The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in an SMI. Provide approved alcohol testing devices on board.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Two adult life jackets were found to be in unserviceable condition and were removed from the vessel. Until the 2 adult life jackets are replaced, the passenger count on board is reduced by 2 passengers. Total persons on board reduced to 49.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have at the operating station, a means to reset devices used to automatically shut down ventilation systems and engines as required by paragraph (a)(3) of this section. The override switch was found non-operational.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in: A space with a through-hull fitting below the deepest load waterline; a machinery space bilge or other spaces subject to flooding from sea water piping within the space; and a space with a non-watertight closure. The engine room bilge audible and visual alarm at the operating station was non-operational.

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. Renew the grabline and 60' lifeline (5/16 inch, buoyant, UV resistant) on the vessel's ring life buoy.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. The required analgesics and burn gel were expired.

On a vessel of at least 26' in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each unmanned space. The high aft high water alarm in the engine space is inoperable. Conduct half hour rounds while underway and log entry into logbook till repairs are made.

A marine employer whose vessel is involved in a casualty or incident, that is, or likely to become, a serious marine incident, shall comply with the requirements of 46 CFR 4.06 in Subchapter A of this chapter. Vessel had expired alcohol swabs. Replace with updated swabs.

Submit electrical line diagram of vessel to the this office and the Marine Safety Center for approval.

A propulsion engine control system, including pilothouse control must be designed so that a loss of power to the control system does not result in an increase in shaft speed or propeller pitch. Engine Throttle Cable terminates at an electronic potentiometer mounted to the engine room overhead, which is connected to the main engine control system via several feet of electrical wire. Conduct power failure testing of the electric propulsion engine controls in accordance with the Marine Safety Center (MSC) accepted and reviewed testing protocols following the substantial upgrade of the propulsion engines. The cognizant OCMI may require that a vessel and its equipment under-go any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Conduct test of propulsion engine control module (ECM) to assess any de-rating, shutdown or condition that otherwise takes away control from the operator [name withheld] Letter 17-08 for amplifying information.

Repair Frame 5 port side, Frame 6 stbd side and in tank space, repair Frame 4 port side.

Install junction box or heat shrink engine room lighting wiring where line is spliced together.

Remove fuse in shore power wiring and properly place in distribution panel.

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. An extension cord is being used for the PA system in the forward space.

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. No portable lights onboard.

Provide 1st aid and CPR training for operator and crew. No crew members had recency in 1st aid and CPR.

Replace expired contents in 1st aid kit.

Vessel missing 2 fire buckets. Provide 2 fire buckets.

Vessel has undergone upgrades/repairs affecting vessel's stability. Net weight change has been documented 517lbs. Total number of persons reduced to 48 and passengers reduced to 46.

Found 1 inch tear on starter cable exposing conductor on port engine. Replace cable.

Water light for life ring inoperable. Repair light.

Engine room bilge high-level alarm visual inoperable. Repair/replace indicator.

Fuel tank fill on port side tank was found without bonding wire. Provide bonding wire for port side tank.

Engine guards on both engines were loose or with cracked brackets. Repair engine guards to protect crew in the space.

Found holes in the bottom of the life jacket container above aft engine room bulkhead. Plug holes with suitable material.

Fwd hatch is propped open w/ anchor stock. Each hatch exposed to weather must be watertight. Replace anchor with smaller suitable anchor for vessel size to be placed in space or find another location to stow anchor.

Repair / replace high water alarm indicator led.

assign MMSI number to VHF radio.
