TWIN TUBE

Before the vessel leaves port and at all times during the voyage each lifesaving appliance must be in working order and ready for immediate use. During the COI inspection the vessel's life float was not in serviceable condition. Vessel's service as a cargo vessel requires the installation of a life raft for primary lifesaving.

Each lifebuoy must be marked in block capital letters with the name of the vessel and the name of the port. During the hull walk, the aft ring buoy was discovered missing the name of the vessel and the home port of the vessel.

Vessel's hull has a hole on the starboard side in way of the 3rd rib frame forward of the cargo tube space bulkhead. Submit repair proposal acceptable to the Coast Guard and complete repairs prior to conducting cargo operations.

Vessel's main propulsion engine transmission is leaking from it's inner seal causing accumulation oil in the vessel's bilge. Renew transmission seal.

Vessel navigational charts are currently not up do date. Ensure updated paper charts are provided at minimum within 6 months of USCG inspection.

Vessel shall provide proof of safety radio certificate.

Vessel Master shall provide medical certificate in conjunction with his license.

Based on the results of the stability test, information shall be prepared by the owners, approved by the Commandant, and furnished to the master which sets forth the stability data necessary to permit efficient handling of the vessel. Where any alterations are made to a vessel so as to materially affect the stability information supplied to the master, amended stability information shall be provided. If necessary the vessel shall have a new stability test. During inspection, the attending marine inspectors observed no stability letter or stability book on board the vessel. Vessel representative was unable to provide any further stability calculations. Additionally, the steel cargo boom had been removed several years ago without any US Coast Guard oversight, which may or may not have affected the stability of the vessel. To determine the current stability, the vessel shall undergo a new stability test.

1. Properly repair buoyant apparatus.

2. Replace damaged float light.

3. Submit plan on how to properly remove "helpless" man overboard.

Vessel's low lube oil alarm and shutdown not functioning properly. Vessel is to make all repairs and demonstrate proper operation to the satisfaction of the attending MI.

Vessel does not have USCG approved first aid kit onboard.

Make proper repairs to the life float as per manufacture's instructions (Repair gel coat over fiberglass to prevent water intrusion).

Re-stencil vessels name on the life float.

Update navrules publication.

Vessel missing coast pilot, light list, current and tide tables, NTM for area. Provide all required pubs.

BII extinguisher in storage room requires recharge. recharge or replace.

Stern light lens is pitted, worn, and fogged due to age. Replace or prove proper intensity IAW COLREGS Inland Nav Rules.

Port boom vang line shows ligns of deterioration. Replace.

Install guard on cargo boom engine IWO radiator fan.

Remove propane bottles from pilot house and properly store.

F/O supply and rturn hose from 40gal tank are old, dry, and cracked. Replace.

Main engine remote shut down cable is tapped together. Provide proper cable clamps.

Provide rose box strainer for engine room bilge suction.

Properly insulate non-jacketed exhaust riser in engine room.

Replace retro on lifefloat

Install weak link for 6 person Lifefloat.

Install lagging to E/R MDE exhaust.

Repair fuel shutoff piping bracket.

Install machinery guard IWO MDE (front).

Make proper repairs or replace deck hatch forward of crane.

Prove proper operation of dog for deck hatch forward of crane.

Provide aspirin/eye wash solution with valid expiration dates.

Label bilge/fire system valves.

Post operating instructions for bilge/fire systems.

Make bulkhead's and overhead in E/R weather (gas) tight

Install gasket to E/R hatch.

Flush out bilge system O/B discharge piping.

Prior to getting underway install temporary discharge line.

Provide means to properly secure O/B discharge valve and prevent tampering with. (Chain/padlock)

Install cover to battery box in E/R.

Install cover to battery located port side of crane.

Double clamp flex hose on raw water intake.

Install closures for E/R vents.

Provide operating instructions for CO2 system.

Install machinery guard to aft end of MDE IWO alternator belt.

Make permanent repairs to leaking fire main system intake piping.

Secure paddles to lifefloat.

Conduct fire drill to the satisfaction of attending Marine Inspector.

Conduct MOB drill to the satisfaction of attending Marine Inspector

Vsl restricted from getting underway until valid servicing certificates for all fire extinguisher systems O/B is provided to the NYCMI (OCMI) and inspected to the satisfaction of attending Marine Inspector.

Vsl restricted from getting underway until all lifesaving equipment is serviceable to the satisfaction of attending Marine Inspector.

Provide proof of servicing of all vsls fire extinguishers.

Replace / repair main engine water pump as necessary.

Prove proper operation of bilge pumping system; install missing pipe and suction strainers. Vessel not to operate until completed.

Re-mount lube oil guage / clean face of gauge for deck boom engine.

HAND BILGE/FIRE PUMP DOES NOT OPERATE. REPLACE WITH WORKING PUMP CAPABLE OF5 GPM WITH PROPER HOSES, FITTINGS ON BOTH SUCTION AND DISCHARGE SIDES.

FIXED CO2 CYLINDERS NEED TO BE SERVICED FOR ANNUAL INSPECTION AND SERVICINGDOCUMENTATION TO BE PROVIDED AFTER COMPLETION.

BILGE SUCTION LINE IN FWD HOLD TERMINATED APPROX. 2' ABOVE BILGE. PIPE NEEDS TO BE EXTENDED DOWN INTO BILGE.

DECK SUPPORT BEAM IN #2 HOLD SUPPORTING CARGO HANDLING GEAR ON DECK ENDS APPROX. 18" ABOVE KEEL. BEAM NEEDS TO BE EXTENDED DOWN TO KEEL AND WELDED IN PLACE.

BILGE SUCTION PIPING IN #2 HOLD IS LOOSE. PIPING NEEDS TO BE SECURELY FASTENED TO AVOID EXCESSIVE VIBRATION.

EXAUST VENT ENDS APPROX. 5' ABOVE BILGES.VENT NEEDS TO BE EXTENDED INTO BILGE.

PROVIDE MAGNETIC COMPASS AND DEVIATION TABLE.

MAKE PERMANENT REPAIRS TO HOLES IN FORWARD BULKHEAD IN ENGINEROOM (2 WHERE RESERVE FUEL TANK PIPING WAS REMOVED, ONE IN OVERHEAD WHERE MANUAL CLUTCH CONTROL WAS REMOVED).

PROVIDE 12 HAND RED DISTRESS SIGNALS.
