Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space and may require the vessel to be dry docked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Items a) through c) shall be repaired prior to re-floating the vessel. Submit a repair proposal for Coast Guard Marine Inspector approval prior to commencing repairs. a) Observed deep cracks in fiberglass of the port quarter. After surface preparation, it appeared that the connection point of the port side shell and transom were significantly compromised. The damage has penetrated the watertight envelope approximately 2 feet above the waterline. b) After surface preparation of the starboard quarter, Coast Guard Marine Inspectors observed insufficient previous repairs to the fiberglass. c) De-lamination of fiberglass on transom, and both the port and starboard interior side shell of lazarette.

The owner or managing operator shall have the vessel ready for inspection of the hull structure and its appurtenances including all accessible parts of the of the exterior and interior of the hull, the watertight bulkheads, and weather decks. Items a) through c) shall be repaired prior to commencing commercial operations. Submit a repair proposal for Coast Guard Marine Inspector approval prior to commencing repairs. a) Observed daylight through 8-12 inch separation of hull-to-deck joint in port lazarette. b) Deterioration and gaps in decking forward of the operating station. c) Plywood deck completely wet underneath in vicinity of forward ventilation pipes creating significant hazard to persons in vicinity of the operating station.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances including all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. Items a) and b) require repair. Submit a proposal for Coast Guard Marine Inspector approval prior to commencing repairs. a) Most of the plywood decking in the forward compartment, engine room and lazarette is completely saturated with moisture and rotted. The deck is part of the weight bearing structure for vertical main deck supports in the forward compartment and for the fuel tank in the lazarette, thus creating a significant hazard to the vessel. b) Lazarette forward bulkhead shows significant signs of deterioration near shaft penetration.

A documented vessel must have on board the original certificate of documentation currently in effect for the vessel. No current COD was provided. Must provide.

The vessel must maintain the minimum number of portable fire extinguishers required. 40-B:C not in satisfactory condition. Replace fire extinguisher.

Every vessel must be fitted with a suitable magnetic compass with illumination. Compass would not illuminate. Provide illumination to compass. Vessel restricted to daylight operations only.

A vessel with installed toilet facilities must have a marine sanitation device that complies with 33 CFR part 159. MSD leaking into bilge. Submit a proposal for Coast Guard Marine Inspector approval prior to commencing repairs.

A vessel of more than 26 feet in length but not more than 65 feet in length must carry a minimum of one life buoy not less than 24 inches in diameter. Ring life buoy is unsatisfactory due to allowance of water intrusion. Repair/replace ring life buoy.

An adult life jacket must be provided for each person carried on board a vessel. In addition, a number of child size life jackets equal to at least 10% of the number of persons permitted on board must be provided. Eight (8) adult life jackets and three (3) child life jackets were found unsatisfactory. Total persons on board per the Certificate of Inspection is reduced to 26, with only one child allowed.

Each vessel must be equipped with at least 2 operable portable lights. One of these lights must be located at the operating station and at the machinery space access. No lights on board. Provide lights.

All machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. Throttle linkage found broken and improperly secured with wire. 46 CFR 182.01-5(a); Cite not in MISLE (old T)

All single screw vessels in ocean and coastwise service shall be provided with an auxiliary steering. A suitable hand tiller will be acceptable for this purpose. The handle is wasted on the auxiliary hand tiller. Vessel is not required hand tiller on a Lakes, Bays and Sounds Route but if the equipment is on board the vessel, it must be in a serviceable condition. All vessels shall be provided with suitable steering apparatus. Excessive leaking found in rudder post packing gland. Repair. 46 CFR 182.30-1(a). Old T cite not in MISLE

Nothing in this subpart limits the marine inspector from conducting such tests or inspections he or she deems necessary to be assured of the vessel’s seaworthiness. Marine Inspectors have observed an excessive amount of moisture throughout the vessel. A survey by a qualified naval architect shall be conducted in the presence of a Coast Guard Marine Inspector to address the watertight and structural integrity of the entire vessel. Provide a survey report prior to commencing commercial operations. 46 CFR 176.500(b)(iv); Cite not in MISLE

All electrical installations shall be to the satisfaction of the OCMI. The vessel needs to remove multiple electrical cables/wires and equipment that are no longer used. Old T: 46 CFR 183.01-15

EACH RING LIFE BUOY WITH A FLOATING WATERLIGHT MUST HAVE A LANYARD OF AT LEAST 3FT, BUT NOT MORE THAN 6FT, SECURING THE WATERLIGHT AROUND THE BODY OF THE RING LIFE BUOY.

EACH LIFE JACKET MUST BE MARKED WITH THE TYPE I RETROFLECTIVE MATERIAL. THERE WERE TWO LIFE JACKETS WITH EXCESSIVELY WORN RETROFLECTIVE MATERIAL. RENEW RETROFLECTIVE MATERIAL.

