Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Hydro static release and registration is expired. Replace and renew. Vessel is limited to no more that 3 miles from shore until rectified.

Each life jacket carried on a vessel on a Great Lakes route, must have a life jacket light. Provide a light for each life jacket. The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. The emergency instructions must be designed to address the particular equipment, arrangement, and operation of each individual vessel. Emergency instructions do not include heavy weather provisions. Response to fire does not include instructions to shut off fuel supply. Update emergency instructions. See 185.512 for further guidance.

The master or person in charge of a ship, including a drilling rig or platform, to which this section applies shall ensure that one or more placards (Garbage) meeting the requirements of this section are displayed in prominent locations and in sufficient numbers so that they can be read by the crew and passengers. For ships while operating on the Great Lakes or their connecting or tributary waters, the placard must—Notify the reader of the following: the discharge of all garbage into the Great Lakes or their connecting or tributary waters is prohibited. A person who violates the above requirements is liable for a civil penalty for each violation, and the criminal penalties of a class D felony. Garbage placard onboard does not match operations on great lakes. Update placard.

Alcohol testing must be conducted on each individual engaged or employed on board the vessel who is directly involved in the SMI (Serious Marine Incident). The alcohol testing of each individual must be conducted within 2 hours of when the SMI occurred, unless precluded by safety concerns directly related to the incident. Alcohol test strips are not onboard. Provide test strips.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible. Radio Technical Commission for Maritime Services (RTCM) approved electronic chart display found not being updated. Update charts on display.

Each flare carried to meet this section must be stowed in one of the following: A portable watertight container marked as required by § 185.614 of this chapter, carried at the operating station. Flares found not stored in watertight container. Provide container. 46 CFR 180.68(e)(1)

Each U.S. flag vessel subject to this subpart must have an inspection of the required radiotelephone installation at least once every 48 months (Great Lakes Radiotelephone Inspection). This inspection must be made while the vessel is in active service or within not more than one month before the date on which it is placed in service. Inspection not conducted. Provide report/certificate of inspection.

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements:(1) Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license.(2) Where the station power exceeds 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a general radiotelephone operator license or higher class license. No crew member has a valid FCC Marine Radio Operator Permit. 47 CFR 80.159(e)

Medical certificates will be issued for the following periods of time:(2) Medical certificates issued to all other mariners will be issued for a maximum period of 5 years. Vessel captain had no valid medical certificate. 46 CFR 10.301(b)(2)

Flexible nonmetallic hose may be used in lube oil, fuel oil and fluid power systems, subject to the following... Flexible hose designed for use with hose clamps may be installed with two clamps, at both ends of the hose, which:(1) Do not rely on the spring tension of the clamp for compressive force; and(2) Are installed beyond the bead or flare or over the serrations of the mating spud, pipe, or hose fitting. Hose associated with fuel oil filter had an end with only one clamp. Install a second hose clamp to that end. 46 CFR 182.720(e)(3)(v)(B)

Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Wire for lighting above engine compartment cover leading to battery is installed so that when engine/battery compartment cover is shut, it becomes pinched, creating a potentially unsafe condition. Make safe or remove pinched wiring. 46 CFR 183.200(b)

A power-driven (and/or sailing) vessel underway shall exhibit..sidelights. Starboard side navigation light was partially burnt out. Replace light/bulb. 46 CFR 183.420, 33 CFR 83.23(a)(iii), 33 CFR 83.25(a)(i)

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. 12VDC electrical bus bar servicing shipboard lighting unprotected. 46CFR183.200(c)

At each initial and subsequent inspection for certification of a vessel, the owner shall be prepared to conduct tests and have the vessel ready for inspection, including all spars, standing rigging, running rigging, blocks, fittings, and sails. Vessel specific preventative maintenance plan (PMP) not onboard IAW NVIC 02-16. 46 CFR 176.802(a)(3)

Your vessel must undergo an annual inspection within the 3 months before or after each anniversary date. The vessel was due for inspection no later than 27 April. The vessel is unable to carry passengers until successful completion of an annual inspection, currently scheduled on 06MAY2024.

Employers must maintain records of chemical tests as provided in 49 CFR 40.333 and must make these records available to Coast Guard officials upon requests. Drug and alcohol program records were not available for inspection.

The employer shall provide an Employee Assistance Program (EAP) for all crewmembers [name withheld] program must include at least the following elements: display and distribution of a community service hot-line telephone number for crewmember assistance. EAP telephone number was not provided for crew assistance.

Each battery used in an item of lifesaving equipment must be replaced during the annual inspection if the expiration date of the battery has passed. Port aft lifering battery light expired.

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. Vessel logged showed minimal fire drills conducted during previous 12 months. Conducted satisfactory fire drill with crew.

A person may not employ or engage an individual in a position required to hold an MMC unless that individual maintains a current medical certificate. Master’s medical certificate was not available for inspection. Captain provided medical certificate.

46 CFR 180.70 (b) Ring Buoys Each ring life buoy on a vessel must: (1) Be approved in accordance with 46 CFR 160.050 in subchapter Q of this chapter. Grab Line on port and starboard buoys frayed. Replace both grab lines.

EACH CHILD SIZE LIFE JACKET MUST BE STOWED IN A LOCATION THAT IS APPROPRIATELY MARKED AND SEPARATED FROM ADULT LIFE JACKETS SO THE CHILD SIZE LIFE JACKETS ARE NOT MISTAKEN FOR ADULT LIFE JACKETS. MI FOUND THAT THE CHILD SIZE LIFE JACKETS WERE NOT SEPARATED FROM THE VESSEL'S ADULT SIZE LIFE JACKETS. 46 CFR 180.78(a)(5)
