No valid COI issued to vessel for oustanding no sail deficiencies from previous inspections. A vessel may not be operated without having on board a valid US Coast Guard Certificate of Inspection.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Owner/Operator is required to conduct Ultra Sonic thickness guaging test on the steel plates listed: forepeak (port and stbd),steel plate amidships, below/above the chime (port and stbd). Provide guaging report to USCG.

During the examination all accessible parts of the vessel's underwater body and all through hull fittings, including the hull plating, appendages, propellers, shafts, bearings, rudders, sea chests, sea valves, and sea strainers shall be made available for examination. Sea chests, sea valves, and sea strainers must be opened for examination. Sea strainer attached to main engine sea chest is worn and needs replacing.

Penetrations and openings in the side shell must be made watertight. Make permanently watertight, the two through hull pipes protruding into the engine room, starboard side near gen exhaust.

Penetrations and openings in watertight bulkheads must- (1)Be kept as high and as far inboard as practicable;and have means to permantly make watertight Make watertight the the electrical wire openings in the fore and aft bulkheads of the engine room.

Where fuel level gauges are used on a metal tank, the flanges to which gauge fittings are attached must be welded or brazed to the tank. Tubular gauge glasses, if fitted to diesel fuel tanks, must be of heat resistant materials, adequately protected from mechanical damage, and provided at the tank connections with devices that will automatically close in the event of rupture of the gauge or gauge lines. Diesel fuel tank was equipped with a plastic sight gauge, replace with a gauge that meets the appropriate heat resistant standards.

Wiring joints and splices shall be mechanically secured and made in junction boxes. Properly secure all joints and splices throughout the vessel and terminate in an appropriate enclosure. 46CFR 183.05-50(b) (<50v) 46CFR 183.10-10 (>50v)

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. (a) Steel hull vessels: (1) Rules and Regulations for the Classification of Yachts and Small Craft, Lloyd's Register of Shipping (Lloyd's); or (2) Rules for Building and Classing Steel Vessels Under 61 Meters (200 Feet) in Length, American Bureau of Shipping (ABS) Vessel owner must submit a repair proposal to address the wasted steel found throughout the hull, and complete repairs upon plan approval from USCG.

Remove carpet immediately adjacent to the bar service space.

All cables installed after 10MAR1996 must meet section 310-13 of the NEC and be listed as UL marine/boat cable. Replace all flexible extension cords used throughout the vessel. 46CFR 120.340 46CFR 183.10-20(a)(2)

Motor current protection set not more than 125% of the motor full load rating or in accordance with Table 183.10-40(a) must be provided. The over current protection for the fire/bilge pump, steering pump, walk in cooler, fresh water pump, and cooler H2O pump were found oversized. Provide an over current device for each motor or the respective data sheets to verify the motors are equipped with integral protection. 46CFR 183.05-50 (<50) 46CFR 183.10-35 (>50)

Proved an electrical one line diagram of the power system and a load analysis to document the new/modified arrangement found, submit to the Marine Safety Center for approval.

Provide fire load calculations with enough detail to verify all new (after 10MAR96) alterations to the vessel's outfitting and finishings. 46CFR 116.427 46CFR 177.10-5 46CFR 116.115

Unless deemed a major conversion in accordance with 46 CFR 116.115, alterations to the vessel’s structure or arrangement are grandfathered to the regulations in effect at the time of construction (31JUL1945). However, non-structural modifications such as décor and furnishings must comply with the regulations in effect at the time the modification/s were carried out. A review of the vessel’s documentation history indicated that the vessel originally operated as a passenger ferry from 1945 to the early 1970’s on the Great Lakes under the vessel name CUMMUTER. In the late 70’s the vessel changed its service from ferry service to excursion service and was rechristened THE ISLANDER. Non-structural modifications during the change-in-service are subject to 46 CFR 177 (Old-T, 1JUN1958 to 10MAR1996). 46 CFR 177.10-5 requires fire hazards to be minimized insofar as reasonable and practical. In addition, it incorporates fire protection standards of 46 CFR, subpart 72.05 for vessel’s certificated for the carriage of more than 150 passengers on or after 1JUL1961. 72.05-15 “Ceilings, Linings, trim, and decorations in accommodation spaces” requires any ceiling linings and furring to be of an approved incombustible material. On April 3, 2020, Jensen Engineering provided the Islander Fire Calculation (Rev.3) which indicated fire loads in the accommodation area to be 14.69 lbs/sqft (main deck) and 10.17 lbs/sqft (upper deck). Theses fire loads present an undue fire hazard, provide a mitigation plan to minimize fire hazards insofar as reasonable and practical on board the vessel. Insofar as reasonable and practical, is considered to be a fire load less than 7.5 lbs/sqft in accordance with 46 CFR 116.427 or the total volume of non-structural material should not exceed a volume equivalent to 1/10 inch veneer of the combined area of the compartment in accordance with 46 CFR 72.05-15.

Vessel of at least five net tons engages in coastwise trade must have a COD. Original certificate of documentation was not onboard.

Vessel required to have a waste management plan onboard. Waste management plan was not onboard.

Operator shall instruct crew member at least once every three months as to their expected duties. Crew training shall be logged. Last fire drill was July 2018 and there was no record of an abandonship drill IAW 46 CFR 122.420 & 122.420 (k).

Self-propelled vessel that is certificated to carry more than 150 passengers required to have Class A, AIS device. Vessel does not have AIS IAW 33 CFR 164.46(b)(i)(iii).

Durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information of the vessel, placard not posted IAW 46 CFR 121.506.

Vessel must carry 160.041 or equivalent approved first aid kits. Vessel does not have complete first aid kit IAW 46 CFR 121.70.

Battery must be located as high above the bilge secured to protect against shifting from vessel movement. Connection must be made to battery terminals with permanent type connectors. Battery was not secured and on the floor of machinery deck battery cables was secured by wing nuts.

Penetrations and opening must have means to be kept watertight. Gasket appears to be unserviceable to keep watertight integrity (watertight door).

Vessel must be fitted with ground tackle and mooring lines necessary for the vessel to be safely anchored or moored. Mooring lines significantly frayed; inadequate mooring arrangement at pier.

Owner or managing operator shall prepare evacuation plan, fire control plan must be posted on the vessel that is visible to all passengers IAW 46 CFR 116.520 & 46 CFR 116.530.

Company required USCG annual DAPI audit. Vessel has not met requirement IAW 46 CFR 16.

Vessel missed its window for completion of annual inspection (46 CFR 115.500(b)(1)). Vessel may not operate prior to completing a new COI inspection.

Provide proof of approved alcohol measuring device or alcohol testing facility availability

Provide year 2016 random testing results

Provide proof of enrollment for entire crew
