Bulkheads or decks of "A" Class shall be composed of steel or equivalent metal construction...and shall be so insulated with approved structural insulation, bulkheads panels, or deck covering that the average temperature on the unexposed side would not rise more than 250 degrees Fahrenheit above the original temperature. Multiple pieces of insulation supporting the vessel's structural fire protection, as discussed with vessel Port Engineer, was missing in the main engine room and generator room. The deficiency shall be rectified prior to the vessel's movement. 46CFR71.05-10(c)(1)

The hull, structural bulkheads, decks, and deckhouses shall be constructed of steel or other equivalent metal construction of appropriate scantlings. The vessel engine room aft bulkheads contained multiple holes inhibiting the integrity of the CO2 protected space. Deficiency shall be rectified prior to vessel's movement. 46CFR72.05-10

Doors in bulkheads required to be Class A-60, A-30, or A-15 shall be of hollow steel or equivalent metal construction solidly filled with approved structural insulation capable of meeting the requirements for a Class A-15 bulkhead. The main engine room port side door was found incapable of closing properly. Rectify deficiency prior to vessel's movement. 46 CFR 72.05-25(b)(1)

The installation of all systems of electrical engineering...shall be in accordance with the requirements of subchapter J of this chapter. The vessel's anchor and starboard navigation light was found inoperable. The vessel is limited to day time operations until the deficiency is rectified. 46CFR77.05(a) 46CFR111.75-17

The installation of all systems of a marine engineering nature, together with the details of design...shall be in accordance with the requirements of subchapter F. The material, design, construction, workmanship, and arrangement of main propulsion machinery...must meet at least equivalent to the standards established by the ABS Steel Vessel Rules, except as otherwise provided by this subchapter. The vessel's #2 center MDE's RPM gauge was inoperable. Rectify deficiency prior to vessel's departure. 46CFR77.03(a) 46CFR58.05-1(a)

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. 22 adult life jackets and multiple children life jackets were found unserviceable. These life jackets were destroyed in the presence of the attending marine inspector. The vessel shall inspect all life jackets on board prior to carrying passengers. 46 CFR 77.06-1 46 CFR 199.190(a)

Valves installed on the outside of the oil tanks must be arranged with a means of manual control locally at the valve and remotely from a readily accessible and safe location outside of the compartment in which the valves are located. The fuel oil supply ball valve from the EGen day tank was found to be spinning freely. 46 CFR 77.03-1(a) 46 CFR 56.50-60(d)(1)

Doors, other than those which are normally locked, such as from staterooms, fan rooms, lockers, etc., shall be of the self-closing type capable of closing against a 3 1/2 degree list. The port side galley entrance door was held back with a foot stop. Remove foot stop or make this door capable of being released from the control station. 46 CFR 72.05-25(a)(9)(i)

Each vessel must have a amplifier-type announcing system. The PA system was inoperable in room 131 and in the laundry. 46 CFR 77.05-1(a) 46 CFR 113.50-5(a)

The chemical storage door located in the galley was found incapable of closing properly. 46 CFR 72.05-25(b)(1)

Electric installations on vessels must ensure: maintenance of system integrity through compliance with the applicable system requirements to which plan review has been approved. Electrical issues noted: -The "E-2" panel located in the EGen space had a temporary 220V wire and portable plug installed directly to the breaker panel. -There was a missing plug found in the battery charger for the EGen starting batteries. -Electrical drop cables to the working stations inside the galley were not properly supported by brackets. -Breaker panel S-1 had missing breaker covers. -Cables entrance on back of main switchboard was not drip proof. 46 CFR 77.05-1(a) 46 CFR 111.01-1(a)(3)

The Marine Inspector (MI) observed the red flashing light did not activate in dead ship condition upon sounding the general alarm in spaces with high levels of noise. 46 CFR 77.05-1(a)

The MI observed no functioning emergency egress lightening located in machinery spaces excluding the emergency generator room during dead ship condition. 46 CFR 77.05-1(a)

Currently there are 4 exit signs are not working properly. 46CFR78.47-40

The vessel crew was unable to start the starboard side rescue boat engine and the davit lubrication points were not be greased. Furthermore, the bow section of the boat was deflated. 46 CFR 70.28-1 46 CFR 199.190 (a)

