STROLLER IV

This subpart summarizes the responsibilities of documented and licensed mariners, marine employers, MRO, SAP and other chemical testing service providers in 49 CFR part 40. Conduct a DAPI audit in accordance with chemical testing program compliance audit checklist.

A vessel must carry either a first aid kit approved under series 160.041 or a kit with equivalent contents and instructions. MI observed expired medications in first aid kit. Replace with approved kit within 7 days. 46 CFR 184.710

The following spaces must be equipped with a fire detection and alarm system of an approved type installed in accordance with 46 CFR part 76, except when a fixed-gas fire extinguishing system that is capable of automatic discharge upon heat detection is installed or when the space is manned: (1) A space containing propulsion machinery. Vessel does not have fire detection in the machinery space. Vessels heat detectors for their fixed gas fire extinguishing system failed to operate and were not properly installed. 46 CFR 181.405

Cylinders continuously in service without discharging shall be permitted to be retained in service for a maximum of 12 years from the date of the last hydrostatic test. Vessels Fixed gas fire-extinguishing system cylinder is over due for a hydro static test. 46 CFR 175.600 46 CFR 147.60 NFPA 12 NFPA

ilge Alarm: A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Vessel does not have an indicator light installed at the operating station indicating the Bilge Pump is operating. 46 CFR 182.530

Engine Exhaust: Exhaust gas must not leak from the piping or any connections. Vessels exhaust coupling at the turbo is deteriorated and the internal bands are exposed. Replace/ repair exhaust coupling. 46 CFR 182.430

There must be a means of accurately determining the amount of fuel in each fuel tank either by sounding or by an installed marine type fuel gauge. Vessels fuel gauge does no accurately display the amount of fuel in the tank. 46 CFR 182.445(b)

Portable fire extinguishers other than wheeled extinguishers shall be installed using any of the following means. (1) Securely on a hanger intended for the extinguisher (2) In the bracket supplied by the extinguisher manufacture. (3) In a listed bracket approved for such purpose. Fire extinguisher by the operating station was not installed in a manufactures approved bracket. 46 CFR 175.600 NFPA 10

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions; (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; (c) Minimize accidental personnel contact with energized parts. Vessel has multiple spliced wires throughout the vessel and wires not indicating which circuit they are protected by. Repair spliced wires and label circuits so they are readily and easily identifiable. 46 CFR 183.200

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: (7)Inspection of all interior spaces to ensure that they are adequately ventilated and drained. De-water bilges in order for CGMI to inspect bilges. 46 CFR 176.802

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Due to multiple shaft failures, the vessel was previously required to submit detailed plans to the Marine Safety Center for review and comment. The Marine Safety Center came back stating that the current arrangement does not meet ABYC or ABS standards and that the current shaft is undersized given the details provided. Vessel must either submit revisions to previously submitted plans that show the current shaft arrangement meets ABYC or ABS standards and gain MSC approval or submit changes to their current design and gain approval from the Marine Safety Center. If changes are required to the current design, all alterations will need to be overseen by an attending marine inspector for installation and workmanship. No passengers are allowed to be carried until this deficiency is rectified.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel's connection to transmission via sheer pin failed causing vessel to loose propulsion and maneuverability while underway with passengers. Vessel must satisfactorily repair failed sheer pin and test under operational loads in the presence of an attending Marine Inspector. Vessel is forbidden from carrying passengers until such time as this deficiency is cleared.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Vessel's crew is required to visually inspect previous replaced sheer pin (beginning today) once a week and record visual inspection in ships log. Vessel must also submit detailed propulsion system casualty/repair history and the following information concerning the propulsion system (bill of materials, design calculations, and engine specifications) to the USCG Marine Safety Center (MSC) for review and comment. Any recommendations returned from MSC will then be reviewed by the O/O and Sector Buffalo Inspection Staff for possible further action.

