The certificates and record of equipment should be drawn up in the form corresponding to the models given in the annexes. The Caribbean Cargo Ship [name withheld] indicates that life saving appliances and equipment are provided for a total of 04 persons. PSCO conducted exam and safe manning certificate requires vessel to be manned with 05 personnel. The record of equipment certificate indicates the rescue boat brand is a Zodiac however an AB brand boat is on board. The rescue boat engine is missing from the record of equipment certificate. The record of equipment certificate indicates the vessel is equipped with a Search and Rescue Transponder however no SART is on board. CCSSC 1997 1.14 - 17bc

Life saving appliances required by this chapter should comply with the technical specifications of the LSA Code. Life raft serial no:12041857 length of painter and max stowage height do not match issued servicing certificate issued 03/13/2018. Additionally no first aid kit was notated on the certificate. Life raft serial no:11803206 is missing servicing certificate. Life raft was not serviced within the last 12 months. CCSSC 4.7.1

Emergency fire pump was unable to continuously run during the test. The pump began excessively leaking oil.

Access hatch to steering gear room was covered with cargo preventing access to space in the event of an emergency. Emergency steering could not be used. Vessel removed cargo on the spot and the crew was educated on the safety concerns of not having access in to the space.

Lazarette hatch is missing one out of four dogs. Replace as origional within fourteen days to the satisfaction of the attending marine inspector. until corrected make hourly rounds on the lazarette and record in ships log while underway.