Each lifeboat engine and rescue boat engine must be run ahead and astern for a total of not less than 3 minutes unless the ambient temperature is below the minimum temperature required for starting the engine. Vessel crew has not been performing the required weekly inspection and tests of the port or starboard rescue boat. Company must provide procedures and ship's crew properly trained on required tests. 46 CFR 70.28-1 46 CFR 199.190 (d)(2)

Launching appliances must be thoroughly examined at intervals not exceeding 5 years and, upon completion of the examination, the launching appliance must be subjected to a dynamic test of the winch brake. The port and starboard rescue boat davits are outside the five year window for the following: replacement of falls, dynamic test of the winch brake, and release gear. 46 CFR 70.28-1 46 CFR 199.190 (i)&(j)

Each vessel must have posters or signs displayed in the vicinity of each survival craft and the survival craft's launching controls. Neither the port or starboard rescue boat had the required operating instructions posted in the vicinity of the boats. All crew must be properly trained. 46 CFR 70.28-1 46 CFR 199.90

Each liferaft or group of liferafts must be arranged for float-free launching. The inflatable liferafts were fitted with proper hydrostatic release units, however, the each painter line was attached directly to the mounting bracket vice the weak link. Rectified on the spot. 46 CFR 70.28-1 46 CFR 199.130(c)(7)

The MI observed the caller's voice could not be heard when making a call using the sound powered phone located in the emergency generator room. Rectified during the inspection. 46 CFR 77.05-1(a)

Nothing in this subpart shall be construed as limiting the inspector from making such tests or inspections as he deems necessary to be assured of the safety and seaworthiness of the vessel. The drills performed for the issuance of the COI were performed by the minimal crew sufficient for transit without passengers. The vessel is limited to operations without passengers until drills are witnessed by the USCG that include the full crew. Schedule a Deficiency Check to clear deficiency prior to taking on passengers. 46 CFR 71.25-50

Machinery must be installed and maintained IAW Subchapter F. Starboard Z-Drive inopperable. Make repairs to satisfaction of attending MI.

The installation of all systems of a marine engineering nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of sub chapter F. Main propulsion machinery must be maintained to manufacturing standards, and provide safe operation. STBD Z-Drive is out of service due to missing Z-Drive lower half, (Prop, Hub, and Tube Assy). Company ETR is unknown. Parts may need re manufactured, machined. Vessel will make repairs as needed along with testing to satisfaction of local CG marine inspectors. Activity 7447163 issued 835v has been cleared, issued by Sector Lower Mississippi.

The installation of all systems of a marine engineering nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter F. Main propulsion machinery must be maintained to manufacturing standards, and operate safely. Chief Engineer noticed problems with the center Z-Drive and shut down for further inspection. Vessel will make repairs as needed to satisfaction of local marine inspectors.

The installation of all systems of a marine engineering nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of sub-chapter F (Marine Engineering) of this chapter. Systems of this type include the following: Steering Systems. On 09MAY2022, the Master of the subject vessel notified Sector Lower Mississippi River that at approximately MM 821, while the vessel's engineers were conducting maintenance on the vessel's STBD Z-Drive unit, it was observed that the entire lower portion of the unit was missing and presumed to have fallen into the river. The vessel shall continue their current voyage to Columbus, KY and work directly with USCG MSU Paducah Marine Inspectors to repair/replace the affected Z-Drive prior to departing Columbus, KY.

All marine engineering details installed on vessel shall be designed, constructed and installed IAW Subchapter F. The vessel was pulling into Natchez, MS when they felt vibrations in the steering and found damage to the port wheel and steering gear.

Every crew member must participate in at least one abandon ship drill and fire drill every month. Drills must be held before sailing when a vessel enters serviced for the first time ,after modifications of a major character, or when a new crew is engaged. This vessel has been in laid up service for over 6 months and several of the crew members are new to this vessel. Drills shall be completed by [name withheld] prior to vessel carrying passengers for hire.

The VSO must ensure that at least one security drill is conducted at least every 3 months, except when a vessel is out of service due to repairs or seasonal suspension of operation provided that in such cases a drill must be conducted within one weeks of the vessel's reactivation. Vessel has been laid up for over 6 months and several crew members are new to this vessel. A security drill shall be completed by [name withheld] prior to vessel carrying passengers for hire.

