The emergency equipment provided, regardless of whether or not required by this subchapter, shall be maintained in good condition at all times. The annual servicing report from Fire Safety Systems (FSS) listed discrepancies and required corrective actions for portable and fixed fire extinguishing and suppression systems and fire detection system. While corrective actions were in process, crew could not show that all items had been sufficiently addressed. Implement corrective actions and provide documentation proving completion of discrepant items in the July2022 FSS annual servicing report.

The emergency equipment provided, regardless of whether or not required by this subchapter, shall be maintained in good condition at all times. During spot checks of the lifesaving capsules onboard, observed the winch brake on Lifeboat #4 to be "stuck" and preventing the boat from being lowered from the stowed position in the davit. While the crew was able to free the brake, this is an atypical occurrence on this particular make/model of equipment. Also observed a removed cotter pin on top of the winch for Lifeboat #1. While the winch was satisfactorily tested, it was not apparent if the cotter pin was missing or left over from servicing. The servicing reports for the four boats currently onboard (#1, 2, 4 and 5) also had corrective items listed without documentation attesting to completion. Confer with Survival Systems International (SSI) (OEM and the last service provider) to determine why the brake stuck on Lifeboat #4 and any mitigations to prevent future occurrences and ensure that brake components on Lifeboat #1 were correctly restored to operational condition. Provide documentation showing discrepancy corrections for items listed in the July/September2022 SSI servicing reports.

Each holder of a lease or permit under the Act shall ensure that all places of employment within the lease area or within the area covered by the permit on the OCS are maintained in compliance with workplace safety and health regulations of this part and, in addition, free from recognized hazards. Observed several stowage cabinets in the SW Column upper utility level not secured that could tip during dynamic motion of the facility, a spare portable extinguisher improperly secured for an extended period in Turbine Generator Room #2 (ZAN-702) and an eyewash station outside of the N door to the MCC Room lacking enough pressure to displace the protective cover. These examples pose a risk to personnel onboard that can easily be overlooked. Conduct a full walk-through of the facility to ensure that all such hazards are correctly mitigated. A statement from the PIC attesting to completion of the same is sufficient documentation to show completion of this requirement.

The compressed air start piping on the fire pumps is rusted and deteriorated. Make repairs to wasted piping necessary to start fire pump. 33CFR142.4

Each cable installation must meet sections 25, except 25.11 of IEEE 45-2002. Cable installations through bulkheads in elec. workshop (2) were installed incorrectly. Audit and make repairs to incorrectly installed transits. 46CFR111.60-5(a)

The transfer procedures required by 33CFR 155.720 must contain... Ensure transfer procedures contain all required information. 33 CFR 155.750(a)

Diesel vent wasted on the storage tank (southwest hull column), crop and renew. 46CFR 56.50-85

Operating instructions for launching lifeboats are missing at multiple locations. Post lifeboat launching instructions.

Emergency instructions for donning lifejackets are missing at both muster locations. Provide instructions for donning lifejackets.

Liferafts are not marked with appropriate symbols for stowage or numbered. Mark all liferafts and stowage locations.

Emergency lights throughout the unit are not marked or improperly marked. Conduct survey of unit and mark all emergency lights.

Lifejackets for the FRB are of recreational/whitewater rafting use and are not CG approved. Replace lifejackets or workvests with CG approved type.

3 of the 4 emergency egress ladders leading to the waterline are secured due to unsafe/excessive corrossion. At a minimum, two means of escape are required from the unit. Repair or replace egress ladders for use.

Liferafts stowed at SW column are stowed such that rafts are unable to launch. Reinstall liferafts for float free launching.

Multiple eyewash stations throughout unit have inadequate/low pressure. Flush and prove proper operation of all eyewash stations.

An "A" class boundary in dry stores above the ANSIL system does not appear to be an approved installation, with red firestop installed over fiberglass containing multiple holes. Provide documentation that installation is approved.

Galley door leading from deck 1 to dry stores does not self close. Adjust door to properly close to maintain Class "A" fire boundary.

Laundry door latch does not close, allowing a gap which could allow for the spread of fire and smoke from a laundry fire to spread on upper accommodiation corridor. Replace latch to correct class "A" fire boundary.

Fire and abandonship drills were poorly executed. Fire teams were unable to quickly don firefighting equipment or activate installed face mask comms equipment. Also, during abandonship drill a missing man pulled by the CG during the drill was mistakenly accounted for. OIM shall provide documentation to CG of proper donning and use of firefighting equipment.

General arrangement and FCP has been revised multiple times and is inaccurate and must be reviewed and re-approved by the CG.

HVAC chase door on 3rd deck in stairwell has been drilled with four holes, compromising A class boundary. Replace chase door.

Obstruction lights are not flashing in unison nor displaying proper flashing characteristics (60 times a min). Repair or replace obstruction lights.

Louver for #2 generator room does not fully close, compromising FM200 system seal. Repair or replace louver.

Laundry chute door does not latch closed. Repair latch to original condition.

The fire doors to the main stairtower on the first and third deck were modified and holes drilled into the door. Renew fire doors with approved fire doors.
