Crew identified water entering into #1 stbd bwt below waterline. Vessel must make temporary repairs prior to departure. Permanent repairs shall be completed by March 2027.

All boilers which have repairs shall be subjected to a hydrostatic test of 1.5 times the mawp of the boiler. While removing mounts for 10 year inspection, a pipe connection on top of the starboard steam drum stripped its threads and needs to be repaired. The old pipe was removed and the threads into the drum were found to be bad. Schwarts Boiler Shop provided repair proposal to re-tap threads for 1". Boiler will need to be hydrostatically tested at 1.5x the MAWP once repaired.

Periodic safety test procedures must be submitted to the Coast Guard for approval. No approved PSTPs on board for SSDGs. Vessel must draft and submit PSTPs to MSC for review and approval. Company submitted PSTPS to MSC for review. Company must provide approved procedures prior to fit out in 2027.

Before the vessel leaves port and at all times during the voyage each lifesaving appliance must be in working order and ready for immediate use. The rescue boat has an approximately 1" by 2" piece of gel coast missing with exposed fiberglass. The viking tech on board recommended that the area be sealed with paint or other sealant.

Each muster station and embarkation station must be adequately illuminated with power supplied from the vessel emergency source of power.

Alcohol testing decives must be used according to the procedures specified by the manufacturer. Alcohol testing device was not calibrated and there was no personnel trained in the use of the equipment.

The steering system switched to Non-Follow up mode after the gear failed in Follow up mode. The ship shall prove proper operation of the steering gear prior to departure and must utilize sufficient tug assistance if they chose to moor in the Port of Cleveland. The Master shall keep the Marine Inspector abreast of changing conditions, and intentions aboard the ship.

All engineering systems shall be in accordance with the requirements in subchapter F. Main propulsion is inoperable due to rotors for main turbine requiring repair. Prove proper operation of main propulsion prior to departure.

Generator for cargo unloading equipment is leaking lube oil.

Vessel has rivets leaking water into the #1 Port Ballast tank at frames 29, 36, and 37. Vessel is restricted from movement from current position on Calumet River Turn Basin #5 until ABS Surveyor attends vessel and submits a repair or plan proposal to the local OCMI for review and approval.

The time delay on the fixed CO2 fire fighting system did not activate the release of agent within the maximum allowed time of 20% beyond the set time.

#1 Port Ballast Tank, temporary repairs made between frames 22 to 28 at side shell, and in way of second longitudinal frame from tank top. Temporary repairs included welding two doubler plates on 2nd longitudinal frame; one doubler 3/8 inch steel plate, apx 8 inch by 12 inch added iwo frames 23-24. Second doubler 3/8 inch steel plate, apx 15 inch by 15 inches added iwo frame 26-27. Additional weldment added in way of observed fractures and brackets. 2nd longitudinal frame from tank top, at frame 23 appears excessively distorted. Recommend with the #1 Port Ballast port side shell and frames between frame 22 (Bow Thruster Tunnel) to frame 28 (Swash or intermediate transvers bulkhead) be closely examined when making permanent repairs. Permanent repairs required to remove doublers installed and to renew any or all excessively distorted or fractured frames between frames 22 & 28 on or before March 31, 2020. IAW 46 CFR 92.01-10

During a drydock extension examination, damage was discovered in the #1 port ballast tank at frames 22, 24, 28 and 36. Provide a repair proposal and complete repairs to the damage within 14 days to the satisfaction of the attending Coast Guard Marine Inspector.

Make permanent repairs to the #6 to #7 Port & Stbd Ballast Tanks Transverse Water Tight Bulkheads, FR 176, to cut, crop out and renew steel in way of temporary weld repairs. Temporary weldments apx 12 inches to 18 inches long were made to WTB's, with temporary weldments made on each side of the bulkheads, all located approximately 5 ft above the bottom shell and apx 8ft inboard of the side shell. Permanent repairs shall be completed on or before March 31, 2021, and shall be repaired to the satisfaction of the attending CG Marine Inspector.

Continuous blowdown overboard discharge for boiler and evaporator leaking water. Complete permanent repairs prior to next annual inspection.

The vessels external covers over the man-hole covers of the power boilers have small amounts of water leaking from them.

Vessel reported fathometer inoperable. Sector SSM issued a Letter of Deviation (LOD) to allow the vessel to transit to Detroit then conduct repair. Sector SSM received a request for a second LOD since the fathometer was unable to be repaired within the time period of the last LOD. LOD denied, make permanent proper repairs to the fathometer to the satisfaction of the attending marine inspector within a week. Vsl is limited to route between Alpena, [name withheld], WI, and Detroit, MI.

The bulkhead between the #2 and #3 ballast tank located inboard near the slope has an approximate 12" fracture allowing free communication between the two ballast tanks. The fracture has been repaired with a doubler as a temp repair. Vessel is to affect permanent repairs no later than 31JAN19. Repair is to be a crop and renew and must be verified by an attending CG Marine Inspector.

Doubler plate installed on forepeak tank top deck plate located at frame 8CL directly forward of anchor-chain hausepipes as a temporary repair. Provide gauging report and submit permanent repair plan to Coast Guard.

Found the starboard side channel at frame #21 to have an inset, in addition the six rivets that were caulked were loose with a minimal amount of water leaking and the three rivets below those were considerably loose and required repairs. Also found the #2 stringer rivets were loose (those rivets were not attached to the hull). Requsested a repair proposal be provided to CGMI for permanent repairs.

The leaking rivets were a hazardous condition on board the vessel. ANDRIE Inc. did not immediately report the hazardous condition to the nearest OCMI as required by 33 CFR 160.216.

General arrangement plan needs to be updated to reflect changes in equipment and stowage.

The new rescue boat davit installation does not have an approved plan/drawing for the electrical installation. Electrical plans shall be submitted and approved by USCG MSC, and verified by a CG Marine Inspector on or before 14 October 2016.

Repair emergency generator main bus bar circuit breaker.

The electrical cable installed between the emergency switch board buss bar to the new rescue boat davit breaker could not be proved to be of a properly approved type of electrical cable IAW 46CFR111.60. Vessel to have the cable renewed with an approved marine type electrical cable IAW 46CFR111.60. Or prove compliance of the exisiting cable.

Mold has formed along the canopy edge on the top bouyancy tube. The fabic is compromised due to the mold.
