While corrective actions were in process, crew could not show that all items had been sufficiently addressed. Implement corrective actions and provide documentation proving completion of discrepant items in the July2022 FSS annual servicing report.

The emergency equipment provided, regardless of whether or not required by this subchapter, shall be maintained in good condition at all times. During spot checks of the lifesaving capsules onboard, observed the winch brake on Lifeboat #4 to be "stuck" and preventing the boat from being lowered from the stowed position in the davit. While the crew was able to free the brake, this is an atypical occurrence on this particular make/model of equipment. Also observed a removed cotter pin on top of the winch for Lifeboat #1. While the winch was satisfactorily tested, it was not apparent if the cotter pin was missing or left over from servicing. The servicing reports for the four boats currently onboard (#1, 2, 4 and 5) also had corrective items listed without documentation attesting to completion. Confer with Survival Systems International (SSI) (OEM and the last service provider) to determine why the brake stuck on Lifeboat #4 and any mitigations to prevent future occurrences and ensure that brake components on Lifeboat #1 were correctly restored to operational condition. Provide documentation showing discrepancy corrections for items listed in the July/September2022 SSI servicing reports.

Each holder of a lease or permit under the Act shall ensure that all places of employment within the lease area or within the area covered by the permit on the OCS are maintained in compliance with workplace safety and health regulations of this part and, in addition, free from recognized hazards. Observed several stowage cabinets in the SW Column upper utility level not secured that could tip during dynamic motion of the facility, a spare portable extinguisher improperly secured for an extended period in Turbine Generator Room #2 (ZAN-702) and an eyewash station outside of the N door to the MCC Room lacking enough pressure to displace the protective cover. These examples pose a risk to personnel onboard that can easily be overlooked. Conduct a full walk-through of the facility to ensure that all such hazards are correctly mitigated. A statement from the PIC attesting to completion of the same is sufficient documentation to show completion of this requirement.