Each passenger vessel must have a public address system capable of broadcasting separately or collectively to the following station: Public spaces used for passenger assembly points. Vessel's sundeck is open to passengers at anytime. During the testing of the PA/GA, inspectors had a hard time hearing the alarms. Vessel needs to install speakers on the sundeck for passengers to hear the emergency broadcasting and alarms.

In a space described in § 113.25-9(a), where the general emergency alarm signal cannot be heard over the background noise, there must be a red-flashing light or rotating beacon, in addition to the general emergency alarm signal. Vessel's rotating beacon with light was inoperable in the stbd generator room and in the emergency generator space.

Where extinguishing systems or equipment are not required, but are installed, the system or equipment and its installation must meet the requirements of this part. Vessel has an ANSUL fire extinguishing system installed in the main galley. System was unable to be tested due to equipment not wired up.

Vsl suffered partial loss of propulsion, STBD z-drive lower mechanical seal failed discharging unit's hydraulic fluid, rendering z-drive in-operable. Vsl authorized to transit w/ passengers to Louisville, KY on two engines to affect permanent repairs. Upon Completion of repairs vsl shall submit technicians report of damage and repairs conducted, prior to departure from Louisville.

The Vessel Response Plan (VRP) must cover all geographic areas of the United States in which the vessel intends to handle, store or transport oil, including port areas and offshore transit areas. The vessels VRP does not have the Sector Lower Mississippi River COTP zone listed on its current VRP.

A temp repair was noted on the centerline M/E jacket water line. Make permanent repairs within 5 days (29 May 2021) to the satisfaction of an attending marine inspector.

This deficiency superceeds the previous 835 issued on 3/12/18. Provide Marine Safety Center approvals for updated vital systems automation, Qualitative Failure Analysis, Design Verification Test Procedures, and Periodic Safety Test Procedures for the vessels vital systems. Once Marine Safety Center letters with comments are issued, they must be answered within 30 days. This 835 will then be extended for another 30 days if needed IAW Chief of Domestics Vessels Sector New Orleans. Sector New Orleans to be contacted when letters recieved and responded to. [name withheld] [phone withheld]. [name withheld] [phone withheld]. 46 CFR 61.40

Prior to getting the getting the vessel u/w provide written report that makes a determination to why the sprinkler pump started by itself and went on running w/o anyone knowing. Pump overheated and testing completed prior to placing sprinkler sys. back in service. 46 CFR 76.28-7, 46 CFR 76.25-9

Stairtower FWD STBD side outside galley being used as flammable + combustible sorage. Remove all this storage. Improper for this safe refuge tower. 46 CFR 72.03-5(a)

Numerous fire screen doors in the diff galleys are being blocked open. This breaks the structural fire boundry being held open. Close these doors provide crew training on fire boundry. 46 CFR 72.05-25(b)(9)(ii)

Located i nthe theater port side aft. Escape hatch blocked by furniture. Move furniture unblock egress escape hatch. 46 CFR 72.10-5

A60 insulation is missing in sections i nthe compartment next to fire station 32. Replace missing sections. 46 CFR 72.05

Located in the overhead crew qtrs by fire station 34 there is plastic sheeting covering the thermal insulation. This sheeting is improper and must be removed. 46 CFR 72.05-40

During the lifejacket inspection 40 lifejackets were not serviceable. Replace these jackets. 46 CFR 70.28

Throughout the vessel there are many portable heaters that do not meet the CFR requirements. Remove all of these space heaters. 46 CFR 111.87-3

The stbd fwd elevator recall to a deck other than the deck of the fire. Once recalled to a safe deck the elevator would not open. 46 CFR 76.27-10(h)

Vessel struck an unknown object causing STBD z drive to become inoperable. Vessel shall proceed to Paducah, KY to embark technicians to survey and repair inoperable z drive prior to vessel getting back underway.

Annual servicing for fire doors required to meet manufacturer's specific instructions. Doors (rolling) have gaps in bottom and no servicing records available. 46 CFR 72.05-25(b)(9)(ii)

Nnumber 3 generator malfunctioned, determined diode on the exciter broke free causing generator malfunction, new part was ordered. Awaiting installation.

Liferafts with hydrostatic releases must be operational. Box beam to which hydrostatic release was attached had wasted/cracked welds.

Indicator light for watertight door required. Class I watertight door did not have functioning indicator light.

Flexible loop and hoses for CO2 system by liquor room and by entrance to emergency generator room found loose.

