The remote fuel shutout valve was not marked showing purpose of valve. Remote fuel shutout valve must be marked in clearly legible letters at least 1 inch high indicating purpose of the valve and direction of operation. Mark fuel shut out valve in accordance with 46 CFR 185.608 to the satisfaction of the attending Coast Guard Marine Inspector.

Past drills were not logged or documented. The master shall conduct sufficient drills and give sufficient instruction to make sure that all crew members are familiar with their duties during emergencies that necessitate abandoning ship or recovery of persons who have fallen overboard. Once each month if reasonable and perfectible but at least one within 3 months period. Provide records or documentation drills are being completed to the satisfaction of the attending Coast Guard Marine inspector.

Vessel FCC permit was not on board in accordance with 47 CFR 80.13 Obtain and provide vessel FCC permit to the satisfaction of the attending Coast Guard Marine Inspector.

Vessel did not have current COI onboard. The certificate inspection must be posted under glass or suitable transparent , material such the that all pages are visible, in a conspicuous place where observation by passengers is likely. Place COI onboard to the satisfaction of the attending Coast Guard Marine Inspector.

On a vessel authorized to carry one or more vehicles, suitable chains, cables, or other barriers must be installed at the end of each vehicle runway. In addition, temporary rails or equivalent protection must be installed in way of each vehicle ramp, in compliance with the following: Rails or equivalent protection must be installed near the periphery of all decks of a vessel accessible to passengers and crew and must be be designed and constructed to withstand a point load of 200 pounds applied at any point in any direction, and a uniform load of 50 pounds per foot applied to the top rail in any direction. Please consult with this office before installation to ensure that it meets the intent of this regulation.

The Master shall take any necessary precautions to see that automobiles or other vehicles have their motors turned off and their emergency brakes set when the vessel is underway, and that the motors are not started until the vessel is secured to the landing. In addition, the vehicles at each end shall have their wheels securely blocked, while the vessel is being navigated.

The power fire pump shall be self-priming and of such size as to discharge an effective stream from a hose connected to the highest outlet. All piping, valves, and fittings shall be in accordance with good marine practice and suitable for the purpose intended.

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Some of the replacement lifejackets you have provided for your vessel are the new style and require different donning instructions in addition to the ones you already have.

Diagnose the cause for the loss of propulsion that occurred and make any necessary repairs. Prior to getting underway with passengers, demonstrate proper operation of the electric motor and the propulsion system to the satisfaction of an attending Coast Guard Marine Inspector.

Crop and renew angle under port forward deck structure.

Replace worn bolts and fasteners in spindle bearings.

Repair fractured welds on ramp support angles.

Repair fractures on vehicle deck.

Crop and renew channel in aft tank port side.

Remove scale and rust in all voids, make repairs, and recoat.

Properly route wiring to machinery space.

Deck plating inboard of manhole cover has 1/4 fracture.

Make permanent repairs to Centerline Bulkhead located between #2 Starboard and #2 Port voids at deck between transverse bottom beams IAW NVIC 7-68.

Portside void #2, Crop and Renew 1st & 2nd transverse bottom beams from ast bulkhead adkacent to center line bulkhead where wasted IAW NVIC 7-68.

Starboard void #2, Crop and Renew 1st transverse bottom beams from ast bulkhead adkacent to center line bulkhead where wasted IAW NVIC 7-68.

Repair or Replace leaking diesel tank. Prove tank meets requirements of 46 CFR 182.440.

Repair area around #2 Starboard void cover where wasted IAW NVIC 7-68.

Repair cracked welds on stern ramp on underside center near hinges IAW NVIC 7-68.

Replace main deck plate where wasted in area of ladder to pilot house IAW NVIC 7-68.

MARK APPROPRIATE LIGHTING CIRCUITS IN PILOT HOUSE ELECTRICAL DISTRIBUTION PANEL NOT LATER THAN THE COMPLIANCE DATE.

ATTACH NOZZLE ON 5/8" FIRE HOSE NOT LATER THAN THE COMPLIANCE DATE.

REPLACE ALL ELECTRICAL WIRE NUTS WITH PRESSURE TYPE WIRE CONNECTORS NOT LATER THAN THE COMPLIANCE DATE.

MAKE PERMANENT REPAIR TO THE HOLED #4 STARBOARD VOID VENTILATION DUCT NOT LATER THAN THE COMPLIANCE DATE.

REPAIR OR REPLACE INOPERABLE RING BUOY WATERLIGHT PRIOR TO NIGHT TIME OPERATION, BUT NOT LATER THAN THE COMPLIANCE DATE.

PERMANENTLY MOUNT A FIXED FIRE PUMP ON BOARD THE VESSEL. THE SUCTION LINE FOR THIS SYSTEM MAY BE OF A TEMPORARY DESIGN, PROVIDED IT IS ADEQUATELY PROTECTED. A PERMANENT SUCTION LINE SHALL BE CONNECTED TO THE SEA CHEST PRIOR TO NEXT REINSPECTION.

REPLACE THE LINE ON THE LIFE RING.

PLACE ADEQUATE GUARDS ON CHAIN SPROCKETS, CHAINS AND FAULK COUPLINGS ON THEPROPULSION MOTORS AND CHAIN ASSEMBLY ON BOTH SIDES OF THE VESSEL.

REPLACE THE FALLS ON THE RESCUE BOAT.

PROVIDE PROPER BRACKETS FOR ALL ONBOARD FIRE EXTINGUISHERS.

PROVIDE A NEW JUNCTION BOX AT THE KENTUCKY END PORT RUNNING LIGHT.

PROVIDE 11 ADDITIONAL ADULT LIFE JACKETS TO REPLACE DESTROYED JACKETS.

VESSEL RESTRICTED TO CARRIAGE OF NOT MORE THAN 36 ADULTS UNTIL THE PROPER NUMBER OF PFDS ARE PROVIDED AND INSPECTED.

BOTTOM PLATING CUT AWAY WHERE HOLED. NEW PLATE 48" X 20" INSERT WELDED IN PLACE OF OLD PLATING.

REMOVE ALL RENEWABLE TYPE FUSES FROM THE VESSEL'S ELECTRICAL SYSTEM, RE- PLACING THEM WITH PROPER EQUIPMENT. THIS ITEM IS TO BE CORRECTED PRIOR TO PLACING THE VESSEL IN SERVICE.

PROVIDE WATERPROOF FIXTURES FOR THE EXTERIOR LIGHTING. THESE UNPROTECTED LIGHTS ARE TO BE OPERATED IN FAIR WEATHER ONLY UNTIL PROPERLY PROTECTED.
