Conductors must be protected in accordance with their current carrying capacities. If the allowable current carrying capacity does not correspond to a standard device size, the next larger over current device may be used provided it does not exceed 150 percent of the conductor current carrying capacity. 14 AWG wire for the machinery space lighting was installed on a 20-amp breaker. Remove 20-amp breaker and install a UL 489 approved 15-amp breaker for proper over current protection of the 14 awg wire. Lighting circuit shall not be energized until this has been rectified.

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Bilge motor to pump coupling in machinery space is missing guard. Install guard to satisfaction of attending marine inspector.

Abandon ship drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information:(1) Date of the drill and training; and(2) General description of the drill scenario.Missing abandon ship training logs. Provide to the satisfaction of the attending marine inspector.

Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Provide monthly inspection log to the attending marine inspector.

The Certificate of Inspection and any SOLAS Certificates must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. COI was not the most current version with updated hull inspection dates. Print updated amended COI and post.

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. Vessel's dry dock examination date was overdue on 01JUNE2025. Vessel is not to carry passengers until a satisfactory dry dock examination is completed.

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces:(1) A space with a through-hull fitting below the deepest load waterline, such as a lazarette. Bilge alarm actuation did not activate audible or visual alarm at the operating station. Repair bilge alarm to the satisfaction of the attending marine inspector.

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. Inflatable life raft hydrostatic release unit expired Jan 2024. Replace life raft hydrostatic release unit to the satisfaction of the attending marine inspector.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Port side Shaft Space 10th side frame aft from fwd bulkhead side frame was split and fell apart upon inspection. Provide repair proposal to the attending marine inspector.

Each EPIRB, other than an EPIRB in an inflatable liferaft, must be tested monthly, using the integrated test circuit and output indicator, to determine that it is operative;The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 (incorporated by reference, see § 175.600 of this chapter) as amended here: Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. EPRIB and Portable Fire Extinguisher monthly inspection records were not available for inspection. Provide monthly inspection logs to the attending marine inspector.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificate to be issued by the Federal Communications Commission, as set forth in 47 CFR 80. Vessel master did not have Marine Radio Operator Permit or License. Provide valid Marine Radio Operator Permit or License to the attending marine inspector.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel did not have a FCC Safety Radiotelephony Certificate onboard. Provide Safety Radiotelephony Certificate to the attending marine inspector.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment and systems, including the following: Inspection of each lifejacket, work vest, and marine buoyant device. (3) lifejackets were removed from service due to tearing of the jacket fabric. Vessel's life jacket count is now (48) and the vessel is restricted to (48) persons on board until they are replaced.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Identification of the items may be in words, or the appropriate symbols in IMO Resolution A.760(18). Letters and numbers must be at least 50 millimeters (2 inches) high. Symbols must be at least 100 mm (4 inches) square. Life jacket location markings were missing. Provide markings.

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage. The lighting fixture at the aft end of the shaft space was found missing a globe and cage, and with a broken bulb in it. Provide bulb and guards or remove fixture.

When, in accordance with § 178.210 of this chapter, a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. Stability letter was not posted. Post at the operating station.

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. Emergency broadcast placard was not posted. post emergency broadcast placard next to the radio installation.

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. Markings restricting passenger access to upper deck was missing. Provide markings communicating restriction of access to upper deck.

Fire Fighting, Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: (1) Date of the drill and training; and (2) General description of the drill scenario and training topics. Drills that were logged lacked type of drill and general description of drill topics. Conduct and log fire, abandon ship and man overboard drills to include type and description of drills conducted.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: (1) Charts of large enough scale to make safe navigation possible; (2) U.S. Coast Pilot or similar publication; (3) Coast Guard Light List; (4) Tide tables; and (5) Current tables, or a river current publication issued by the U.S. Army Corps of Engineers or a river authority. The vessel was unable to provide revision data for electronic charts and was unable to provide light list and coast pilot.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. On board first aid kit was not complete and contained expired items. Provide complete and current first aid kit.

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. The arrangement of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16) (incorporated by reference; see 46 CFR 175.600). Two child sized life jackets were found with degraded retroreflective material. Provide life jackets marked with correct amount of serviceable retroreflective material.

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operation. Shutoff valves missing direction of operation. Properly mark valves.

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The use of plastic tie wraps must be limited to bundling or retention of multiple cable installations, and not used as a means of support, except that on vessels of not more than 19.8 meters (65 feet) in length, installations in accordance with paragraph 14.h of ABYC E-8 and paragraph 15.h of ABYC E-9 (both incorporated by reference; see 46 CFR 175.600) are acceptable as meeting the requirements of this section. Power carrying conductor in engine space found hung with plastic wire ties. Provide appropriate hangers.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: (1) Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. Excessive rot was observed in the transverse frame in way of sonar dome mounting in the space aft of the forepeak. Remove rot and provide damage assessment and repair proposal.

Crew has not completed pre-employment drug testing. Complete testing prior to operating vessel.

Port running light inoperable. Demonstrate proper operation of light.