Properly update circuit directory of breaker panels. Rectify pior to next quarterly. Found breaker panels in coffee room, mezzanine, officer's stateroom and generator room with improper/difficult to read circuit directories.

Painter lines to release liferafts tied to deck. Required to be attached to weak link.

Hold back device on z-drive room's self closing door found.

Evaluate and exact repairs to the port # 1 generator, conduct sat operational test to the satisfaction of an attending CG Marine Inspector.

Evaluate and exact repairs to the port # 2 generator, conduct sat operational test to the satisfaction of an attending CG Marine Inspector.

Evaluate and exact repairs to the port # 3 generator, conduct sat operational test to the satisfaction of an attending CG Marine Inspector. Cleared 17SEPT18 CWOD

All pins/bolts used to secure steering gear on port z-drive sheered. Replace all with OEM parts and have operational test conducted to satisfaction of CG inspector.

Repair/replace crankshaft on starboard CAT 3508 prime mover. Do not operate starboard Z-drive prior to conducting an operation test with a CG Inspector in attendance.

Provide Marine Safety Center approvals for updated vital system automation: Qualitative Failure Analysis, Design Verification Test Procedures, and Periodic Safety Test Procedures for the vessel's vital systems.

Generator Failed

Upon arrival at the first destination of Nottaway Plantation the vessel along with the contractor must submit a proposal to USCG Sector New Orleans to make temp repairs to the fractured paddle wheel hub. A permanent repair proposal must also be included in the temp repair proposal.

1. Temp repairs have been made to the paddle wheel. These repairs are to be monitored by the ships crew every watch and logged. Any future fractures must be immediately reported to SECTOR New Orleans USCG and the local OCMI, the terminate the use, and free wheel the paddle wheel.

2. A permanent repair to the paddle wheel fracture must be completed during the vessel's layup dockside schedule the beginning of 2018.

Do not use the paddle wheel for propulsion allow wheel to free wheel.

Vessel is to remain at Nottaway plantation until repairs and approval of a plan for repairs is completed.

GQ alarm contactor was found INOP in Engine Room. Repair required Immediately.

Install new hand rails on the port and stbd stair towers aft. They have recently arrived on board. Extra manning in these toweres until rails installed.

Pilot House hyd ram system not lowering the pilot house correctly, Make repairs prior to lowering the pilot house.

Lincoln Library stairs, install hand rails on blkhd side of stairs. Provide additional manning of these shorts stairs until rails installed.

Provide updated automation PTSP, FEMA and DVTP approved by MSC. Currently all automation tested satisfactory changes made to procedures and procedures were sent to MSC for approval. On 14Sep2017 the vessel requested extension of the 835. Extended until 14Oct2017. 29Sep2017, extended until 14Nov2017.

Provide updated Fire Control plan for approval by local OCMI at SECTOR New Orleans. On 14Sep2017 the vessel requested extension of the 835. Extended until 14Oct2017. 29Sep2017 extended until 14Nov2017.

Review all MSC comment letters, resolve all MSC comments and ensure systems match approved revisions of the drawing.

Complete the installation of structural fire protection not in way of wall coverings into include draft stops as per approved drawings.

Complete the required stability tests and receive a valid stability letter.

Complete installation and testing of installed fire detection system.

Hydrotest is completed and satisfactory, complete testing and installation of the automatic sprinkling system as per approved drawings and per nfpa 13(1996)

Complete installation and testing of emergency lighting.

Complete installation and testing of the general alarm system.

Complete installation and testing of the inernal communication system.

Complete installation and testing of the public address system.

Complete the installation and testing of the fixed CO2 system in the liquor room.

Complete installation and testing of the hydraulic systems for 3rd deck, fwd capstan, and paddle wheel system.

Recieve approved automation test procedures and complete design verification test procedures.

Complete installation and inspection of crew and passenger accomodations.

Complete installation and testing of approved rescue boat davits

Complete installation and inspection of all required placards and labeling.

Demonstrate "means of escape" from the port side of the MSD space.

Demonstrate that the vessel has sufficient fire station hydrants to provide two streams of water with out violating protected bulkheads to each space.

Vessel has made a major modification as per MSC letter and is required to have a tonnage and dimension remeasurement.

Provide updated COD with new tonnage IAW new tonnage cert. On 14Sep2017 the vessel requested extension of the 835. Extended until 14Oct2017. On 29Sep2017 extended until 14Nov2017.
