The arrangements for storage distribution and utilization of other flammable oils employed under pressure in power transmission systems, control and actuating systems, and heating systems shall be such to ensure the safety of the ship and persons on board. PSCO observed several breakers on the emrgency switchboard not labeled and in the "half" position (tripped) and not able to be put in the off position. SOLSAS(04)II-1/40.2

Unless overpressure is prevented by the pump characteristics, means shall be provided which will prevent over pressure in any part of the system. PSCO observed both feedwater pump seals leaking and the hotwell in extremly poor condition, missing insulation and protective coverings. SOLAS(04)II-1/32.4

The machinery, boilers, and other pressure vessels, associated pipingsystems, and fittings shall be of a design and construction adequate for the services for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces, and other hazards. All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel or other equivalant material permanently and strongly attached to the bulkhead, and framed, stiffened and fitted so that the whole structure is of equivalent strength to the unpierced bulkhead and weathertight when closed. Both PORT and STBD weathertight doors to underdeck passage severly corroded and wasted with poor gaskets. ILLC reg 12.1

Every ship of 100GT and above, and every ship which is certified to carry 15 or more persons, and fixed or floating platforms shall carry a garbage management plan which the crew shall follow. Reference DNV survey statement with Job ID 1580017 in MISLE to verify repairs are made at first US port after sailing foreign and that ship is reporting non-conformities/casualties IAW vessel SMS as required in ISM.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with additional requirements which may be established by the company. Due to the objective evidence in the deficiencies noted below, the vessel is not being maintained and not in substantial compliance with relevant conventions, calling into question the adequacy and implementation of the vessel’s SMS under the ISM code. An external audit conducted by the Flag or RO is required before the ship will be released from its detention to determine whether the ship is operating in accordance with the ISM Code. Provide a copy of the completed audit report to USCG prior to departure from port.

The safety management system should include procedures ensuring that non-conformities, accidents and hazardous conditions are reported to the company, investigated and analyzed with the objective of improving safety and pollution prevention. PSCO review of ship records found no defect or internal NCR's have been processed by crew in accordance with vessels safety management system, III.2.9.7.

The panels were all open exposing high voltage(440V)electrical components causing a significant potential shock hazard.

PSCO observed used gear oil being collected and stored in open 55-gallon drums in the forward bos'n stores, which presents a fire hazard to the ship and its crew.

Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergency source of electrical power. PSCO observed emergency lighting fixture supports for the forward life raft is severely corroded and is only supported by its electrical supply cable. This condition is causing the light fixture to only work intermittently.

Every ship shall carry a garbage management plan which the crew shall follow. PSCO observed trash stored in webbing of the ship's hull outside of the BOSN stores on the port side which is not in accordance with the vessels garbage management plan.

Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergency source of electrical power. PSCO observed the electrical plug for the disembarkation portable light for 6 person life raft at bow of vessel is unable to stay seated in designated socket for proper operation and the light fixture is unable to be fixed to the point of disembarkation as per design.

PSCO observed one of the cargo hatch drainage pipes on the port side midsection is severely deteriorated and has lost its intended weather-tight integrity. Several of the rubber drainage pipe covers throughout the cargo hatch drainage pipes have also deteriorated losing its intended weather-tight integrity for the No. 3 cargo hold.

The fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: fire detection and fire alarm systems. PSCO observed the smoke alarm in the welding workshop in the engine room failed to produce an alarm during testing by ship’s crew.

After any survey of the ship under the present regulations has been completed, no change shall be made in the structural arrangement, machinery, equipment and other items covered by the survey, without the sanction of the Administration. The PSCO observed that the paint locker fixed ventilation system has been retrofitted with a flexible non-metallic hose that has not been approved by its flag Administration.

07199 - Other (fire safety) - The condition of the ship and its equipment shall be maintained to conform with the provision of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed that the condition of the flammable liquids locker was in complete disarray and there are numerous open containers causing a significant buildup of flammable vapours. 74 SOLAS (14) I/11.a
