Gauges to indicate engine revolutions per minute (RPM), jacket water discharge temperature, and lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. No gauges for lube oil pressure or jacket water temperature were visible from the operating station.

Each life jacket kept in a stowage container must be readily available. Stowage container near galley is difficult to open.

Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. If the storage cylinders are separate from the protected space, the instructions must also include a schematic diagram of the system and instructions detailing alternate methods of releasing the extinguishing agent should the local manual release or stop valve controls fail to operate. Each control valve to a distribution line.Required to replace current instruction with step by step instructions to deploy the system and the authority needed to deploy the system. Also, required a schematic diagram at cylinder location.

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. After speaking with Captain, determined that they will have laminated checklists on the bridge to cover these events. The checklists will better mimic what the students will see in the field.

Sight-flow glasses may be used in lubricating-oil systems provided it has been demonstrated, to the satisfaction of the Commanding Officer, Marine Safety Center, that they can withstand exposure to a flame at a temperature of 927 °C (1700 °F) for one hour, without failure or appreciable leakage. The lube oil sight-flow glass is plastic. Required to replace with a heat resistant material.

Ring life buoys shall be of first class workmanship and free from any defects materially affecting their appearance or serviceability. The plug on one of the ring life buoys is cracked. This is an extra ring life buoy; therefore giving a year to replace. Can also just remove the ring life buoy.

General alarm bells shall be marked in not less than 1/2-inch red letters: “General Alarm - When Bell Rings Go to Your Station.” Ensure all general alarms are labeled.

An alarm for a fixed gas fire extinguishing system must be clearly and conspicuously marked “WHEN ALARM SOUNDS-VACATE AT ONCE. CARBON DIOXIDE BEING RELEASED”. Where a different extinguishing agent is installed, that agent shall be marked in place of “carbon dioxide.” Required to label the time delay alarm in the engine room.

Multiple areas of wasted were found in the fuel tank, hull, and bulkhead frames. Repair wasted area to the satification of the Marine Inspector.

Demonstrate MDE shutdowns for low lube oil and engine speed. 46 CFR 58.05-10

01 fire hose damaged/leaking and 01 fire hose not on board. As per 46 CFR 181.15-10, one length of fire hose shall be provided for each fire hydrant. Replace 02 fire hoses.

01 ring buoy waterlight battery dead, rendering waterlight ineffective. As per 46 CFR 180.30-1(b), replace waterlight battery.

Port and starboard exhaust hose missing 2nd hose clamp. As per 46 CFR 182.15-20(c), install hose clamps as necessary.

Ring buoy retro-reflective tape going worn away. As per 46 CFR 160.050-4(f), ring life buoys shall be free from any defects materially affecting their appearance. 46 CFR 180.30-1(a)

Fuel vent, discharge end missing flame screen. As per 46 CFR 182.20-35(d), the discharge end of vent pipes shall be fitted with removable flame screens or flame arrestors. Install flame screen at discharge end.

02 dead end wires and 01 junction box missing cap in forward passenger compartment. As per 46 CFR 183.01-15(a), all electrical installations shall be to the satisfaction of the [name withheld] cap on junction box and place dead end wires into junction box.

Adult and child lifejackets not stored separately. 46 CFR 180.25-10(c) says life preservers for children, when provided, shall be stowed separately. Stow separatly.

Aft light penetrating hull transom above waterline not watertighet. As per 46 CFR 176.25-5 hull penetrations must be made watertight. Make aft light penetration watertight.

COI expired 07JAN16. Vessel opperated approximately 17 times since expiration. New COI will be issued when No Sail 835 for the Bilge system is cleared. Vessel is restricted to the pier, Investigators have been notified.

Fresh Water Pump was found to be wired with no tuffing tube and to be grounded to the bulkhead of the E/R. This is unlike any other pump in the E/R. Vessel must prove that this is proper installation to attending Marine Inspector. 46CFR183.200

On aft E/R bulkhead there is wiring connections in between battery banks, terminals for these connection were found to be freyed and need to be renewed. 46CFR183.200(b)

Bilge Forward of E/R could not take suction fom spaces.

Make proper temporary repair to existing bilge piping located at forward bulkhead in rudder room per 46 CFR 182. 510.

Remove dead end wire in the wheel house.

Make operable remote fuel shutoff.

Update compass deviation table as per good marine practice. The last time the deviation table has been updated is 1998.

Two Portable extinguishers need to be serviced.

Expired flares removed from vessel. Remaining flares met the requirements of the regulations.

No emergency broadcast placard was present on the bridge.

The vessel does not have a fixed public address system installed.
