A hydraulic hose on the stern ramp hydraulic line system ruptured upon lowering of the ramp, preventing it from further operation. 74 SOLAS 1/11(a)

Liferaft painter system shall provide a connection between the ship and liferaft and shall be so arranged that when released, and in the case of an inflatable liferaft, will inflate and not be dragged under by the sinking ship. PSCO observed both liferaft's painter lines were hooked up incorrectly to the hydrostatic release. SOLAS 74 (1997 Cons.) III/38.6.1 10c
