A vessel must be operated in accordance with applicable laws and regulations in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons to be transported. The below deficiencies provide objective evidence that the vessel is substandard and not proactively managing technical and operational requirements. Vessel must make correction to the below deficiencies and get underway with marine inspectors to perform satisfactory operational test and drills prior to any movement.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. The following non-conformities were identified: - Port engine has a fuel oil leak on injector tubing - Starboard engine has fuel oil leak on the fuel pump - Fuel shut off valve at fuel tank leaking - Port Racor fuel filter leaking fuel - Linear actuator for steering system leaking oil - Belt on starboard propulsion engine loose

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Multipule electrical wires on aft poop deck display mast are burnt through plastic casing exposing interior.

At each inspection for certification and at every other vessel inspection, quarters, toilet and washing spaces, galleys, serving pantries, lockers, and similar spaces may be examined to determine that they are serviceable and in a sanitary condition. There is raw sewage inside of a plastic pipe which is open on both sides of the pipe that is on the top of one of the sewage holding tanks in the sewage holding compartment directly under an accommidation space, making the space not suitable for human occupancy.

The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard. Not all crew members of this vessel are participating in drills.

Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 . Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Monthly inspection of portable fire extinguisher are not being conducted.

If corrective lenses are required in order to meet the vision standards above, a mariner may not serve under the authority of the endorsement unless corrective lenses are worn and spare lenses are carried onboard a vessel. This operational limitation will be placed on their medical certificate. Captain did not have a spare set of glasses onboard vessel. 46 CFR 10.305(d)

Each individual referred to in paragraph (a) of this section must hold an MMC that serves as identification, with an appropriate endorsement for the position in which the seaman serves, and the MMC, along with a valid medical certificate, must be presented to the master of the vessel at the time of employment or before signing Articles of Agreement. Fill pipes and sounding pipes must run as directly as possible, preferably in a straight line, from the deck connection to the top of the tank. Such pipes must terminate on the weather deck and must be suitably marked for identification. Fuel fill is not labeled "DIESEL".

Each lifejacket must be approved under approval series in subparts 160.002, 160.005, 160.055, 160.115, 160.176, or 160.255 in part 160 of this chapter, or other standard specified by the Commandant. 19 life jackets were found to be damaged and no longer servicable.

A vessel must be provided with a main steering gear that is: Of adequate strength and capable of steering the vessel at all service speeds. The port side connection rod between the the rudder shaft and the crossbar is missing the pin to keep the connection rod in place.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. The following non-conformities were identified: - Life jacket storage on poop deck is missing labels - Life jacket storage in aft passenger compartment has worn and missing labeling

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Waste receptacles are constructed of combustible materials.

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. The remote fuel shutoff station is not labeled.

On a vessel on which the number of passengers permitted on the upper. decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximin number of passengers allowed on the upper ·decks must be indicated by a durable marking of numbers and letters at least one inch in height at the entranceway to each such deck. The max persons allowed on the aft poop deck is not posted. 46 CFR 185-3 (e)

Mechanically propelled ''S" vessels which are ferry vessels, or which carry more than 49 passengers and mechanically propelled "L” vessels shall comply, with the provisions of' Subchapter F of this chapter. (a) Plastic pipe installations must be in accordance with IMO Resolution A.753(18) and IMO Resolution MSC.313(88) (both incorporated by reference, see § 56.01-2) and the following supplemental requirements. (ii) Where fire endurance is required in Appendix 4 of IMO Resolution A.753(18) the pipe must, at a minimum, be approved as meeting the fire endurance level required in Appendix 4. Ratings of “0” in Appendix 4 indicate that no fire endurance test is required. Ratings of “N/A” or “X” indicate that plastic pipe is not permitted. The bilge suction grates are made of a plastic material in the fuel tank and sewage holding tank compartments. 46 CFR 182.40-5 (b), 46 CFR 56.60-25 (a)(ii)

All passenger accommodations must be arranged and equipped to provide for the safety of the passengers in consideration of the route, modes of operation, and speed of the vessel. The glass windows in the aft concessions area are broken with sharp edges exposed.

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. The ring life buoy located near the operating station does not have Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant on it.

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Many life jackets found with name worn off. CORRECTED ON THE SPOT

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel's drydock and ISE expired 1/31/2025.

Centerline generator is inop due to crew conducting a full rebuild/overhaul. Conduct a engine test after to ensure is fully operational.

During COI renewal, MIs found excessive corrosion in all compartments. Excessive corrosion was found under port side engine where the corrosion looks wet. Vessel is required to go to drydock as soon as possible.

ALL CABLES AND WIRES MUST: BE INSTALLED WITH METAL SUPPORTERS SPACED NOT MORE THAN 24 INCHES APART AND IN SUCH A MANNER TO AVOID CHAFING ANF OTHER DAMAGE. THE USE OF PLASTIV TIE WRAPS MUST BE LIMITED TO BUNDLING OR RETENTION OF MULTIPLE CABLE INSTALLATIONS AND NOT USED AS MEANS OF SUPPORT; VESSEL HAS AN EXCESSIVE AMOUNT OF ZIP TIES IN MEANS OF SUPPORT OF CABLES.
