge of carrying out the plan. No person designated to carry out the plan. Provide a garbage management plan as required.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Vessel did not have a complete first aid kit. Provide approved first aid kit as required.

Life jackets are required to be marked with the vessels name. 19 life jackets were found not marked with the vessels name. Mark all lifejackets as required.

A periodic examination made in accordance with the requirements of this subpart shall be of sufficient scope to determine that these items are in a satisfactory conditions for the service intended. Excavate rot in lazarette deckboards and renew to satisfaction of [name withheld] CORRECTED PRIOR TO 2019 SEASONAL SOUTH VOYAGE 46 CFR 176.15-5

Vsl is broadcasting greater than expected vessel length and incomplete dimension information (AIS Antenna Position “X” equals 0)

Vessels operated beyond 20nm must be equipped with a MF-DSC (single side band) radio. Vessel’s single side band radio is inoperable. 47 CFR 80.905(a)(2) Route limited to 20nm.

All machinery shall be installed to afford protection against fire. Slow fuel leak observed from fuel pump on Engine room fwd bulkhead.46 CFR 182.01-5

Wiring joints and splices shall be mechanically secure and in junction boxes. Dead end wires in paint locker aft, in battery space, and on new generator install shall be removed or terminated in junction box. Terminal block in Foc’sle port aft bunk shall be mounted. 46 CFR 183.05-50.

Dry pipe vertical exhausts are permissible if suitably insulated. Lagging is missing on upper portion of main engine exhaust. 46 CFR 182.20-15

The number and identification of the items stowed inside and their sizes must be marked in clearly legible letters and numbers on each container for life jackets. Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets. Life jacket stowage not properly marked and child jackets not segregated. 46 CFR 185.604(f); 46 CFR 180.78(a)(5)

Placards containing instructions for the donning and use of life jackets aboard the vessel must be posted. No donning placard posted.

Found 03 life jackets with inop water lights, 01 jackets requiring stenciling, and required 02 additional life jackets in ordrer to comply with COI. Vessel total passenger count reduced to 74.

Vessel must stay one mile from shore while pirmary lifesaving is being serviced.