Gages to indicate oil pressure shall be provided for propulsion engine. The gauge on the vessel is not currently functioning. Provide proof of correction within 14 days. 46 CFR 182.20-5 (b)

The life ring on the stern of the vessel is wasted and needs to be replaced or repaired. Replace or repair within 14 days and provide proof to Coast Guard.

Multiple items in the first-aid kit are expired. Replace within 14 days and provide proof to Coast Guard.

The battery on the water light that attaches to the ring buoy is expired. Replace within 14 days and provide proof to the Coast Guard.

During the test of the fixed fire extinguishing system, the tech rep was not able to show a test of the 02 heat activated devices nor provide manufacturers testing procedures. Prove proper operation of 02 heat activated devices to the satisfaction of a Coast Guard Marine Inspector prior to carrying passengers.

No major repairs or alterations affecting the safety of the vessel shall be made without approval from the Office in Charge of Marine Inspections. All items listed below will require a written repair proposal submitted to our office within 07 days: 1. New Shaft Arrangement 2. Planking on the Port Side 3. All repairs of frames on the port side. 4. Transom caulking issues to include bottom planking and exhaust piping. 5. Frames in the area forward of the collision bulkhead. 6. Collision bulkhead repair.

Received notification from the owner/operator that during a trip their shaft sheared off from the transmission and dropped into the bilge rendering the vessel with no means of propulsion. All machinery must be installed and maintained in such a manner as to afford adequate protection against machinery failure. Provide the specifications for the new shaft that will be installed and prove proper operation to Coast Guard Marine Inspector prior to carrying passenger.

On the 19Jun2018 Deficiency Check, we discovered water in the bilge that appeared to be running steadily from aft to forward collecting near the bilge pump under the operating station. We were not able to determine if the water was coming from rain water into the aft hatch or from the hull. The bilge needs to be kept dry and monitored over the next 30 days to determine if further actions needs to be taken.

The emergency procedures for the vessel to be written for that specific vessel's operation. The man overboard procedures state that they will put a crewmember in the water and the fire drill procedures state the Coast Guard would be called in an emergency. Both of these are not accurate to the vessel's operation. Update to reflect accurate procedures.

At least one ring life buoy must be fitted with a lifeline. The 60ft line attached to the ring life buoy is wasted and needs to be replaced.

A vessel must carry one floating waterlight. The battery on the waterlight was marked 2016. The battery needs to be replaced annually.

Electrical equipment exposed to the weather must be watertight. Wires going to the stern light were made with an unauthorized splice. Wires need to be spliced in accordance with 46 CFR 183.340 (g)

The vessel's crew was not able to demonstrate the ability or knowledge to render basic medical treatment during a man overboard drill. The crew needs training to be able to provide this treatment.

The crew should be trained prior to getting underway for emergency situations to include responding to a fire. The vessel's crew was not able to explain how to operate the fuel shut offs. Provide training to all crew members prior to getting underway.

Provide the most updated copies of charts, Coast Pilot and Light List on the vessel for the waters operated.

The first aid kit on board the vessel contained expired aspirin and neosporin. The kit needs to be updated with new items.

A small amount of water is coming in through the deck on the stern. Water was found in the port aft corner of the transom. Find source of water intrusion and correct.

Provide adequate storage for life jackets, demonstration required.

Replace five (05) adult type I life jackets, vessel is limited to 45 passengers until corrected.

Reduce excessive leaking from shaft.

Mark fire buckets as required by cite.

Remove from vessel & destroy 3 adult & 1 child life jackets found unsatisfactory condition.

Provide corrosion resistant clip for 3' lanyard for life buoy. When stowed the line must be attached around the body of the ring buoy.

Provide battery for water light with current expiration date or replace battery annually.

e-water all spaces & maintain dry & free of oil. Verification by Masters required prior to each daily operation. Entry in vessels log shall be completed.

Identify leaking seams in mid-body port side & transom; inspect wood & replace planks or repack as appropriate to make vessel watertight. Make vessel available for in water internal inspection prior to 2017 operating season.

Provide annual servicing record for portable & fixed fire extinguishers.
