All boilers which have repairs shall be subjected to a hydrostatic test of 1.5 times the mawp of the boiler. While removing mounts for 10 year inspection, a pipe connection on top of the starboard steam drum stripped its threads and needs to be repaired. The old pipe was removed and the threads into the drum were found to be bad. Schwarts Boiler Shop provided repair proposal to re-tap threads for 1". Boiler will need to be hydrostatically tested at 1.5x the MAWP once repaired.

The rescue boat has an approximately 1" by 2" piece of gel coast missing with exposed fiberglass. The viking tech on board recommended that the area be sealed with paint or other sealant.

#1 Port Ballast Tank, temporary repairs made between frames 22 to 28 at side shell, and in way of second longitudinal frame from tank top. Temporary repairs included welding two doubler plates on 2nd longitudinal frame; one doubler 3/8 inch steel plate, apx 8 inch by 12 inch added iwo frames 23-24. Second doubler 3/8 inch steel plate, apx 15 inch by 15 inches added iwo frame 26-27. Additional weldment added in way of observed fractures and brackets. 2nd longitudinal frame from tank top, at frame 23 appears excessively distorted. Recommend with the #1 Port Ballast port side shell and frames between frame 22 (Bow Thruster Tunnel) to frame 28 (Swash or intermediate transvers bulkhead) be closely examined when making permanent repairs. Permanent repairs required to remove doublers installed and to renew any or all excessively distorted or fractured frames between frames 22 & 28 on or before March 31, 2020. IAW 46 CFR 92.01-10

Make permanent repairs to the #6 to #7 Port & Stbd Ballast Tanks Transverse Water Tight Bulkheads, FR 176, to cut, crop out and renew steel in way of temporary weld repairs. Temporary weldments apx 12 inches to 18 inches long were made to WTB's, with temporary weldments made on each side of the bulkheads, all located approximately 5 ft above the bottom shell and apx 8ft inboard of the side shell. Permanent repairs shall be completed on or before March 31, 2021, and shall be repaired to the satisfaction of the attending CG Marine Inspector.

Vessel reported fathometer inoperable. Sector SSM issued a Letter of Deviation (LOD) to allow the vessel to transit to Detroit then conduct repair. Sector SSM received a request for a second LOD since the fathometer was unable to be repaired within the time period of the last LOD. LOD denied, make permanent proper repairs to the fathometer to the satisfaction of the attending marine inspector within a week. Found the starboard side channel at frame #21 to have an inset, in addition the six rivets that were caulked were loose with a minimal amount of water leaking and the three rivets below those were considerably loose and required repairs. Also found the #2 stringer rivets were loose (those rivets were not attached to the hull). Requsested a repair proposal be provided to CGMI for permanent repairs.