MEDICAL CERTIFICATES WILL BE ISSUED FOR A MAXIMUM PERIOD OF 5 YEARS. MASTER'S MEDICAL CERTIFICATE WAS EXPIRED. RENEW MEDICAL CERTIFICATE.

THROUGH HULL FITTINGS AND SHUTOFF VALVES SHALL BE ADEQUATE FOR ITS INTENDED SERVICE AND ENVIRONMENT. THE OVBD BILGE SHUTOFF VALVE LOCATED ON THE STBD SIDE OF THE FUEL TANK SPACE IS STUCK IN THE OPEN POSITION. DEMONSTATE VALVE IS CAPABLE OF BEING SHUT OFF. OLD T: 46 CFR 182.40-1(a)(4)

A GAUGE TO INDICATE LUBRICATING OIL PRESSURE SHALL BE PROVIDED FOR THE PROPULSION ENGINE. LUBE OIL GUAGE WAS INOPERABLE. REPAIR GUAGE. OLD T: 46 CFR 182.20-5(b)

EACH VESSEL MUST PERFORM AN INTACT STABILITY TEST IF THE [name withheld] STABILITY CHARACTERISTICS. THIS VESSEL MUST COMPLETE A SIMPLIFIED STABILITY TEST IN THE PRESSENCE OF THE [name withheld] 31DEC19.

Exhaust gas must not leak. Vessel had noticeable exhaust leak from exhaust manifold. Repair exhaust manifold.

Vessels must have navigation lights in compliance with Navigation Rules; stern light must show a 135 degree arc of visibility (Rule 21). Vessel had a stern light with six different LED's showing around 180 degrees arc of visibility. Replace with appropriate stern light.

The battery for each ring life buoy waterlight must be in compliance with sub-chapter Q. The battery was expired. Replace in kind.

Vessels must carry first-aid kits approved under 46 CFR 160.041 or equivalent. Vessels first-aid kit contained expired contents. Replace expired contents/first-aid kit.

Exposed hazards must be protected. Vessel had no guards on the main diesel engines moving parts. Install guards to prevent injury of crew.

Exhaust gas must not leak from piping. Vessel had noticeable exhaust leak from piping in engine room. Repair/re-wrap exhaust piping.

Nonmetallic piping must be adequate for its intended service and be approved by the [name withheld] hose section IWO water pump was deteriorated. Replace hose.

Nonmetallic piping must be protected and installed to the satisfaction of the [name withheld] for closed coolant system was supported with a wire. Provide adequate support that will not damage coolant hose.

Electrical equipment must be installed and maintained to minimize accidental contact. Multiple dead-end wired were found in forepeak, engine room, and behind bar on main deck. Remove/cap dead-end wires.

The master shall conduct and log drills for review by Coast Guard. Masters personal log did not include drills; vessel logs had last drill in August 2018. Maintain a logbook IAW applicable regulations.

The number and size of lifejackets shall be marked on each container for lifejackets. Child lifejacket stowage had no labeling. Label stowage area appropriately (2" lettering/numbering).

Stowage for life jackets must not be capable of being locked. Adult lifejacket stowage was capable of being locked. Remove lockable hasp/provide adequate means for stowage.

Prior to a voyage, the passenger count must be communicated verbally or in writing, and available ashore. Vessel was not providing a passenger count. Provide adequate means of relaying passenger count IAW regulations.

The Certificate of Inspection must be posted under glass/suitable material, conspicuously placed. Current posted COI was amended. Post amended/effective COI and remove old documentation from vessel.

The hailing port must be marked on a visible exterior part of the stern of the vessel. Vessel was newly painted, no hailing port was visible on stern. Add hailing port to stern (4" height of letters).

Each ring life buoy must be marked with the vessel's name. Ring life buoy did not have vessel name. Mark ring life buoy with vessel name.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed. Vessel experienced loss of lube oil on MDE and subsequent loss of propulsion on 05Jan2018. Demonstrate safe operation of MDE to the satisfaction of the attending Marine Inspector.

The OCMI may require an inspection and testing whenever a repair or alteration is made. After replacing deck in-king with marine grade plywood, make vessel available for inspection. New deck is to be hose tested for watertightness in the presence of attending marine inspector.

Reinstall intermediate courses in railing at embarkation station.

Reinstall permanent seating and make available for inspection.

Install suitable diesel fuel tank fill and vent pipes and make available for inspection.

Perform operational test of main engine and steering system while underway with attending marine inspector.

Vessel's name shall be marked on port/starboarwd bow and stern of the vessel. Hailing port shall be marked on the stern. Currently the name is only on the port bow.

Exhaust is dripping water into the lazarette due to poor alignment/loose seal in vicinity of the muffler. Tighten seal/realign to secure leak.

Complete a credit annual exam. Vessel annual exam is overdue as of 25Jun17
