As appropriate for the intended voyage, a vessel must carry adequate and up-to-date publications, vessel did not have a COLREGS on board. Provide a COLREGS.

Vessels operating on an Oceans Route must have an interconnected fire detection system in compliance with 46 cfr 181.450.

The number of penetrations in a watertight bulkhead must be minimized. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. Watertight penetrations leading from the machinery space, in the vicinity of the steering pumps to the engine room were not watertight.

The materials and construction of fuel lines, including pie, tube, and hose must comply with the requirements of this paragraph. Fuel piping was deteriorated in the vicinity of the emergency escape from the engine room.

Vessels shall have a fire hose for each hydrant. Port side fire hose had several leaks throughout. Hose was condemned in front of inspectors. Hose was replaced prior the completion of the Inspection.

Vessel on an ocean service route shall have sufficient lifefloats for intended service. Lifefloats had not been properly maintained. 3 lifefloats did not have adequate retro-reflective tape and another had a broken floating waterlight. *Vessel is limited to day time operations until waterlights have been replaced. *Vessels passenger count is restricted to 49 until all reflective tape has been replaced.

Vessels shall be fitted with ground tackle sufficient for the vessels service. Anchor and hawsers where not installed and/or ready to be released.

Vessel shall not be in a condition where the vessel is considered unsafe. Galley Distribution panel was missing a blank in the panel make the panel a potential shock hazard.

Vessel shall not be in a condition where the vessel is considered unsafe. Electric panel in the pilot house indicated a H2S detector was off. There is not detector onboard. Installed wires need to be blanked off and housed or a blank needs to be installed in the panel.

Vessel shall not be in a condition where the vessel is considered unsafe. Engine Exhausts were missing sufficient insulation. Install more insulation at the discharge point.

Discharge ends of vent pipes shall be fitted with removable flame screens or flame arrester which shall consist of a single screen or at least 30 x 30 mesh. Install flame screen at both fuel tank vent openings.

Shutoff valves, installed so as to close against the fuel flow, shall be fitted in the fuel supply lines one at the tank connection and one at the engine end of the fuel line to stop fuel flow when servicing accessories. The shutoff valve at the tank shall be operable from outside the tank compartment, preferably from an accessible position on the weatherdeck. Both shut off valve lines found with too much slack to effectively close from outside the space. Port valve line found with wasted away wire clamp. 2 valve wheels also found to be wasted. Slack to be removed from lines. Port wire clamp to be replaced. 2 valve wheels to be replaced. 46 CFR 182.20-40(b)(3)

Protective gratings shall be provided at such locations where persons or gear might come in contact with the exhaust pipe. Exhaust pipe lagging securing device found to be loose. Lagging to be re-wired. 46 CFR 182.15-20(g)

The material specification shall show that the rigid nonmetallic material possesses characteristics adequate for its intended service and environment and shall be approved for use by the Officer in Charge, Marine Inspection. Flexible non-metallic joint on stbd outboard M/E found to be crushed between both connection points. Flexible non-metallic joint to be replaced with material suitable for gap distance. 46 CFR 182.40-1(a)(4)

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Port M/E crankcase pressure gauge missing on cover, resulting in an electrical hazard. Opening in panel to be covered. 46 CFR 183.01-15(a)

In general, compliance with the standards established by a recognized classification society will be considered satisfactory evidence of the structural adequacy of a vessel. Broken/fractured welds found as follows: 02 in space fwd of collision bulkhead, 04 found in PAX compartment void, 04 found in fuel tank void. Make satisfactory repairs to welds. Approved extension until scheduled dry-dock. 46 CFR 177.10-1(a)

In general, compliance with the standards established by a recognized classification society will be considered satisfactory evidence of the structural adequacy of a vessel. Petition bulkhead stbd side in PAX compartment void found to be excessively bulging. Bulge to be cropped and renewed. Approved extension until dry-dock. 46 CFR 177.10-1(a)
